MI observed that vessel did not have detection system meeting UL217 and was not interconnected. Install system meeting UL217. Restricted from oceans or coastwise until rectified. 46 CFR 175.110(c)(2), 46 CFR 181.450(a)(1)

"A submersible electric bilge pump may be used as a power bilge pump... provided that: a positive shutoff valve is installed at the hull penetration." MI observed overboard line did not have shutoff valve. Install valve or seacock. 46 CFR 182.520(e)(7)

"A hatch cover must: have securing devices." 03 hatch covers on main deck (02 aft and 01 fwd) missing means of securing. Secure covers.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: ... (h) Operational test of the means provided for pumping bilges; and... Operator was unable to demonstrate function of bilge dewatering system. 46 CFR 176.804(h)

(c) Irrespective of build date, a vessel to which this subchapter applies must meet 46 CFR ... 181.450...if it— (1) Has overnight accommodations for passengers; or (2) Is operating on a Coastwise or Oceans route. IAW 46CFR 181.450, the vessel must have an interconnected detection and alarm system. Vessel is restricted to a Lakes, Bays, and Sounds route until installed and functional to satisfaction of USCG. 46 CFR 175.110(c)

(a) Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; Vessel operator unable to produce evidence of crew member enrollment in Drug and Alcohol Program. 46 CFR 16.230(a)(1)

(a) Alcohol testing. (1) Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the Serious Marine Incident (SMI). (i) The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident... Vessel is certificated for a route that necessitates the presence of alcohol swabs onboard for the required testing to follow a SMI. Alcohol swabs were not onboard during inspection. 46 CFR 4.06-3(a)(1)

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. The employer may establish the EAP as a part of its internal personnel services or the employer may contract with an entity that will provide EAP services to a crewmember. Operator did not have EAP present/posted for employee's. 46 CFR 16.401

(a) Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant... Vessel did not have operational lifejacket lights for 20 persons (17 passengers, 3 crew) IAW with COI. 46 CFR 180.75(a)

(h) Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Numerous lifejackets had insufficient or old retroreflective material present. 46 CFR 185.604(h)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. 1st aid kit that meet the requirements above was not present at the time of inspection. 46 CFR 184.710

The master of the vessel shall ensure that: (a) Each EPIRB... must be tested monthly... No log of monthly EPIRB testing present. 46 CFR 185.728

(a) The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. No log of drills present. 46 CFR 185.524(a)

The master shall conduct sufficient drills... to make sure that all crew members are familiar with their duties... that necessitate...the recovery of persons who have fallen overboard. No log of drills present. 46 CFR 185.520(a)

a) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; Operator did not have req'd charts. Produce up-to-date charts and/or receipt of purchase. 46 CFR 184.420(a)

(4)Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009... The following noncombustible materials may be used ...though not specifically approved under this subpart: (a) Sheet glass, block glass, clay, ceramics, and uncoated fibers. (b) All metals, except magnesium and magnesium alloys. (c) Portland cement, gypsum, and magnesite concretes having aggregates of only sand, gravel, expanded vermiculite, expanded or vesicular slags, diatomaceous silica, perlite, or pumice. (d) Woven, knitted or needle punched glass fabric containing no additives other than lubricants not exceeding 2.5 percent. Engine room ceiling boundary not intact due to wasted plate. Propose suitable replacement material to USCG. 46 CFR 177.410(c)(4); 46 CFR 164.009-3

"Your vessel must undergo an annual inspection w/in 3 months before or after each anniversary date." Vessel missed its annual inspection date and must apply for a new COI.

Employers must maintain records of chemical tests and must make these records available to Coast Guard officials upon request. DAPI records not onboard.

Abandon ship, man overboard drills, fire drills, and training shall be logged or otherwise documented for review by the Coast Guard upon request. Record of drills not available onboard.

The owner or operator must provide satisfactory evidence of the required fire protection equipment servicing to the marine inspector. Fire protection equipment servicing records not available onboard.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for certificates to be issued by the FCC. FFC Radio Safety Certificate dated as expired. If vessel intends to maintain ocean route MF radio must be certified

An adult life jacket must be provided for each person. Vessel restricted to carrying no more than 72 passengers. Only 72 adult life jackets found onboard.

A pre-engineered fire extinguishing system must automatically shutdown all engines that draw intake air from within the space. The generator engine did not shutdown during test of the preengineered system. 46CFR181.420(a)(3)

A vessel must carry at least one floating water light. The floating waterlight for the ring buoy was inoperable. 46CFR180.70(d)

Each lifefloat must be fitted with a floating waterlight. Floating waterlights were inoperable. 46CFR180.175(d)

all vessels shall have navigational lights in compliance with the international and inland navigational rules. The port side nav. light was inoperable. 46CFR183.420

vessels shall carry up to date nav. charts. The vessel's electronic charts were from Nov. 2006 w/out updates. 46CFR184.420(a)(1)

Vessel failed testing of its bilge pump. Provide proof of operation of bilge pump.

Vessel did not have current/updated charts. Provide current updated navigational charts for area of operation.

Markings on life floats have faded. Replace marking on life floats.

04 paddles for 02 life floats are splintering and are in non serviceable condition. Replace 04 paddles for life floats.

The grab lines on the ring buoy had deterioated. Replace/repair grab lines for ring buoy.

Provide service record for fixed fire extinguishing system.

Provide DAPI enrollment and annual drug testing report.

Provide proof of compliance of meeting requirements for alcohol testing within 2 hours of a SMI.

Replace/repair life jackets (adult and child) missing lights and retro reflective tape. Vessel restricted from operating in waters more than 20nm fromm shore until vessel meets the requirements of life saving equipment for Oceans and Coastwise routes.

LH01: An enclosed or partially enclosed space within a vessel must be adequately ventilated. Repair power ventilation in lazarette space.

LH02: Install gasket around opening/covers to lazarette.

LH03: Provide proof of first-aid training.

LH04: Life jacket placarding must reflect number of life jackets stowed in placarded space.

LH05: Readily available/mounted in the vicinity provisions must be made for closing all supply ducts to diesel machinery space. Engine ventilation ports must have covers.

LH06: Install junction box for wire connections in engine room and lazarette space.

Ring buoy light must be clipped to the ring buoy lanyard.

Provide unexpired first aid kit contents, approved under 46 CFR 160.041 or with equivalent contents.

Provide EAP for all crewmembers.

Provide current crew list enrolled in DAPI.

Provide 2017 Light List publication.

Prove proper operation of fire pump.

Provide + mount unexpired Hydrostatic Release Unit for EPIRB. Restricted to <3NM from shore.

Program MMSI number in VHF radio.

Identify cause of engine loss. Repair/replace and provide proper operation of port MDE.

Replace intake hose on STB MDE.

AF01- Replace batery in EPIRB. Vessel restricted to >3 miles.

AF02- Obtain current copy of all employees enrolled in Drug Program.
