The cindition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed oil accumilation at lube oil cooler.

An engineers' alarm shall be provided to be operated from the engine control room or at the manoeuvring platform as appropriate, and shall be clearly audible in the engineers' accomodation. PSCO oberved that the audible alarm was turned off.

ALL ACCESS OPENINGS IN BULKHEADS AT ENDS OF ENCLOSED SUPERSTRUCTURES SHALL BE FITTED WITH DOORS OF STEEL OR OTHER EQUIVALENT MATERIAL, PERMANENTLY AND STRONGLY ATTACHED TO THE BULKHEAD, AND FRAMED, STIFFENED AND FITTED SO THAT THE WHOLE STRUCTURE IS OF EQUIVALENT STRENGTH TO THE UNPIERCED BULKHEAD AND WEATHERTIGHT WHEN CLOSED. THE MEANS FOR SECURING THESE DOORS WEATHERTIGHT SHALL CONSIST OF GASKETS AND CLAMPING DEVICES OR OTHER EQUIVALENT MEANS AND SHALL BE PERMANENTLY ATTACHED TO THE BULKHEAD OR TO THE DOORS THEMSELVES, AND THE DOORS SHALL BE SO ARRANGED THAT THEY CAN BE OPERATED FROM BOTH SIDES OF THE BULKHEAD. PSCO OBSERVED LIGHT BETWEEN GASKET AND BRIDGE WING SLIDING DOORS. ICLL Reg 12(1)

EVERY SHIP OF 100GT AND ABOVE SHALL CARRY A GARBAGE MANAGEMENT PLAN WHCIH THE CREW SHALL FOLLOW. PSCO OBSERVED A SIGNIFICANT AMOUNT OF GARBAGE ONBOARD NOT PROPERLY LABELED OR SEGREGATED IN ACCORDANCE WITH THE SHIPS GARBAGE MANAGEMENT PLAN. MARPOL (V) 10.2

INDICATORS SHALL BE PROVIDED FOR ALL SLIDING DOORS AND FOR HINGED DOORS IN WATERTIGHT BULKHEADS INDICATION SHOWING WHETHER THE DOORS ARE OPEN OR CLOSED SHALL BE GIVEN ON THE NAVIGATION BRIDGE. PSCO OBSERVED BURNT LIGHTBULBS ON WATERTIGHT BULKHEAD INDICATOR PANEL FOR BOTH OPEN AND CLOSED POSITIONS ON BRIDGE. 74SOLAS(01) II-1/23-1.2

THE ARRANGEMENTS FOR THE STOWAGE, DISTRIBUTION, AND UTILIZATION OF OTHER FLAMMABLE OILS EMPLOYED UNDER PRESSURE... SHALL BE SUCH AS TO ENSURE THE SAFETY OF THE SHIP AND PERSONS ON BOARD. PSCO OBSERVED WET FUEL OIL ON F.O. PURIFIER #3, FRONT OF A/E #2, AND ACTIVE FUEL LEAKS ON THE F.O. INJECTORS ON A/E #2 74SOLAS(01)II-1/15.4

PSCO OBSERVED CREW NOT FOLLOWING LOCK OUT/TAG OUT PROCEDURES OR MAINTAINING LOG. ADDITIONALLY, PSCO OBSERVED CREWMEMBER WORKING ALOFT WHILE INSERTING FPD FOR LIFEBOAT, APPROXIMATELY 5-6 METERS ABOVE DECK, WITHOUT A HARNESS PER SMS PROCEDURES. FINALLY, PSCO OBSERVED CREW NOT PERFORMING ROUTINE MAINTENENCE AS EVIDENCED BY OIL LEAKS/CONDITION OF ENGINE ROOM AND CONDITION OF LIFERAFTS. INTERNAL ISM AUDIT REQUIRED 90 DAYS. 74SOLAS(20) IX/3.1 ISM CODE 10.1

THE CARRIER, ITS AGENTS, AND ANY PERSON DESIGNATED FOR THIS PURPOSE BY THE CARRIER OR ITS AGENTS MUST PREPARE A DANGEROUS CARGO MANIFEST. IT MUST CONTAIN THE FOLLOWING INFORMATION: STOWAGE LOCATION OF HAZARDOUS MATERIAL ON BOARD. PSCO OBSERVED NO STOWAGE LOCATIONS LISTED ON THE DANGEROUS CARGO MANIFEST FOR ANY HAZARDOUS MATERIAL ON BOARD. 49CFR176.30(a)(7)

Terminations and joints in all conductors shall be so made as to retain the original electrical, mechanical, fire/retarding, and where necessary, fire-resisting properties of the cable. PSCO found numerous deck lights with failed enclosure connections. 74 SOLAS(04)45.5.6

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with additional requirements which may be established by the company. Due to the deficiencies of a technical and/or operational nature identified below which individually or collectively provides objective evidence of a serious failure, or lack of effectiveness of the ISM code. An external audit shall be conducted by the Flag Administration before vessel will be release from its detention to determine whether vessel is operating in compliance with the ISM code. A copy of the completed external audit shall be provided for USCG review prior to departure from the port of Mobile. 74 SOLAS (20)IX/3.1 ISM Code Part A/10.1

The vessel’s safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. Captain was unable to produce objective evidence that previously identified non-conformities were investigated and analyzed with the objective of improving safety and pollution prevention on board the M/V GSL ARCADIA. 74 SOLAS (20) IX/3.1 ISM Code Part A/9.1

The company should establish procedures for the implementation of corrective action, including measures intended to prevent recurrence. PSCO observed multiple deficiencies recorded on the vessel’s SMS required quarterly reports that remain outstanding for more than one year without implementing correction action and communicating measures to prevent recurrence. 74 SOLAS (20) IX/3.1 ISM Code Part A/9.2

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO observed main propulsion engine cylinder fuel oil leaks. Flag State surveyor also observed small oil leak on #1 auxiliary generator prime mover. Vessel SMS quarterly report indicates oil leaks was reported and remains outstanding since 12APR2022. 74 SOLAS (01) II-2/15.2

An embarkation ladder complying with the requirements of paragraph 6.1.6 of the code extending, in a single length, from the deck to the waterline in the lightest seagoing condition under all conditions of trim of up to 10° and a list of up to 20° either way shall be provided at each embarkation station or at every two adjacent embarkation stations for survival craft launched down the side of the ship. PSCO observed the lack of the embarkation ladder on the starboard side of the vessel making it impractical for the life raft required by 74 SOLAS (20) III/31.1.4 to be in a position ready for use. This issue made the liferaft unable to be launched and embarked within 5 minutes. 74 SOLAS (20) III/11.7

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Flag state surveyor identified two cracks in way of port aft quarter cleat of starboard side life boat, company proactively scheduled life raft technician prior to USCG arrival. 74 SOLAS (20) III/20.2

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment other than a space permanently appropriated for the carriage of fresh water, water ballast, oil fuel or liquid cargo and for which other efficient means of pumping are provided, under all practical conditions. PSCO observed a temporary repair to bilge piping in the engine room that is compromised causing excess water in the engine room bilges. 74 SOLAS (04) II-1/26.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. PSCO observed exhaust leak and jacket water leak on #2 generator. 74 SOLAS (04) II-1/26.1

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed steam emitting through lagging on lube oil purifier piping in the main machinery space. 74 SOLAS (04) II-1/33.1

In addition to the requirements elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society which is recognized by the Administration in accordance with the provisions of SOLAS 74 (04) regulation XI/1, or with applicable national standards of the Administration which provide and equivalent level of safety. PSCO observed inoperable bilge system high level audible and visual alarm in shaft alley. 74 SOLAS (04) II-1/3-1

Oil fuel lines shall be screened for otherwise suitably protected to avoid, as far as practicable, oil spray or oil leakages onto hot surfaces, into machinery air intakes, or other sources of ignition. PSCO observed deteriorated lagging on various sections of oil system piping, located in way of port side engine room aft of the oily water separator, overhead near the HFO service tank, and the port passageway to the steering gear space. 74 SOLAS (01) II-2/15.2.11

The means for securing and maintaining weather-tightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions. PSCO observed cargo hold ventilation cover on starboard side bay 59 was deteriorated and unable to close. ICLL Reg 16.4

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. PSCO observed multiple deck lighting extinguished, to include but not limited to: lighting fitted on the bow to illuminate the port side anchor control system, illumination of aft mooring station, and illumination of starboard gangway. Presently, the lighting system does not provide adequate illumination at parts of the ship normally used by crew and utilized under various emergency conditions. 74 SOLAS (04) II-1/40.1.2

Voyage Data Recorder (VDR) must have a copy of the certificate of compliance issued by the testing facility, stating the date of compliance and the applicable performance standards and shall be retained on board the ship. Vessel could not produce a copy of the valid Certificate of Compliance for the approved VDR. 74 SOLAS (20) V/18.8

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the water of the United States must follow these practices: Rinse anchors and anchor chains when the anchor is retrieved to remove organism and sediments at their places of origin. PSCO observed accumulation of sediment on both of the vessel’s anchors. 33 CFR 151.2050(e)

74SOLAS(04)Chapter II-1/43.3.1.2 code: 17AC Where the emergency source of electrical power is a generator it shall be started automatically upon failure of the main source of electrical power and shall be automatically connected to the emergency switchbooard. The emergency generator does not connect automatically to the emergency switchboard after failure of the main source of electrical power.
