The arrangements for the storage, distribution and utilization of oil used shall be such as to ensure the safety of the ship and persons onboard. PSCO observed three remote quick-closing valves did not operate upon activation. 74 SOLAS 01 II-2/15.2

Stairway enclosures shall have direct access to the corridors and be of a sufficient area to prevent congestion, having in view the number of persons likely to use them in an emergency. Within the perimeter of such stairway enclosures, only public toilets. lockers of non-combustible material providing storage for safety equipment and open information counters are permitted. Only public areas, corridors, public toilets. special category spaces, other escape stairways required by regulation 28.1.5 and external areas are permitted to have direct access to these stairway enclosures. PSCO observed unaffixed table and chairs in aft stairwell on deck four and table with alcohol on deck seven. 74 SOLAS 01 II-2/29.2

Doorways and corridors and intermediate landings included in the means of escape shall be sized in the same manner as stairways. Shall not be less than 900 mm in clear width, the minimum clear width shall be increased by 10 mm for every one person provided for in excess of 90 persons. PSCO observed escape route obstructed by piano in theatre. 74 SOLAS 01 II-2/28.1.5.3

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self-closing and be capable of closing against an angle of inclination of up to 35 degrees opposing closure. PSCO observed fire doors A60 6.205 and A0 9.254 did not self-close. 74 SOLAS 01 II-2/30.4.1

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, other than power operated watertight doors and those which are normally locked, shall satisfy the following requirement: the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement with the ship in the upright position. The approximate uniform rate of closure for sliding fire doors shall be of no more that 0.2 m/s and lo less than 0.1 m/s with the ship in the upright position. PSCO observed fire doors A60 9.258, A0 9.254, A04.137, A01.1, and the door from ECR to ER did not self-close within allotted time frame. 74 SOLAS 01 II-2/30.4.2

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. PSCO observed starboard sides of lifeboats 5 and 6 being damaged with exposed FRP and evident delamination. Maintenance records do not reflect proper repair work completed. 74 SOLAS 20 III/20.3.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Crew member was unfamiliar with who the ship’s security officer was, and the procedures to following for a security incident. 74 SOLAS 20 III/19.2.1 (16c)

Openings shall be provided with permanently attached means of closing which shall be at least as effective for resisting fires as the divisions in which they are fitted. FSD A0 4.120 was found to not fully close and latch. 74 SOLAS 01 II-2/30.1 (10c)

Fireman’s outfit shall consist of personal equipment comprising protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water-resistant. Fireman’s flash gear found with hole in it. 74 SOLAS 01 II-2/17.1.1.1 (10c)

Only public toilets, lifts, lockers of non-combustible materials providing storage for safety equipment and open information counters may be located within the stairway enclosure boundaries. Nonafixed trashcans were located on ever deck of the cat 2 stairway. 74 SOLAS 01 II-2/41-2.4.4 (10c)

The minimum integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.2. Foud 03 holes, deck thinning and excessive scale in an A)0 deck in the A/C Space (category 10). The A/C space is above the purifier room and evaporator room (category 12) 74SOLAS(01) II-2/26.1

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall be resistance to fires as well as to the passage of smoke and flam. Daylight can be seen trough EDG room door gasket. 74SOLAS(01) II-2/30.2

The markings must enable passengers to idnetify all of the routes of escape and readily identify the escape exits. Escape exits in Panorama Lounge and Observation Deck were not marked. 74SOLAS(01) II-2/28

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. A sprinkler head and its ampule located in a crew space corridor (adjacent to FSD5.162) had been painter over.

Category 13 spaces are those where combustible materials can be used for storage. There was a substantial amount of air filters and cardboard boxes found within an A/C space and several other spaces (linen stores, paint locker, and boson stores) with combustible materials stored outside of the sprinkler system nozzle range.

Fire doors in "A" class divisions shall be self-closing. Fire screen door 5.162 would not self close after testing.

Openings in "A" class divisions shall be provided with permanently attached means of closing which shall be at least as effective for resisting fires as the divisions in which they are fitted. Sliding fire screen doors 8.245 would not latch all the way and left a small gap at the closing point, door 7.227 would take too long to close (55 sec.) and door 5.477's safety bar would not work thus not reopening if an object was in its path.

Means of escape shall be marked by lighting or photoluminescent strip. There was a small section of low lighting of a about 3 feet on deck 9, at the boundary between MVZ 1 and 2 that was not illuminating.

Detectors shall be operated by heat, smoke or other products of combustion, flame or any combination of these factors. All detectors shall be of a type such that they can be tested for correct operation and restored to normal surveillance without the renewal of any component. PSCO observed no smoke detectors of the consillium type activate when tested. 74SOLAS(01) II-2/13.1.11 17(a)(c)

The construction of all doors and door frames in "A" class divisions with the means of securing when closed, shall provide resistance to fire as well as the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. PSCO observed fire door 4.143 in the main galley is warped and doesn't provide A60 boundary. 74SOLAS(01) II-2/30.2 50(a)(c)

The minimum fire integrity of all bulkheads and decks shall be as prescribed in table 26.1. PSCO observed a hole cut out for cable penetration in the bulkhead between BOSN stores and a corridor. The penetration was not sealed. 74SLOAS(01) II-2/26.1 10(c)

At least one international shore connection shall be provided. The connection shall be kept aboard the ship with eight washers. PSCO observed the international shore connection was missing the washers. 74SOLAS(01) II-2/19.4 10(c)

Means of Escape shall be marked by lighting or photoluminescent strip. Marking must enable passengers to identify all routes of escape. PSCO observed areas throughout deck 3 that had various equipment stored in front of photoluminescent strip blocking the identification of escape routes. 74 SOLAS (01 cons) II-2/28.1.10

"A" Class divisions are those divisions formed by bulkheads and decks that shall be constructed to be capable of preventing the passage of smoke and flame, PSCO observed that the roller shutter door creating the "A" class division on for the grill on deck 8 did not fully close. 74 SOLAS (01 cons0) II-2/3.3
