Ventilation systems of each of the following groups of spaces should be entirely separated from each other. 1. Machinery spaces, 2. Galley, 3. Cargo spaces, and 4. Accommodation spaces and the control station. The arrangement of each ventilation system should be such that fire and one space should not readily spread to the other spaces. PSCO observe, ventilation system for the galley, accommodation spaces, and navigation bridge are not separate and would allow the passage of smoke from the galley or accommodation spaces to the navigation bridge. Subject to the provisions of three period 16.1.2 and 16.1.3, the fire safety requirements of bulkhead, decks, doors, and stairways should as far as practical and reasonable in the opinion of the administration, comply with the applicable requirements of the 1974 SOLAS convention for such bullheads, doors and stairways. In addition to comply with the specific provisions for fire integrity of bulkhead and decks of cargo ships the minimum fire integrity of bulkheads index shall be as prescribed in the tables 9-5 and 9-6. The approved fire control plan does not identify the fire integrity of the bulkhead separating the galley accommodation spaces and the bridge. The administration shall provide the ship with an appropriate document as evidence of compliance of construction and equipment with the requirements of this regulation. The vessel document of compliance for dangerous goods does not specify that dangerous goods cannot be stowaged above the machinery space or below the quarter deck as indicated by the yellow line painted on the deck and signature, indicating that dangerous goods should not be stored past that line. 74SOLAS(20) II-2/19.4 50bc

The frequency of the test and inspections required by this section must be and for vessels annually or as part of the biannual and mid period inspections. The vessel could not provide records of testing for the fuel transfer pipe system. The vessel is prohibited from conducting bunkering operations in the sector Miami COTP zone until a valid test report is provided. 33CFR160.170(f)(3) 50bc

Every ship sahll be provided with a receiver capable of receiving international NAVTEX service broadcasts if the ship is engaged on voyages in any area in which an international NAVTEX service is provided. PSCOs observed NAVTEX equipment performed successful self test but crew could not produce any records of service broadcasts. 74 SOLAS (20) IV/7.4 Code 50ac

The Oil Record Book, whether as aprt of the ships official logbook or otherwise, shall be in the form specified in appendix III to this annex. PSCOs observed last weekly sounding of waste oil sludge tank listed in item 3.1 of form A of the IOPP certificate was on 11April2025. Soundings were not being logged weekly per ORB part 1 section (C)11. MARPOL I/17.1 Code 10c

All machinery and electrical installations, mechanical and electrical equipment and appliances, boilers and other pressure vessels, associated piping systems, fittings and electrical cables and wiring shall be of a design and construction adequate for the service for which they are intended and shall be so installedand protected as to reduce to a minimum any danger to persons on board.. PSCO received report of main engine malfunction. CCSSC Reg 2.3

The emergency fire pump did not produce a jet of water at least 6 meters from the bridge. The crew spent over 10 minutes resolving the problem before both hoses were able to produce steady jets of water. CCSSC Ch. 3.3.7.2

Doors fitted in machinery space boundaries shall... where such doors are not weathertight or watertight, they shall be self-closing. 02 doors in the engine room are not self-closing and not marked as a fire door of any type. CCSSC Reg. 3.17.5.8 17bc

Ships intended for the carriage of dangerous goods shall comply with the special requirements as specified in Chapter II-2 of 1974 SOLAS convention. Arrangements shall be made to insure immediate availability of a supply of water from the fire main at the required pressure either by permanent pressureization or by remote arrangements for the fire pumps. E-fire pump did not have remote starting arrangements. CCSSC Reg. 3.25.1, SOLAS 74 (2020 Cons) II-2/19.3.1.1 16bc

The International Oil Pollution Prevention Certificate shall be drawn up in the form corresponding to the model given in appendix II to this annex. Vessels IOPP Certificate lists the sludge tank having a capacity of 4.73 cubic meters and the vessels sounding tables show 7.09 cubic meters. -MARPOL ANNEX I Reg.9

1680 - A record shall be kept to the satisfaction of the administration and as required by the radio regulations of all incidents connected with the radio communications services which appear to be important to safety of life at sea. 0725 - Fire Fighting - In every ship, spaces containing fuel oil units, shall be provided with a fixed fire extinguishing system complying with the provisions of the FSS Code Chapter 5. At each location there shall be clear instructions relating to the operation of the system having ragard to the safety of personnel. There are no clear instructions posted for the engine room CO2 system. - CCSS CODE(2015) 3.10.1 FSS CODE 5.2.1.3.3

All securing of such cargoes, cargo units, and cargo transport units, in accordance with the cargo securing manual, shall be completed before the ship leaves the berth. PSCO observed cargo secured by chain to the port and stbd gunwale due to wasted pad eyes on deck. SOLAS 74 (14 Cons.) VI/5.6

03 - 1499 - CCSSC CHAP 2.29.2 THE EMERGENCY SOURCE OF ELECTRICAL POWER SHALL BE LOCATED ABOVE THE UPPERMOST CONTINUOUS DECK AND SHALL BE READILY AVAILABLE FROM THE OPEN DECK. VESSEL DOES NOT HAVE AN EMERGENCY SOURCE OF POWER OUTSIDE OF THE ENGINE ROOM.

03 - 0740 - CCSSC CHAP 3.3.10.1 AN EMERGENCY FIRE PUMP SHALL BE POSITIONED OUTSIDE THE ENGINE ROOM AND PERMANENTLY CONNECTED TO THE FIREMAINS. THE USE OF PORTABLE FIRE PUMPS SHALL BE AVOIDED. SHIP DOES NOT HAVE FIXED EMERGENCY FIRE PUMP.

05 - 0735 - CCSSC CHAP. 3.11.1 ALL SHIPS SHALL BE PROVIDED WITH AT LEAST TWO FIREMAN'S OUTFITS. VESSEL ONLY HAD ONE FIREMAN'S OUTFIT.

0628-Stowage of Liferafts- Every liferaft should be stowed with its painter permanently attached to the ship and with a float-free arrangement. Life raft on starboard bridge wing has painter line tied improperly. CCSSC CH4.9.5; 10c

0650-Lifebouys-Lifebouys should be marked in block capitals of the roman alphabet with the name and port of registry of the ship on which it is carried. Life bouys on starboard side are missing letters. CCSSC CH4.12.5.3; 10c

0936: Main steering gear shall be of adequate strength and sufficient to steer the ship. Jockey bar on starboard side sheared approximately two feet from tiller yoke rudder. PSCO observed dry upper rudder bearing. CCSSC/2.33.3 / 17bc
