All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. Multiple lights in the engine room were found unlit when energized. 46 CFR 183.01-15(a)

An MMC must be retained by the mariner to whom it was issued and, while valid, must be produced to verify qualifications when requested by an authorized official. Licensed master's medical certificate is current but was not able to be produced at time of inspection. 46 CFR 10.203(b)

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles on board were all combustible. 46 CFR 177.405(f)

At all inspections special attention shall be paid to insure that no fire hazards exist. The port generator had a moderate lube oil leak on the lube oil pressure gauge line. 46 CFR 176.25-10(b)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. A container for adult life jackets was not placarded, and a placard for child life jackets was not conspicuously placed for the actual stowage location. 46 CFR 185.604(f)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Sewage overboard valves were not secured. 33 CFR 159.7(c)

Each battery used in an item of lifesaving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. Battery for the floating waterlight was found expired. 46 CFR 185.726(d)

Each survival craft must be stowed so that when the vessel sinks the surival craft floats free and, if inflatable, inflats automatically. Hydrostatic release unit for the starboard IBA was installed upside down and the painter line was not attached to the weak link. 46 CFR 180.130(a)(2)

At all inspections special attention shall be pad to insure that no fire hazards exist. The intake vents on the vessel’s port and starboard generators had accumulated debris and dust. 46 CFR 176.25-10(b)

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Neither painter or HDRU were correctly installed to permit proper deployment.

Each survival craft must be provided with means to prevent shifting. The port IBA’s strap which prevent shifting was not installed.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is install and capable of operating at constant marine load under such requirements without exceeding its design limitations. The starboard generator has a leak (approx. 1 drip per minute) which is splashing onto electrical wires. 46CFR182.05-1

Battery connections must be made to battery terminals with permanent type connectors. The port battery has a connection which is not permanent and can easily be lifted off the terminal without tools.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a laz and (2) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. No visual or audible alarms were operable in the pilot house for the forepeak and lazarette spaces.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The EPIRB’s registration is expired.

The master of the vessel shall ensure that each EPIRB is tested monthly, using the integrated text circuit and output indicator, to determine that it is operative. The EPIRB test shall be logged or otherwise documented as applicable. The operator was unfamiliar with how to perform the tests and unable to provide evidence of a test log history.

At each initial and subsequent inspection for certification, the provisions of 176.25-30 shall apply. Main propulsion boilers, auxiliary boilers shall be given special consideration and shall be subjected to such tests and inspections as deemed necessary and specified by the officer in charge, marine inspection, to ensure that they are in safe condition. Periodic tests and examinations shall be as described in part 61 subchapter F of this chapter, except heating boilers as defined in part 53 of subchapter F of this chapter shall be tested or examined every three years. The vessel has a heating boiler with an unknown inspection history. The boiler's electronic components also appear to be an electrical hazard. The vessel shall lock and tag-out this system until a satisfactory inspection of the system by the MI is complete. 46CFR176.25-32

A vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. The current charts are void of updates and were printed in 2008.

Each ungrounded system must be provided with a suitably sensitive ground detection system located at the respective switchboard that provides continuous indication of circuit status to ground with a provision to momentarily remove the indicating device from the reference ground. The vessel’s ground detection shows voltage and no change to the voltage detected when engaged. Vessel shall determine if the problem is with the detection system or detected voltage.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: date of the drill and training; and general description of the drill scenario and training topics. The vessel has no records of training performed in 2022 or 2023.

Hatch covers must have securing devices. The lazarette’s hatch cover lacks functional securing devices and is using bungee chords from the inside.

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: date of the drill and training; and general description of the drill scenario and training topics. The vessel has no records of training performed in 2022 or 2023.

No vessel subject to inspection under 46 U.S.C. 3301 will be navigated unless it is under the direction and control of an individual who holds an appropriate license or officer endorsement on his or her MMC. The master was unable to provide a copy of his medical certificate required by 46 CFR 10.301.

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of 46 CFR shall comply with the requirements of § 4.06 in subchapter A of 46 CFR. The vessel’s alcohol swabs expired in 2022 and it is not reasonable that they would be able to return within the 2 hr requirements for testing ashore.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. The waterlight has no lanyard attaching it to the ring buoy. Vessel is limited to daylight hours until the waterlight is installed with a lanyard.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The ring buoy behind the operating station lacks the vessel's name.

At least one ring life buoy must be fitted with a lifeline of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The installed lifeline is not made of dark synthetic or resistant to UV light.

Electrical equipment exposed to the weather must be watertight. The shore connection is missing a collar preventing it from being watertight.

A lighting fixture must be permanently installed. The starboard light fixture aft of the operating station is loose.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The shore connection’s grounding wire is broken creating unsafe connection.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. The main deck has a light with a missing guard.

Electrical equipment must be installed and maintained to minimize accidental personnel contact with energized parts. The most aft electrical outlet on the main deck is missing an outlet cover.

A National Pollutant Discharge Elimination System (NPDES) permit issued under 122.28 authorizing a category of discharges under the Clean Water Act within the geographical area. No Vessel General Permit kept onboard. Apply through EPA for VGP and keep onboard at all times. 40CFR122.28, CG-543 Policy Letter 11.01

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] engine room escape exit according to regulation.

A weather-tight door must be provided for each opening located in a deck house or companionway. All weather-tight doors are missing gaskets. Make all doors weather-tight in pilot house.

Electrical equipment used in a location normally exposed to splashing, water wash-down, or other wet conditions within a galley must be drip-proof. Electrical outlet cover missing in galley. Replace and secure new cover.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical outlet cover missing in pilot house. Replace and secure new cover.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Electric charts are not up-to-date. Update charts.

Each ring life buoy must be marked with Type II retroreflective material approved in 46CFR164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Ring Buoy reflective tape missing on (3) upper deck life ring buoys. Renew retroreflective tape and place back into service as intended. 46CFR185.604(i), IMO Res. A.658(16)

Electrical equipment in spaces containing machinery powered by, or fuel tanks for, gasoline or other fuel having a flashpoint of 43.3C (110F) or lower must be explosion-proof or ignition-protected, or be part of an intrinsically safe system. Glass bulb light cover missing around light in aft engine room. Recover to make safe of hazards.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Flashlight in engine room not operational. Renew batteries and satisfactory test.

A vessel must be fitted with ground tackle necessary for the vessel to be safely anchored. The ground tackle provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] swivel and shackle were heavily corroded and was unable to attach anchor to deploy rapidly. Renew swivel and shackle.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a space with a non-water-tight closure, such as a space with a non-watertight hatch on the main deck. No alarm sounded in the pilot house for the forepeak bilge alarm. Correct before sail.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must: Be buoyant; Be of at least 18.3 meters (60 feet) in length; Be non-kinking; Have a diameter of at least 7.9 millimeters (5/16 inch); Have a breaking strength of at least 5 kilonewtons (1,124); and Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Vessel's lifebuoys were not equipped with a lifeline.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. One of the ring life buoys did not have the vessel’s name marked.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. The vessel had a donning placard but it was not posted.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine inspection. One of the fire extinguishers was attached by bungee cords and was not installed in the approved bracket made by the manufacturers.

Connections must be made to battery terminals with permanent type connectors. Vessel’s battery are installed with temporary wing nuts. 46 CFR 183.05-20(c)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date tide tables and current tables. Vessel's does not have an updated tide/current tables.

EPIRB shall have valid NOAA registratio. NOAA registration is expired. Provide current registration.

Portable fire extinguishers must be serviced. Extinguishers not serviced due to no available technicians, due to COVED-19. Service fire extinguishers prior to carrying passengers.

All vessels must have navigation lights that are in compliance with Inland Navigation Rules. Vessel's stern light is inoperable. Vessel restricted to daytime operations.

At each inspection for certification, an MI may require that a MOB drill be held. Vessel requested MOB drill to be held after crew change in June. MOB drill must be conducted in the presence of an [name withheld] 20Jun19 (90-day expiry date from renewal COI--20Mar2017).

Vessels shall be equipped with a suitable number of portable battery lights. Portable battery light at operating station is inoperable. 46CFR184.30-1(a)

Portable fire extinguishers shall be installed to the satisfaction of the [name withheld] fire extinguisher located aft in main deck cabin unsecured.

For items not covered by standards, inspection shall be IAW standards acceptable to the OCMI, as good marine practice. Port and starboard fire hydrant stations are unmarked. 46CFR176-25-1

VESSEL SHALL NOT BE PLACED INTO SERVICE UNTIL A COAST GUARD APPROVED STABILITY LETTER IS ISSUED AND POSTED IN ACCORDANCE WITH 46 CFR 185.12.
