Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. Marine Inspector identified that the supply fuel piping was deteriorating and in unsatisfactory condition. Conduct repairs to the satisfaction of Marine Inspector. (46 CFR 182.700)

Electrical equipment exposed to the weather must be watertight. Marine Inspector identified that the port aft light receptacle on the upper deck was wasted and not watertight. Conduct repairs to the satisfaction of the Marine Inspector. (46 CFR 183.210(b))

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 or be specifically approved by the Commandant. Observed that the coolant return hose on starboard generator was not an authorized type. Replace hose to the satisfaction of the Marine Inspector. (46 CFR 182.720(e))

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Observed that the port life ring waterlight lanyard is deteriorating. Replace the lanyard to the satisfaction of the Marine Inspector. (46 CFR 180.70(d)(2))

The ring life buoy shall be so placed as to be readily accessible. It shall be capable of being cast loose, shall not be permanently secured in any way, and shall have attached to it a line of at least 60 feet in length. Observed that the port life ring lifeline is deteriorating. Replace the lifeline to the satisfaction of the Marine Inspector. (46 CFR 180.30-10(a) [OLD T])

Life floats, together with their oars and paddles, shall be conspicuously marked with the vessel's name. Observed that the vessel's name was not present on the life float paddles. Write the vessel name onto the paddles to the satisfaction of the Marine Inspector. (46 CFR 185.30-5(a) [OLD T])

A vessel must carry either a first-aid kit that meets requirements in 46 CFR 199.175(b)(10) or kit with equivalent contents and instructions. Observed that the kit had expired eye wash treatments. Replace the items to the satisfaction of the Marine Inspector. (46 CFR 184.710)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Observed that the test swabs on board were expired. Replace with swabs to the satisfaction of the Marine Inspector. (46 CFR 4.06-15(a)(1))

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Observed that the Vessel Bridge-to-Bridge Safety Radiotelephony Certificate and Communications Act Safety Radiotelephony Certificate were expired. Update certificates to the satisfaction of the Marine Inspector.

On a vessel of more than 65 feet in length, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The pump outlet must be fitted with pressure gauge. Observed that 01 of the 02 gauges was not displaying accurate psi measurements. Rectify or replace the gauge to the satisfaction of the Marine Inspector. (46 CFR 181.300(b))

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. The vessel has not completed a drydock examination within the last two years. Conduct satisfactory drydock examination prior to the carriage of passengers. 46 CFR 176.600(c)(1)

At each initial and subsequent inspection for certification, the marine inspector shall inspect all lifesaving equipment to determine its serviceability. 46 CFR 176.25-22(a) Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve... MI observed 2 of the vessels buoyant apparatuses had exposed foam, grab line deterioration, retro wastage, and no spacers between the stacked life floats/BA’s. MI also found 3 paddles for the life floats were wasted, improper size, not labeled with vessels name, and one of them wasn’t properly attached to the life float. Make repairs or replace 2 of the buoyant apparatuses, and replace 3 paddles to the satisfaction of the MI.46 CFR 180.175(a)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Vessel had insufficient number of child sized life jackets in the compartment that is dedicated to their storage. Vessel added 4 child life jackets to cabinet on the main deck to meet the requirements of their COI. Create placard or label the compartment with the additional 4 child life jackets for passenger and crew awareness to the satisfaction of the MI. 46 CFR 180.78(a)(5)

Sea rails, with suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. MI found the galley range is missing sea rails. Install sea rails to the satisfaction of the MI. 46 CFR 184.220(e)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. When testing the fire pump MI observed fire pump outlet pressure to be 42 PSI, and the pressure gage to be a significant distance from the pump outlet. Install outlet pressure gage closer to the pump outlet and test fire pump pressure to the satisfaction of the MI. 46 CFR 181.10-1(c)

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Vessel was not logging monthly EPIRB tests. Vessel tested and logged monthly EPIRB during inspection, corrected on the spot. 46 CFR 185.728(a)

Electrical equipment exposed to the weather must be watertight. MI observed exposed wires to a light for the bow thruster station on the port side weather deck. Cover the exposed wires or remove the light installation to the satisfaction of the MI. 46 CFR 183.210(b)

A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. Vessel's public address system is not operable from the operating station.

At each initial and subsequent inspection for certification of a vessel, the owner and managing operator shall be prepared for inspection and operation of all watertight closures in the hull and decks. MI discovered the lazarette hatch is not fully watertight.

At each initial and subsequent inspection for certification of a vessel, the owner and managing operator shall be prepared for inspection of all accessible parts of the interior of the hull. MI discovered water leak into the vessel's interiors stemming from the stbd side ceiling of the second deck of the vessel. Vessel owner shall make repairs to the satisfaction of the cognizant OCMI.

Each ring life buoy must be marked with Type II retro-reflective material approved under subpart 164.018 of this chapter. The vessel's ring buoys were missing retro-reflective tapes.

The name of a vessel must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. Name of vessel is missing from one of the life floats.

At each initial and subsequent inspection for certification of a vessel, the owner and managing operator shall be prepared for inspection of all accessible parts of the exterior of the hull. MI observed that the ladder to access life floats is missing multiple bolts. 46 CFR 176.802(a)(1)

The marine employer must ensure that alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the serious marine incident within two hours. The vessel's alcohol testing strips were out of date. Vessel operator to acquire new testing strips prior to operating the vessel that will require the vessel to be more than 2 hours of an alcohol testing facility.

At each subsequent inspection for certification all vessels shall conduct the following tests: The fire pump shall be tested and the fire hose shall be subjected to the available fire pump pressure. During the operational test of the fire pump, the discharge piping was actively leaking due to a piping failure. Repair piping to the satisfaction of the Marine Inspector prior to the carriage of passengers. 46 CFR 176.25-25 'Old T'

All vessels except non-self propelled vessels shall be provided with suitable steering apparatus. The steel cross member support is wasted, and the fiberglass longitudinal supports are showing signs of delamination. Submit repair proposal to satisfaction of Marine Inspector for work to be completed prior to credit dry dock exam. 46 CFR 182.30-1 'Old T'

Mechanically propelled "L" vessels shall comply with the provision of Subchapter F. Mechanically connected fittings must be designed and constructed IAW ASTM F 1387. Firemain piping was discovered to be replaced with mechanical fittings not designed to ASTM F 1387. Replace piping prior to credit dry dock exam. 46 CFR 182.40-5 'Old T'

Shutoff valves shall be fitted in the fuel supply lines. The shutoff valve at the tank shall be operable from outside the tank compartment. The Port Main Engine F/O shutoff valve is worn. Replace F/O shutoff valve. 46 CFR 182.20-40(3) 'Old-T'

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. The vessel's high water level audible alarm was found to be uninstalled. Operator corrected on the spot. 46 CFR 182.530

Penetration in watertight bulkheads must have means to make them watertight. There 02 penetrations in the engine room that are not watertight. One is at the forward bulkhead at the penetration of the halon discharge. The other is in the aft bulkhead at the fire main penetration. 46 CFR 171.114(a)(2) (Old T)

Watertight bulkheads must not have sluice valves. A sluice valve (gate valve) is installed on aft side of collision bulkhead that is left over from the old bilge piping. Remove and plug within 30 days. Permanent repairs required during next drydock. 46 CFR 171.114(b) (Old T)

Each buoyant apparatus on the vessel on March 11, 1996, may be used to meet the requirements of this part as long as the survival craft is continued in use on the vessel, and is in good and serviceable condition. Multiple lacerations and deterioration found on buoyant apparatuses.

Employers must ensure that they and their crewmembers meet the requirements of this part. Vessel employed deckhand that is not enrolled in vessel's chemical testing program. Provide enrollment in program and compliance with 46 CFR 16.210 (pre-employment testing).

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Linkage rod between rudders has severe corrosion and needs further inspection at next drydock. 46 CFR 182.30-1 (Old T)

For a vessel, the construction of which was begun prior to approval of the plans and information required by paragraphs (a) and (b) of this section, the cognizant OCMI may require any additional plans and information, manufacturers' certifications of construction, testing including reasonable destructive testing, and inspections, which the OCMI determines are necessary to verify that the vessel complies with the requirements of this subchapter. Four of nine upright stanchions missing on aft main deck. Deck above carries passengers, these stanchions may be required to maintain adequate strength of the upper deck.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Strainer missing on forepeak and engine room pick ups. 46 CFR 182.510(b)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Several exposed wires found in space behind pilot house steering console. 46 CFR 183.200(b)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Fire extinguisher located in upper deck bar not properly secured for sea. 46 CFR 181.520

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Wing type connectors are being used on both battery sets behind generators in engine room. 46CFR183.350(d)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Bilge line serving forepeak was found with unapproved repair. Location of the unapproved repair is in the engine room in front of the port engine near the bulkhead. 46 CFR 176.700(a)

Life floats and buoyant apparatus, when stowed in tiers, the separate units must be kept apart by spacers. No spacers currently used for stowage. 46 CFR 180.137(f)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB battery is currently expired. Route reduced to L/B/S until corrected. 46 CFR 180.64

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. Each buoyant apparatus has cracking and evidence of prior ineffective repairs. Repair or renew with life floats. 46 CFR 185.700

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. IBA painter lines were found not connected to weak link. 46 CFR 180.130(a)(2)

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals and six hand orange smoke distress signals. 12 of 12 distress flares expired. Replace expired distress signals.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. Required light for life float inoperable, replace expired battery.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Guard over starboard SSDG present but not covering moving pulleys and not bolted down. 46 CFR 177.35-15(a)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Exposed wiring for bilge system in center void presents hazard; secure wires within a watertight enclosure.

Rigid non-metallic hose in center void needs to be replaced with ferrous material. 46 CFR 182.40-5 (b)

That part of the exhaust system between the point of cooling water injection and the engine manifold shall be water-jacketed or effectively insulated. Placement of protective covers on both SSDGs were not providing effective insulation. 46 CFR 182.15-15

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. Wiring for ice maker is improperly mounted; install in accordance with regulations.

All enclosed spaces within the vessel shall be properly vented or ventilated. Dryer is currently vented into a void causing excessive lint build-up. Ventilate dryer outside of the space. Dryer not to be used underway until corrected. 46 CFR 177.20-1

Grounding wire for common ground in vessel's cathodic protection system (lazarette) found parted and requires replacement/installation IAW 46 CFR 183.01-15(a) (Old T).

Puncture/tear found on lifefloat, which requires repair or replacement IAW 46 CFR 180.01-5 (Old T).

Portable cocktail bar and several catering items were stored in front of local controls for fixed fire extinguishing system; operator removed all items IAW 46 CFR 181.20-15(a) (Old T).

Installed Carpet (nylon) tested to ASTM E-648 and designated as Class 1 which is detailed as having a Critical Radiant Flux (CRF) of not less than .45 watts/cm2. Carpet does not meet applicable standard of CRF of not less than .8 watts/cm2. Vessel may operate prior to the completion of the plan review for interior renovation provided that two, AII foam (2.5gal)or Dry Chem (10lb) portable fire extinguishers are maintained on board in the accommodation space and ensure there will be no open flame on board.
