Found 01 missing dog and multiple frozen dogs on Natural Exhaust Motor RM#2 (1-38-1) fire damper; Found multiple frozen dogs IWO 01-64-2 & 01-67-2 fire dampers; Found missing gasket sections on Discharge Exhaust Vent for the Bosn' Storeroom (02-26-0). Found deteriorated gasket on the Intake Supply vent (02-27-02).

All lifesaving appliances shall- be fitted with retro-reflective material where it will assist in detection and in accordance with the recommendations of the Organization. All 04 lifeboats had deteriorating reflective tape.

General Alarm System shall be audible throughout all accommodation spaces, normal crew working spaces and open deck. Found general alarm bell in cargo pump room loose, therefore not hitting the striker, causing no audible sound.

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers or in compartments assigned principally in paint lockers unless appropriately certified for safe usage in the dusts, vapors or gases likely to be encountered. Found 02 non-extrinsically safe portable fans located in the ships paint locker.

Each cargo tank of a tankship must be equipped with an intrinsically safe high level alarm and a tank overfill alarm. 2C high level alarm is inoperative.

Deck foam system; Systems of this type are designed to give primary protection to the spaces over the cargo tanks. Multiple nozzles are clogged and inoperative. Check valves between zones are inoperative.

Each helicopter landing deck on which fueling operations are conducted must have a fire protection system that discharges foam. Nozzles are clogged or inoperative on helo pad.

Cargo pipeline testing intervals shall coincide with vessels regular dry dock intervals. Cargo pipelines must undergo two static liquid tests with in a 5 yr period, and not more than three years my elapse. Per MSC/USCG MOA dated 02Jul18 Cargo line was last tested in Feb 18. vessel just underwent a dry dock period.
