Tank Deck AFFF system suffered the following casualties: -Spool for Monitor One (01) at frame 33, developed a large hole with 200 gal/min escaping the hard piping system. -during test for Zone Two (02), no water flow was coming from the sprinklers. -fire and ballast pump could not be stopped after activation. Liquid cargo operations are restricted until repairs to the AFFF fire monitor and sprinkler system are made to the satisfaction of Class.

A person may not employ or engage an individual, and an individual may not serve in a position in which it is required by law or reguation that the individual hold an MMC endorsed with a national endorsement, as well as a corresponding endorsment for service outside the boundary line. The mariner filling the chief engineer position has an incorrect propulsion type allowance on their STCW endorsement as Chief Engineer and OICEW.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Mariner in the postion of bosun/AB-Deck was found with expired national medical certificate.

The normal equipment of a fast rescue boat shall include a VHF radiocommunication set which is watertight. Due to flooding damage, VHF on port side rescue boat no longer functional. LSA 5/5.1.4.11

Every lifeboat to be launched by fall or falls, except free-fall lifeboat, shall be fitted with a release mechanism complying with the following requirements: the release mechanism shall be so arranged that all hooks are released simultaneously. During inspection, Marine Inspectors found that lifeboat #1 could not release on-load without a significant delay in release of aft and forward release hooks. Additionally, crew appears to regularly practice bypassing the on-load release procedure during operational tests/drills. 74 SOLAS (20) III/34; LSA 4/4.4.7.6

During preparation and launching, the survival craft, its launching appliance and the area of water into which it is to be launched shall be adequately illuminated by lighting supplied from emergency source of electrical power required by regulation II-1/42 or II-43, as appropriate. Marine Inspector found that all lights that point into water on starboard side, and all but one on port side, for survival craft launching/embarkation/recovery are not operable. 74 SOLAS (97) III/15.7

Any required...fire detection and fire alarm system shall be capable of immediate operation at all times and no action by the crew shall be necessary to set it in operation. Marine Inspector found trouble indication on fire alarm panel and found several fire detectors that were disconnected and hanging from their mounts. 74 SOLAS (81 Amended) II-2/12.1.1

Structural members...and all other fittings used in connection with launching equipment shall be designed with not less than a minimum safety factor on the basis of the maximum working load...a minimum safety factor of 4.5 shall be applied to all davit and winch structural members. Port side rescue boat davit designed as a tilting davit, however due to long term deterioration and painting over of components it has been seized in place. Vessel to secure all pivot points by mechanical fastening/welding to ensure boat does not accidentally shift and prove proper launching utilizing only davit. 74 SOLAS (97) III/ 48.1.8

Personal equipment shall consist of electrical safety lamp of an approved type...Electric safety lamp on tankers and those intended to be used in hazardous areas shall be of an explosion proof type and axe with a handle provided with high-voltage insulation. During fire drill, firefighters were found to be without radios and axes. SOLAS (20) II-2/10.1.1 FSS Code 3.2.1.1

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Wiring for general alarm at aft port side of midships unrep station found to be routed through a sharp edged rat hole and spliced outside of junction box. Make repairs to include proper weather-tight cable transit and make wiring connections inside junction box. 74 SOLAS (04) II-1/45.1.3

Where necessary, relatively tight compartments such as refrigeration spaces, paint lockers, etc., shall be provided with suitable means for relieving excessive pressure accumulating within the compartment when the carbon dioxide is injected. Door to forward cargo store room installed backwards. Does not open outwards from fixed gas system. U.S. Supplement ABS 2003

A general emergency alarm system complying with requirements of regulation 50 shall be provided. General alarm bell on starboard side aft of house found disconnected and missing. 74 SOLAS (97) III/6.4.2

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. A/C unit that services ammunition locker base is wasted. Crew could not determine if unit worked or not. Temperatures in ammunition locker showed evidence of exceeding 100 degrees F. Unit is being held up with lashings. Repair base and install A/C unit to satisfaction of ABS. 74 SOLAS (04) II-1/40.1.3

Guard rails or bulwarks shall be fitted around all exposed decks. The height of the bulwarks or guard rails shall be at least 1m from the deck. Marine Inspectors found damage to rail on aft corner, starboard side, of forecastle. Crew stated that this was damaged from a crane operation at the shipyard. Additionally, damage was done to a blue light on rail just aft of Forecastle. Make repairs to satisfaction of ABS. ICLL (88) II/25(2)

Each nonmetallic transfer hose must: Have no unrepaired loose covers, kinks, bulges, soft spots or any other defect which would permit the discharge of oil or hazardous material through the hose material, and no gouges, cuts or slashes that penetrate the first layer of hose reinforcement as defined in § 156.120(i)...Have no external deterioration and, to the extent internal inspection is possible with both ends of the hose open, no internal deterioration... Hoses not meeting the requirements of paragraph (c)(1)(i) of this section may be acceptable after a static liquid pressure test is successfully completed in the presence of the COTP. The test medium is not required to be water. Marine inspectors found that virtually all cargo transfer hoses showed signed of external deterioration. Hoses shall be subject to additional inspection and testing prior to being returned to service. 33 CFR 156.170

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have on board an emergency means to enable the person in charge of a transfer operation to a facility, to another vessel, or within the vessel to stop the flow of oil or hazardous material. Emergency shutdowns throughout the vessel were found to have unreadable signage, exposed wiring, or broken covers. 33 CFR 155.780(a)

A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe. Marine Inspector found several containment plugs missing. 33 CFR 155.320(a)

HP Pressure tank system on the cargo deck found with multiple painted over and zip tied pressure relieving devices while in service.

Annual Inspection. Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Due to vessel conducting repairs vessel has gone past 3 month limit. Schedule a date to renew prior to movement of vessel.

Un approved modification was made in way of the transverse expansion joint on the 01 level at frame 56.

A self-contained emergency source of electrical power shall be provided. Ship crew reported that EDG shut down after over-speeding, in addition that local and switchboard stop switch are not operating as intended. 74 SOLAS (2004) II-I REG 43.1.1

Means shall be provided to close all openings which may admit air to or allow gas to escape from a protected space. 06-92-1 Engine room air supply damper louvers are inoperable. 74 SOLAS (1997) II-2 Reg 5.1.4

Every ship shall be provided with fire pumps, fire mains, hydrants, and hoses complying with applicable requirements of this regulation. During operational test Fire main expansion joint ruptured IWO main deck P/S ZONE 3.

Title 46, CFR, Part 113.25-12 (b): Each general emergency alarm signal must be an electronically-operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. Repair and prove operation of the following general alarms: 1 x general alarm in way of SWB P4 Tank, port side on tank top, 2 x general alarms in way of fire station no. 61, port side, 1 x general alarm in way of fire station no. 63, port side, 1 x general alarm with inoperable bell in aft truck tunnel, 1 x general alarm with inoperable rotating light in aft truck tunnel.

74 SOLAS (2014) Chapter III, Regulation 20.10: Containers, brackets, racks and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with recommendations of the Organization, indication the devices stowed in that location for that purpose. Replace missing symbols for ring buoys with attached float light at the following locations: 2 x on port side tank top and 1 x focsle near intake no. 02-12-2 Supply vent.

74 SOLAS (2014) Chapter II-2, Regulation 14.2.2.1: Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the Organization and in a manner due regard to ensuring the reliability of fire-fighting systems and appliances. Portable fire extinguishers throughout house and work spaces were inspected with overdue annual service tags as of June 2020. Per vessel’s Captain, 3rd party service technician is scheduled to attend the vessel in Guam before end of July to complete inspection of vessel’s firefighting detection/suppression and portable extinguishers. Submit copy of inspection/service report to Sector Guam.

Oil filtering equipment referred to paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. MARPOL Annex 1/14: In addition, it shall be provided with alarm arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 parts per million. Vessel was unable to demonstrate that the oily water separator and oil content meter is operating as designed.

74 SOLAS (2014) Chapter 1, Regulation 11: The condition of the ship and its equipment shall be maintained to confirm with the provision of the present regulations to ensure that the ship in all respects will remain fit to sea. Repair sound power powered phones at the following locations and prove operation: Focsle and in Motor Rooms 1, 2 and 3.

74 SOLAS (2014) Chapter II-2, Regulation 14.2.2.1: Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the Organization and in a manner due regard to ensuring the reliability of fire-fighting systems and appliances. CO2 flooding system for Motor Rooms 1, 2 and 3 displays incorrect time delay of 20 seconds for exiting spaces. Time delay units are marked with minimum time delay of 60 seconds. Update placards with correct time delays for motor rooms 1, 2 and 3.

ICLL (1966) Annex I, Regulation 8: The ring, lines, letters shall be painted in white or yellow on a dargk ground or in black on a light ground…The marks shall be plainly visible. Paint in contrasting color starboard deck line per vessel's current International Load Line Certificate to the satisfaction of Class.

74 SOLAS (2014) Chapter I, Regulation 11: The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service they are intended. Repair COJP low suction wheel valve with vice-grip on gland bolt to as original.

74 SOLAS(14) II-1/54.4.1: The control system shall be such that the services needed for the operation of the main propulsion machinery and its auxiliaries are ensured through the necessary automatic arrangements. One of the three main engine's salt water stand-by cooling pump could not auto start and change over upon loss of pressure from the primary cooling pump.

Ventilator openings and air pipes shall be provided with weather tight closing appliances. Vessel was found with multiple ventilator closures on main deck and 01 deck missing hardware, painted over, corroded or otherwise incapable of being closed. ILLC Reg 19.4 and Reg 20.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The Port Side rescue boat davit experienced hydraulic hose failure during testing rendering davit INOP and rescue boat unable to be launched or recovered. Solas 74 (14 amend) III/20.1

The machinery, associated piping, and fittings shall be so installed and protected to reduce to a minimum any danger to persons on board. The Emergency Generator Set prime mover was found leaking diesel fuel from fuel hose fitting and filter housing during operational testing. SOLAS 74 (83 amend) II-2 Reg 26.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The #2 lifeboat sliding door was found missing hardware and not able to be properly secured closed. Solas 74 (14 amend) III/20.1

Cables and wiring shall be installed and supported in such a manner as to avoid chaffing and other damage. Overhead wiring adjacent to emergency generator access hatch was found unsupported due to broken clip on framing. Solas 74 (83 amend) II-2/45/5.5

After any survey of the ship has been completed, no change shall be made in the structural integrity, machinery, or equipment covered by the survey without the sanction of the Administration. Addition of cooling fan in steering room needs approved and permanently installed to include proper utilization of junction boxes and wire supports as appropriate.

Provide proper batteries for all onboard water lights. Water lights on bridge wings have a max weight requirement of 1.49 lbs. Installed batteries have weight of 1.95 lbs. None of the water light batteries throughout the vessel are listed on the manufactures recommended battery list.

No lifeboat launching instructions.

Grab lines on 4 Liferings in substandard condition.

Spare extinguishers not properly stowed in foam room.

Number 4 Generator had bad tie to switchboard

#SSDG 3 had a alarm light for ground, vessel crew took the generator off line and secured operation until furhter troubleshooting can be done. Request proper repair, and operatinal test.
