MEDUSA SPAR

Each rescue boat must be capable of being launched with the unit anchored or bearing on the bottom in a current of up to 5 knots. Each rescue boat launching arrangement must permit the rescue boat to be launched in the shortest possible time. It was reported to the USCG by Murphy Oil Corp that the 1100 crane on board Medusa Spar was observed by third party maintenance to have a cracked flex plate/drive plate rendering it out of service. This crane is intended for launching the Fast Rescue Boat and there are no other cranes in service. Facility has contracted a standby vessel to serve the purpose of the fast rescue craft in any case of a man overboard. Standby vessel shall remain with the facility until the crane is repaired and restored to its satisfactory working condition.

Emergency drills shall be conducted by the person in charge (PIC). The drills shall be conducted as if an actual emergency existed. At least once a year, all elements of the Emergency Evacuation Plan (EEP) must be exercised. Facility personnel have not exercised the lowering of the lifeboats in the last year. (33 CFR 146.125).

The emergency equipment provided, regardless of whether or not required, shall be maintained in good condition at all times. Rescue boat found with inflatable collar in disrepair – missing reflective tape, broken grab lines, degraded condition. (33 CFR 146.15).

Murphy’s Medusa lifeboat weekly maintenance checklists require weekly lowering and recovery of both lifeboats. Records and statements from Murphy’s Medusa STS indicate boats are not lowered at weekly intervals. (33 CFR 146.15).

Emergency equipment provided, regardless of whether or not required by 33 CFR subchapter N, shall be maintained in good condition at all times. Fire stations and hose reels were found inoperable during inspections.

Life saving appliances issues were found as follows: Lifeboats have at least 1 each air bottle below minimum pressure. Life saving maintenance program is not at a minimum meeint the manufacturer's instructions for maintencance for lifeboats weekly, monthly, and semi-annual for releasing gear, monthly, weekly, and quarterly maintenance is not as per manufacturer's recommendation.

2008 approval letter requires twice in 5 years ISIP inspections to resume 18 August 2019. Submit matrix change to resume twice in 5 years ISIP Inspection cycle.

Provide SOLAR report upon completion of repairs to both turbine generators to ensure proper operation.

When lifeboat engine was started, exhaust fumes from the engine were leaking through a hole in the exhaust pipe. A temporary patch was installed and tested. No exhaust was detected after repairs were made. Make permanent repairs to exhaust pipe.

Lifeboat's internal air pressure relief valve was seized and did not operate correctly. Additionally the vent window aft door would not seal to allow pressure to build up. Repair to original condition. Relief valve was repaired prior to departure.

A large amount of water was leaking into compartment through a seatbelt bolt hole. Water seemed to be isolated behind the one bolt hole, other bolts were checked with negative results. When lifeboat was hung on lifeboat hooks, the lifeboat did not lean in the direction of the leak nor was damage apparent on hull. Lifeboat must be examined by a third party to ensure the extent of water damage.

The aft door, when closed did not seal properly. Adjust/repair door to original condition.

The maintenence/stop valve on E-gen CO2 system did not appear to be in correct posisition for system to operate. The valve was not labled and facility personnel could not tell what posistion it should be in. Ensure valve is operating correctly, is labled and personnel are trained in system.

Two of the fire extinguishers in service showed low pressure on guage on must be taken out of service and replaced.

According to foam analysis, foam in P06 and P07 was bad. Facility had no documentation showing foam was replaced. Replace foam in the fire stations or provide documentation foam was replaced.

Several control valves throughout facility were difficult to operate and valve handles were severely corroded. Repair/renew valves to original condition.

To meet the requirements 46 CFR 107.231 each crane must meet the following rated load tests at both the maximum and minimum boom angles ususally employed in material transfers over the side of the unit. ABS completed rated load test at a single angle pull not at both the maximum and minimum boom angles. Provide documentation attesting to legitimacy of single pull test. -46 CFR 107.260(a)

In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sets. Vessel had an inoperable gas turbine generator from a lube oil level switch that wouldnt allow the turbine to start. Provide plan of action in the event of failure of one turbine and provide report.

During the testing of the limit switches on lifeboat "B" the contacts welded together causing the lifeboat to be pulled up into the davit. Lifeboat received fiberglass damage to roof, stern hook was pulled .25" out of boat, forward hook was bent, and falls were stretched.
