Each rescue boat must be capable of being launched with the unit anchored or bearing on the bottom in a current of up to 5 knots. Each rescue boat launching arrangement must permit the rescue boat to be launched in the shortest possible time. It was reported to the USCG by Murphy Oil Corp that the 1100 crane on board Medusa Spar was observed by third party maintenance to have a cracked flex plate/drive plate rendering it out of service. This crane is intended for launching the Fast Rescue Boat and there are no other cranes in service. Facility has contracted a standby vessel to serve the purpose of the fast rescue craft in any case of a man overboard. Standby vessel shall remain with the facility until the crane is repaired and restored to its satisfactory working condition.

Emergency drills shall be conducted by the person in charge (PIC). The drills shall be conducted as if an actual emergency existed. At least once a year, all elements of the Emergency Evacuation Plan (EEP) must be exercised. Facility personnel have not exercised the lowering of the lifeboats in the last year. (33 CFR 146.125).

The emergency equipment provided, regardless of whether or not required, shall be maintained in good condition at all times. Rescue boat found with inflatable collar in disrepair – missing reflective tape, broken grab lines, degraded condition. (33 CFR 146.15).

Records and statements from Murphy’s Medusa STS indicate boats are not lowered at weekly intervals. (33 CFR 146.15).

Emergency equipment provided, regardless of whether or not required by 33 CFR subchapter N, shall be maintained in good condition at all times. Fire stations and hose reels were found inoperable during inspections.

Life saving appliances issues were found as follows: Lifeboats have at least 1 each air bottle below minimum pressure. Life saving maintenance program is not at a minimum meeint the manufacturer's instructions for maintencance for lifeboats weekly, monthly, and semi-annual for releasing gear, monthly, weekly, and quarterly maintenance is not as per manufacturer's recommendation.

2008 approval letter requires twice in 5 years ISIP inspections to resume 18 August 2019. Submit matrix change to resume twice in 5 years ISIP Inspection cycle.

Provide SOLAR report upon completion of repairs to both turbine generators to ensure proper operation.
