The marine employer must have a sufficient number of alcohol testing devi es readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol test strips were expired. Provide testing devices for total crew onboard. 46 CFR 4.06-15(a)(1)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of 46 CFR, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Waterlight is not of the approved type. Replace current unauthorized waterlight with correct type.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel's propulsion main engine began overheating while underway, forcing vessel to anchor and shutdown engine. Vessel must diagnose and submit diagnosis and repair to the USCG and perform underway operational test to the satisfaction of an attending Marine Inspector.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Vessel was not able to dewater engine room bilge or mid-ship bilge with installed bilge system. Make bilge system in compliance with regulations and to satisfaction of attending Marine Inspector

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Vessel did not have valid COD on board. Provide proof of valid COD to attending Marine Inspector.

Child size life jackets were stowed with adult life jackets. Containers for life jackets were not correctly marked. Make containers and stowage as required by regulation and attending Marine Inspector.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of 46 CFR. Waterlight had incorrect type battery. Replace battery as per manufacturer requirements.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electronic control panel under helm broken. Repair broken control panel to satisfaction of attending Marine Inspector.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Vessel is only allowed 56 passengers on its upper deck, but is marked for 65. Correct markings as required and to the satisfaction of attending Marine Inspector.

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. The point and uniform loads do not need to be applied simultaneously. Rails on upper weatherdeck were not yet fully installed and required hot work. Repair/replace rails to satisfaction of attending Marine Inspector.
