At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Starboard side's transmission, overboard discharge pipe for raw cooling water was found severely deteriorated. Instead of renewing the pipe, a piece of flexible hose with two clams at each end was installed. Crop and renew damaged section of pipe and test for the satisfaction of an attending USCG Marine Inspector. 46 CFR 115.830 Code 17C.

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Starboard side engine room (CO2 protected space) forward watertight bulkhead piping, penetrating the bulkhead towards the forward void (diesel fuel tanks)was found to have a gap. Properly seal with an approved fire retardant filler material and make water & airtight. 46 CFR 116.415 & 46 CFR 171.114 Code 17C.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. (b) If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. (c) Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Several cables were found not connected (dead-end),and other with twist-on connectors in the following locations: 1. underneath the operating station. 2. portside engine room adjacent to the engine controls in the aft watertight bulkhead. 46 CFR 111.60-17 Code 16C.

Found that port ship generator exhaust lagging was installed incorrectly which left a gap in two areas that exposed the underside of the two boxes to melt, the dripping plastic contacted the hot exhaust piping causing a fire. 2.
