At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Starboard side's transmission, overboard discharge pipe for raw cooling water was found severely deteriorated. Instead of renewing the pipe, a piece of flexible hose with two clams at each end was installed. Crop and renew damaged section of pipe and test for the satisfaction of an attending USCG Marine Inspector. 46 CFR 115.830 Code 17C.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (3) A means to reset devices used to automatically shut down ventilation systems and engines. Starboard side main diesel engine emergency override actuator device failed to operate upon actuation, and engine couldn't be re-started from the local control at the operating station. 46 CFR 118.420 Code 17C.

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Starboard side engine room (CO2 protected space) forward watertight bulkhead piping, penetrating the bulkhead towards the forward void (diesel fuel tanks)was found to have a gap. Properly seal with an approved fire retardant filler material and make water & airtight. 46 CFR 116.415 & 46 CFR 171.114 Code 17C.

Exhaust gas must not leak from the piping or any connections. Exhaust leak was found in stbd side pipe riser, above the engine room access hatch. 46 CFR 119.430 Code 16C

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. (b) If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. (c) Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Several cables were found not connected (dead-end),and other with twist-on connectors in the following locations: 1. underneath the operating station. 2. portside engine room adjacent to the engine controls in the aft watertight bulkhead. 46 CFR 111.60-17 Code 16C.

The selector knob for generator 1 and 2, in the electrical cabinet located at the main deck was missing, and a metallic conducting wrench is being used as a substitute of a non-conducting electrical selector knob. 46 CFR 115.830 code 16C

46 CFR 182.100 This part contains requirements for the design, construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vsl conducted repairs to steering system. Schedule sea trial with SSJ Prevention. Conduct underway test of steering system repairs to the satisfaction of the attending marine inspector.

46 CFR 119.430(b) Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. High levels of CO2 and H2S was discovered in the port engine room causing inspectors 4 gas meters to alarm. Properly correct exhaust leaks the satisfaction of the marine inspector

46 CFR 115.830(b) At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Stbd side engine room, outboard bulkhead has oil leaking behind the engine room insulation from an unknown source. Find source of oil leak and repair to the satisfaction of the attending marine inspector.

46 CFR 117.205 Each vessel operating in warm water may be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. 46 CFR 117.205 Vessel is required in accordance with the Certificate of Inspection to have 03 Survival crafts (inflatable Buoyant Apparatus (IBA) on board, however, currently only have 02 installed with a total capacity for 100 Persons. Vessel is restricted to carrying a total of 200 persons on board including crew and passengers, until the additional required IBA is installed for service.

46 CFR 115.814 At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Pump #2 low level hydraulic fluid audio alarm was found inoperable.

46 CFR 174.220 Each hatch exposed to the weather must be watertight. The lazzarette starboard side access hatch was found with worn gasket seal.

46 CFR 115.830 (b) At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Oily rags have been found in both port and starboard engine rooms.

46 CFR 115.830 At each inspection for certification and at every other 46 vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Portside engine room fuel filter flame arrester metallic bowl was found with diesel fuel.

46 CFR 11.840 The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Clean corrosion of portside fuel tank top for detailed inspection by USCG.

46 CFR 115.816 The owner or managing operator shall be prepared to test and make available for inspection all items in the ship's outfit, such as ground tackle, navigation lights and equipment, markings, and placards, which are required to be carried by the regulations in this subchapter, as necessary to determine that they are fit for the service intended. The switchboards located at the operating station Nav pannel I, Nav Panel II, and 12V DC, were found several closed circuit "red LED" indication light inoperable and two incorrectly connected.

46 CFR 122.420 The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including the duties listed in the station bill required by § 122.514. Provide crewmember training in their assigned duties as stated in the posted station bill, provide evidence of training to the USCG Marine Inspector.

46 CFR 121.420 As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Publication have been found last updated in YR2022. Deficiency was corrected prior finishing the exam.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operation at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of power on the starboard engine. Engine unable to maintain operation under load. 46 CFR 119.200(a)

07199: At each subsequent inspection for certification a vessel shall be prepared to conduct operational tests of all valves and fuel lines by operating locally and at remote operating positions. Fuel shut off valves for both diesel engines and generators were unable to be tested.

CG001: The Certificate of Inspection (COI) must be posted. The vessel did not have the most current COI posted.

01119: Each vessel must ensure the following document is carried on board and made available to the Coast Guard upon request: a letter signed by the vessel owner stating which alternative security program (ASP) the vessel is using and certifying that the vessel is in full compliance with that program. The vessel is not listed on the current ASP.

01209: No vessel may be navigated unless it has in its service and on board the crew complement required by the COI. The Senior Deckhand was not designated in writing as indicated on the COI.

09112: A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The first-aid kit did not contain the required contents.

04101: On a vessel of more than 65 ft in length, the public address (PA) system must be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members [name withheld] speaker on the port side outer deck was not operational.

Proposed alterations must be approved by the OCMI before work is started. 1. Found that port ship generator exhaust lagging was installed incorrectly which left a gap in two areas that exposed the underside of the two boxes to melt, the dripping plastic contacted the hot exhaust piping causing a fire. 2. The lagging prevents the pressure cap on the diesel generator cooling expansion tank from being removed. 3. Both batteries are required to be relocated. 17C 46 CFR 115.700(a).

Proposed alterations must be approved by OCMI before work is started. Found that battery acid had spilled on the port generator and migrated to the bilge. The battery acid to be removed from the generator and the hull plating so the hull plating can be evaluated for damage. 17C 46CFR115.840.

Proposed alterations must be approved by OCMI prior work is started. 1. Found that the two steering pumps/tanks had been replaced, provide specifications on the old and new units, i.e. weight of units, electrical specifications. 2. Found non-metallic hose had been replaced with stainless steel piping, provide specifications showing the piping meets the requirements in 46 CFR Subpart G. 3. Found that the stainless steel piping that replaced the non-metallic piping for the steering system was not properly supported; piping flexed 2-3 inches when moved by hand. 17C 46CFR115.700

Found that all voids aft of the port & starboard engine rooms exhibited excessive aluminum salt forming on overhead, longitudinals, and hull plating, all areas to be cleaned for evaluation. Remove nuts and bolts and miscellaneous items on the longitudinal bilges. 46CFR115.840 17C.

A dry chemical chemical extinguisher had to be used to put out a fire in way of the diesel generator, open port generator for inspections. 46CFR115.840 17C.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel was taken out of service to prepare for COI annual. Vessel operator rescheduled inspection date on multiple occasions. On 27MAR23, a further postponing of the annual inspection was requested. At that time it was reported to the USCG that work was being done to the vessel's steering assembly. Additionally, it was reported that damage was sustained to a battery bank resulting in melting and lagging onboard the vessel within a machinery space. The SUMMER WIND shall be made available for inspection the morning of the 28th of MARCH for the purpose of conducting a damage survey/in service inspection. 46 CFR 115.840

A self-priming power driven fire pump must be provided on each vessel. The fire pump must provide a steady stream of at least 50 psi. The starboard hull’s fire pump was not easily primed and the gauge showed a pressure of 30 psi while testing.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts or wires. There were several loose wires located at the port engine compartment in-way-of the generator.

The bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard. On the port hull engine compartment there was an excessive amount of oily water noticed on the bilges.

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected. The starboard main engine had noticeable oil and fuel leaks throughout the engine.

Each EPIRB must be tested monthly to determine that it is operative. The EPIRB test required shall be logged or otherwise documented. The last 02 logged EPIRB monthly tests were in June 2022 and Sep 2021.

The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: A self-propelled vessel of 65 ft or more in length engaged in commercial service and a propelled vessel that is certificated to carry more that 150 passengers. The installed AIS is not fully operational.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The required public address system was not operational.

At each initial and subsequent inspection for certification, the owner or operator shall have the vessel ready for inspection of its fire protection equipment to check for excessive corrosion and general condition. The portable fire extinguisher in the starboard engine room was found with a broken nozzle.

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected. There was a fuel leak identified on the fill pipe on the port hull, void space no. 2.

A fire control plan must show the location of fire protection and emergency equipment. The current posted fire control plan does not correctly identify the number of heat detectors installed on board. In addition, the heat detector on the port side, above the engine room ventilation is missing.

An adult life jacket must be provided for each person carried on board the vessel. In addition, child life jackets equal to at least 10% of the passenger carrying capacity of the vessel must be provided on the vessel. There were two (02) adult life jackets and four (04) child sized life jackets need to be replaced as they were in an unserviceable condition.

All observed unsafe practices or other hazardous situations must be corrected. There are several fractured welds on void spaces 1P, 2P & 2S. Submit repair proposal prior to starting repair process.

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question. Shaft should be pull out and inspected to the satisfaction of the attending marine inspector.

Each vessel msut have permanent draft marks at each end of the vessel. The aft port draft marks are missing.

In the presence of a Marine Inspector complete the testing of the Propulsion Control Qualitative Failure Analysis (QFA) and Design Verification and Periodic Test(DVTP) procedures per MSC Plan review letter E2-2100376 dated February 17, 2021. Provide all references listed in the letter prior to conducting the testing.

Each survival craft shall be secured to the vessel by a painter with a float free link permanently attached to the vessel. The painter line for each IBA is set up incorrectly. The painter line for each IBA was secured straight to the vessel. The painter line must be attached to the weak link.

The vessel owner must ensure restricted areas are designated on board the vessel. The navigation bridge is identified as a restricted area and is not properly marked.

The ASP is sensitive security information and must be protected in accordance with 49 CFR 1520. Sensitive security information must be stored in a secured container. ASP does not have a designated secured container.

Vessel is required to carry at a minimum 40-B;C type portable fire extinguishers in each machinery space. Vessel did not have the correct type of fire extinguisher in the machinery spaces.

Supervisory personnel must receive 60 minutes of training in regard to the Employee Assistance Program.

Vessel's ground detection meter is inoperable. Once ground detection meter is operational, vessel must prove proper operation by introducing a ground.

Due to repair/installation of new engine propulsion equipment, vessel must provide a statement of all propulsion equipment removed, rebuilt, and replaced, and in addition, a bill of materials including spec sheets.

Port and Stbd E/R: Fire dampers inoperable.

Stbd E/R: Improper patch w/ hose clamp on bilge pump discharge pipe.

Port E/R: Replace broken pressure gauge on fire pump disharge.

One of the vessel's 50-person inflatable buoyant apparatus has been removed for annual servicing. Total persons allowed on board when operating on a Limited Coastwise route beyond three nautical miles from shore is limited to 200 prior to 1 October and 150 from 1 October on until the IBA has been re-installed to the satisfaction of an attending marine inspector.
