The Garbage Plan is not ship specific. The [name withheld] has not listed the color coding of containers for garbage. Mixed garbage was found throughout the vessel. The crew is disposing fluorescent lights overboard after breaking them and medicines are not being tracked when disposed of ashore.

The new Ballast Plan issued by Lloyd's was not completed. Items for training, sample locations, pumping / piping arrangements and ballast water handling log forms were not identified in the plan. The [name withheld] has been using an old Ballast Water Record Book issued by the company, which was also in disarray. One plan needs to be put together and used.
