A vessel must carry one floating waterlight...Each light must be approved in accordance with 161.010 in subchapter Q of this chapter. Floating waterlight is not approved type under 161.010. Replace waterlight with an approved type under 161.010 within 7 days to the satisfaction of the attending marine inspector.

A vessel more than 26 feet in length but no more than 65 feet must carry one life buoy of not less than 24 inches in diameter. Ring life buoy found cracked and deteriorated. Replace life buoy within 7 days to the satisfaction of the attending marine inspector.

Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Flame impingement bowl under starboard engine fuel filter is missing. Replace bowl to the satisfaction of the attending marine inspector.

Repairs or alterations to the hull that affect the safety of the vessel must not be made without approval of the cognizant [name withheld] rot found along the perimeter of benches in main seating area and in port side aft corner of the upper deck beam. Grind areas back to good material and determine extent of rot. Upon determining extent of rot, provide repair proposal for review and approval of attending marine inspector. Once approved, make repairs to the satisfaction of the attending marine inspector.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The battery fuses are MRBF terminal fuse block fuses. Replace with UL approved cartridge fuses of the same voltage and amperage to the satisfaction of th attending marine inspector.

An adult life jacket must be provided for each person carried on-board a vessel. Attending marine inspector observed then non-serviceable life jackets. Replace with serviceable type 1 life jackets. Total persons allowed is reduced until deficiency is cleared. 46 CFR 180.71(a)

Provisions shall be made by means of plugs, covers, etc to prevent the admission of air into the space protected. Attending marine inspector observed an unsealed opening in the stbd engine space's fwd bulkhead. Restore air-tight integrity of the bulkhead to the satisfaction of the attending marine inspector. 46 CFR 181.20-8

A power driven vessel underway shall exhibit a masthead light forward. Attending marine inspector observed the forward masthead light fail to energize. Restore function to the satisfaction of the attending marine inspector. 33 CFR 83.23(a)(1)

On a vessel which the number of passengers permitted on upper decks is limited by stability criteria... the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) high. Attending marine inspector identified the upper deck unlabeled with passenger count. Corrected on the spot by vessel master. 46 CFR 185.602(g)

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). Attending marine inspector identified the forward most starboard bilge pump inoperable. Corrected on site by deckhand by pump replacement. 46 CFR 182.520(a)

By March 15 of the year following the collection of the data in paragraph (a) of this section, marine employers must submit the data on the form titled U.S. Department of Transportation Drug and Alcohol Testing MIS Data Collection Form... Vessel unable to provide proof of MIS submission. Submit and provide to the attending marine inspector proof of MIS documentation. 46 CFR 16.500(b)(1)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Attending marine inspector identified a section of rot in the port, aft-most, compartment on the inboard side immediately bellow the hatch. Repair in accordance with the verbally approved repair proposal to the satisfaction of the attending marine inspector. 46 CFR 177.300(c)

The intent of this part is to insure that the main propulsion machinery installed on each vessel is suitable for each vessel and its operations and that the auxiliary machinery is suitable for the purpose intended. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Stbd salt water hoses for the engine are collapsing just past the sea valve and chaffed just past the strainer. Replace two sections of hoses. 46 CFR 182.01-5

Provisions shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. Air supply plugs to close off the engine spaces have lines that are too short for the forward plugs on the port and stbd side. Replace with a longer line so that ventilation plugs can be put into place with ease. 46 CFR181.20-35

Check the conditions of the superstructure, flying bridge, masts, stacks, fishing platforms and similar arrangements constructed on the hull. In sailing vessels inspect all spars, running rigging, blocks, fittings, and sails. Cotter pins for all turnbuckles that attached the stays and shrouds to the chain plates were meat hooking. Ensure all cotter pins are turned inside to avoid injury or getting snagged. 46 CFR 176.25-5(a)(3)

Lamp, paint, and oil lockers and similar compartments shall be constructed of metal or lined with. MI discovered paint being stored in the port-side, forward compartment which is not constructed nor lined with steel. Paint stores to be removed prior to the carriage of passengers. 46 CFR 177.10-5(c)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy grab line is fraying. Replace grab line. 46 CFR 176.808(b)

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. Mesh on port and starboard flame screens were not completely attached to vent pipes. Reattach mesh to eradicate gaps. 46 CFR 182.20-35(d)"Old T"

Wire connectors utilized in conjunction with screw type terminal blocks must be of the captive type such as the ring or the flanged spade type. Non-captive connectors on bus bar under console. Replace with approved type. 46 CFR 183.340(k)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked "First-Aid Kit". A first-aid kit must be easily visible and readily available to the crew. First-aid kit is incomplete. Provide USCG-approved kit or a kit that complies with the list in 46 CFR 160.041. 46 CFR 184.710

The owner or managing operator of a qualified servicing facility shall conduct inspections and tests as applicable for fixed gas fire extinguishing systems and must provide evidence of the required servicing to the marine inspector. Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Vessel's fire extinguishing systems serviced but reports were missing. E-mail report detailing satisfactory evidence of required inspections, maintenance procedures, and tests by a qualified servicing facility. 46 CFR 176.810(b)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Company has had DAPI Audit conducted within the past year. Provide satisfactory proof that all crew onboard have passed pre-employment testing by providing company's crew list and testing results. 46 CFR 16.210(a)

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant [name withheld] worn. Replace buckets. 46 CFR 181.610

The owner shall be prepared to conduct tests and have the vessel ready for inspection of the following: On a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel does not have in place a preventative maintenance plan in accordance with USCG NVIC 02-16. Operator shall schedule a 3rd party rigging survey to include rigging inspection aloft, and provide the survey report prior to operating under sail to [email withheld]. 46 CFR 176.802(a)(3)

Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. To be completed at next drydock but no later than 31 March 2023. 46 CFR 185.602(c)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel broke free of its mooring and was found washed ashore on 07DEC2021 on the Sand Bar located in Kaneohe Bay. Vessel is prohibited from carrying passengers until the hull is made available for inspection by USCG Marine Inspectors to determine if the hull or its appendages were damaged while drifting or during the subsequent grounding. 46CFR176.600(d)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. Vessel does not have a valid Safety Radiotelephony Certificate. Vessel shall not operate more than 1,000 feet from shore without a valid Safety Radiotelephony Certificate.

All vessels must have operable navigation lights in accordance with the Navigation Rules. Port running light was inoperable. Provide navigation lights that are in compliance with the Navigation Rules. Vessel is restricted to day light only operations until all navigation lights are functional.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Raw water intake on the starboard engine is missing the hose clamp at the joint of the flexible hose and the metallic shutoff valve. Install hose clamp.

A vessel must comply with the structural design requirements of one of the standards listed. Third step from bottom on bow embarkation ladder has a fractured weld on the starboard side. Submit a repair proposal for pitting to the satisfaction of Marine Inspector.

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Vessels 12VDC switch panels do not have overcurrent protection. Each circuit shall be protected against overcurrent’s.

A vessel must be provided with bilge pumps in accordance with Table 182.520 (a). The following fixed hand bilge pumps are not operational, port storage compartment, port forepeak, port compartment forward of the lazarette, stbd forepeak and stbd fuel tank compartment. Additionally, there was no portable hand bilge pump onboard the vessel during the inspection. Demonstrate the proper operation of the five non-operational fixed hand bilge pumps and missing portable hand bilge pump to the satisfaction of the attending USCG Marine Inspector.

A vessel of more the 26 Feet in length, but not more than 65 feet, must carry a minimum of one life buoy of not less than 24 inches in diameter. The vessel's ring buoy is no longer serviceable. Provide a USCG approved 24 inch life buoy to the satisfaction of the attending USCG Marine Inspector.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The vessel's stbd running light is not operational. Demonstrate the proper operation of the vessel's stbd running light to the satisfaction of the attending USCG Marine Inspector.

An adult life jacket must be provided for each person carried on board a vessel. Each lifejacket must be approved in accordance with 160.002, 160.005 0r 160.055 in subchapter Q of this chapter. One adult life jacket's retro reflective tape is no longer serviceable. Provide a serviceable adult life jacket to the satisfaction of the attending USCG Marine Inspector. The vessel's total persons allowed has been reduced from 51 to 50 until this deficiency has been cleared by a USCG Marine Inspector.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are employed. The following sail rigging components were deemed to need replacing or verification that they are suitable for continued service by conducting non-destructive testing (NDT), the upper and lower mast tang sets and the headstay delta plate. Additionally, the port and stbd aft lower shrouds are missing cotter pins. Replace or conduct NDT of the upper and lower mast tangs and headstay delta plate and replace the port and stbd aft lower shroud cotter pins to the satisfaction of the attending USCG Marine Inspector. The vessel is not authorized to operate under sail until this deficiency has been cleared by a USCG Marine Inspector.

Sail rigging and components are to be maintained in accordance with good marine practice. Observed boom held in place by frayed topping lift and the tang connecting the backstay to the boom is cracked and corroded. Provide repair plan to ensure adequate rigging configuration per vessel's Preventative Maintenance Plan.

A portable hand bilge pump must be capable of pumping water. Starboard aft void portable hand bilge pump was not operable. Provide operable hand pump capable of pumping water from the space.

Each lifejacket must be marked in legible block capital letters with the vessel's name. 18 life preservers were not labeled with vessel's name. Mark in clearly legible block letters with vessel name.

Deck hatches in the main passenger deck do not prevent the ingress of water. Hatches observed with penetrations. Properly renew hatch covers to prevent ingress of water.

Vessel required to have one 24" ring lifebuoy fitted with 60' lifeline and floating water light attached with a corrosion resistant clip to allow quick disconnect. Ring lifebouy found cracked and faded. Provide serviceable ring lifebuoy onboard vessel and properly arranged.

Electrical equipment must be installed to protect from electrical hazards. Electrical wiring underneath operating station has several exposed and dead end wires. Properly remove or cover to prevent potential shock or fire hazards.

Code 99101: Operator does not have in place a preventative maintenance plan to adequately assess the safe condition of their sail rigging system in accordance with USCG NVIC 02-16. Provide 3rd party rigging survey to assess the condition of the sail rigging system and establish a beginning date for a PMP to be implemented. 46 CFR 176.800(a)

Code 08199: A vessel of 26 ft or more in length must have a visual and audible bilge high water alarm in spaces with through-hull fittings and a machinery space. High water alarm in starboard engine room found inoperable. 46 CFR 182.530(a)

Code 02108: Electrical installations shall be to the satisfaction of the Officer in Charge of Marine Inspections. Observed corroded wires at terminal connections on the main deck behind the port and starboard fuse panels. Observed corroded terminal connections between the house batteries on the main deck. 46 CFR 183.01-15(a)

Code 02199: The owner/operator shall have the condition of the hull, spars, standing rigging, running rigging, blocks, and fittings ready for inspection. Observed starboard back stay chain plate fastener (upper fastener) backed out away from the hull. Removal of the fastener revealed brown rot in the wood hull core. Provide repair proposal and refasten chain plate with repaired core. 46 CFR 176.802(a)(3)

Code 02108: Conductors must have overcurrent protection in accordance with their current carrying capacities. Operator placed a new electrical installation onboard, DC to AC inverter. Inverter conductor between battery and inverter is missing a disconnect means on the supply side of a fuse and is also missing a UL listed class cc, g, h, j, k, RK1, RK5, or T fuse. 46 CFR 183.380(a) and (k)(1)

Provide drip pans for fuel filters on both engines. 46 CFR 182.455(b)(6)
