The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: A self-propelled vessel of 65 feet or more in length, engaged in commercial service. Vessel's AIS is not operational due to inability to broadcast. Provide root cause and repair AIS to the satisfaction of the Marine Inspector. 33 CFR 164.46(b)(1)(i)

Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use. Marine inspector observed crack to corner of topmost buoyant apparatus on aft stack. 46 CFR 122.720(a)

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Operation of the fire main system. MI observed leaking valve stems at deck two fire hydrant, and in engine room at fire pump. 46 CFR 120.220(d)

All electrical equipment and circuits must be clearly marked and identified. Breaker panel in vicinity of bar on deck one has unlabeled switches. 46 CFR 115.810(a)(3)

Annual Inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel missed inspection window. Receive inspection prior to carriage of passengers.

Each lighting fixture…must have a guard…Lazarette lighting found with holed protective cover.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 80.1101(c)(2). Vessel’s VHF radio installation consisted of a radio that was not connected to GPS. Make repairs and have inspected by FCC certified radio technician for compliance with Radio Safety Telephony Certificate.

As appropriate for the intended voyage, a vessel must carry adequate and up to date: Charts large enough scale to make safe navigation possible. Vessel did not have a current chart onboard.

Each child sized life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so that the child size life jackets are not mistaken for adult life jackets. Marine Inspector found one child life jacket in box labeled for adults. Rectified prior to end of inspection.

Each disposable hydrostatic release unit must be marked in clearly legible letters with an expiration date of two years after the date on which the unit was installed. Marine Inspector found HRU with incorrect dates. Master fixed prior to end of inspection.

A hatch exposed to the weather on a vessel that operates on protected waters must be weathertight. The vessel’s forepeak hatch was not weathertight and needs to be made as such. 46 CFR 116.1160(a)(3)

Doors in “A” Class bulkheads shall have a latch with a minimum throw of ¾ inch which can be operated from either side of the door. The vessel’s fire doors for the galley and the stairways were missing a latch for the doors. Door handle and latch shall be replaced and shall be of sufficient grade to ensure "A" class fire resistance of door. 46 CFR 72.05-25 (b)(3) 46 CFR 177.10-5 (a) (OLD T)

Holdback hooks, or other means of permanently holding the door open, not subject to control station release, for doors in stairway enclosures and Main Vertical Zone bulkheads are not allowed. The vessel had holdback hooks for doors in stairway enclosures on the second deck and needs to be either removed or replaced with control station release device. 46 CFR 72.05-25(b)(9)(ii)

All vessels shall be provided with not less than two avenues of escape where crew may be normally employed. The Forward storeroom emergency escape scuttle dogging mechanism was inoperable and needs repaired. 46 CFR 177.15(a) OLD T

6. Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due location and route of vessel. Several windows were improperly sealed exposing gaps to the weather. One window on the port side main deck has a crack in the lower left hand corner. Much of the steel that supports the windows is severely corroded to unknown conditions and need repaired due to structural deterioration and concern of strength. Vessel shall develop window replacement plan to repair/replace windows. In the mean time due to the cost and in depth scope of work, the vessel shall ensure all windows on main deck and 2nd level deck are made weather tight. 46 CFR 116.1020

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to have vessel ready for the inspection of all accessible parts of the exterior and interior of the hull. Vessels mount for the masthead light was severely deteriorated and needs replaced. 46 CFR 115.802(a)(1)

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. 03 Buoyant Apparatuses were found to have cracks, holes, and deterioration and need replaced with sufficient Life Floats. 46 CFR 117.175(a)

A weather tight door must be provided for each opening located in a deck house or companionway. The STBD side galley door is not weather tight and needs to be made as such.

Alterations to equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel to submit plans for the installation of the electric powered horn. 46 CFR 115.700 (a)

The installation of portable fire extinguishers must be to the satisfaction of the cognizant [name withheld] extinguisher in upperdeck bar was loose and sitting on the deck. Fire extinguisher needs to be mounted. 46 CFR 118.520

An exterior lighting fixture in an electrical system operating at more than 50 volts must comply with the requirements of UL 595. The glass globe for the port aft deck light on the stern is missing and needs replaced. 46 CFR 120.410(d)

The vessel’s frames for the two exterior forward doors on the second deck showed significant deterioration near bottom and around window. Vessel made temp repairs to framing and will make permanent repairs at next drydock. 46 CFR 115.802(a)(1)

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. Numerous pieces of steel lattice used in beteween rail courses was loose or popping out posing a threat to passengers and crew. Deckhands sufficiently secured the lattice once identified during the inspection. All SAT. 46 CFR 177.35-1(a) OLD T

Kerosene and commercial standard fuel oil No.1, NO2, and No 3 are prohibited for cooking, heating or lighting on ferry or passenger vessels. Vessel must supply letter from Coast Guard approving tea lights by December 10 2021 in order to not receive action 60 or to remove all tea lights.

All vessels shall be provided with not less than two avenues of escape from all general areas where crew may be normally employed. The door that separated galley to main salon had 1 bolt missing in the hinge. The door was able to close and open with manipulation by lining up the hinges. The door leading from galley to stores below that led to engine room was sticking and hard to open. Vessel will be completing drydock on January 3 2021 and will be required to address structural issues that are placing weight on doors that are dis-aligning hinges. Vessel will be required to temporarily fix the doors to provider easier move-ability to the satisfaction of the attending MI.

Vessels operated solely within 20 nautical miles must be equipped with a VHF-DSC radiotelephone installation. This vessel is required to carry 02 VHF-DSC installations. One of the vessel's radios is not connected to GPS and therefore rendering DSC inoperable. The 2nd radio has a screen that is unreadable due to damage and it is unclear as to what channels and functions the radio is operating on. Repair 1st radio to conform to DSC. Repair or replace second radio and ensure compliance to DSC capability. 47 CFR 80.905

The name of a vessel must be marked or painted in clearly legible letters and numbers on each buoyant apparatus. The number of persons capacity must be marked or painted in clearly legible letters and numbers on each buoyant apparatus. Vessel had 02 BA without the vessel's name and capacity.

Rugs and carpets shall not extend up vertical surfaces more than 4". Carpets are not permitted where the spillage or leakage of flammable liquids is possible including areas immediately adjacent to bar service areas. Carpet in excess of 4" around outboard seating and carpet around bar areas on both interior decks. Make proper repairs to satisfaction of attending marine inspector. 46 CFR 116.423

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.050. Ring life buoy line on the second deck is worn and must be replaced. Vessel must rectify deficiency within 7 days. 46 CFR 117.70 (b)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Deck directly under urinal in men's restroom has been been temporarily patched with a hard putty. Restore deck to original condition at next drydock period. 46 CFR 115.802 (a)

The master or person in charge of a ship to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. Vessel was missing garbage placard at the time of inspection. 33 CFR 151.59

46CFR 177.10-5 & 46CFR 72.05 - The general constuction of hte vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Insulation missing from underneath a portion of the forward stairway.

46CFR 171.124(b) - Each hatch cover must have a securing device. Forward deck hatch missing securing device.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Marking for fuel shutoff station was hidden underneath the deck plate. Marking must be moved to be in a visible location.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Stowed so that when the vessel sinks the survival craft floats free and inflates automatically. Vessel IBA painter line was attached to the deck of the vessel and not to the float free link. Preventing the IBA to float free as designed.

INSTALLATION SHOULD BE SAFE FROM FIRE AND/OR ELECTRICAL SHOCK.REPLACE 110V OUTLET COVERS AND ADDRESS EXPOSED WIRING TO MEET 46 CFR 183.46 CFR 183.01-5

SEWAGE PIPE LEAKING ON GALLEY STORES IN STORAGE ROOM AFT OF ENGINE ROOM ON STARBOARD SIDE. STORAGE SPACES ARE REQUIRED TO BE IN A SANITARY CONDITION. REMOVE AND CLEAN SEWAGE COVERED ITEMS. REPAIR LEAKING PIPE. PERMANENT REPAIR OF PIPING MUST BE COMPLETED PRIOR TO CREDIT DRY-DOCK. 46 CFR 176.25-45 (a). Temporary Repair completed to make it to drydock.

ALL 5 HIGH WATER BILGE ALARM INDICATION LIGHTS ILLUMINATE WHEN ANY ONE SPACE IS TESTED. A VISUAL ALARM MUST BE PROVIDED THAT INDICATES A HIGH WATER LEVEL IN EACH REQUIRED SPACE.
