Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel missed its annual inspection window deadline of November 9, 2025. The vessel is not authorized to operate until an annual inspection is conducted.

A submersible electric bilge pump may be used as a power bilge pump provided that: (7) A positive shutoff valve is installed at the hull penetration. - Positive shut off valve for potable water tank space bilge pump has a broken valve. - No positive shut off valve installed for steering compartment submersible electric bilge pump

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. The following nonconformities were identified: - Keel guard on bow broken / exposing fastener holes in FRP - 1" diameter damage to FRP below waterline port side bow - 1" diameter damage to FRP below waterline port side approximately 10' from bow. - 1" diameter damage to FRP below waterline forward of port propeller.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Sewage space has sewage on deck.

On or before March 11, 1999, an existing vessel must comply with the bilge high level alarm requirements in § 182.530. (a) On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Compartment with valve below the water line was found without a high water level alarm installed.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. The below deficiencies provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must make correction to the below deficiencies and get underway with marine inspectors to perform satisfactory operational test and drills prior to any movement.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Crew was unable to provide records of drills and training being completed.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. Fire and man overboard drills have not been completed.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Belts for starboard engine alternator and bilge/fire pump loose and not making constant contact with drive pulleys.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter). No monthly maintenance of firefighting equipment was observed on board.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Guards to prevent entanglement of persons in engine pully/assessor drives missing on both main engines.

A vessel must be provided with a main steering gear that is: (1) Of adequate strength and capable of steering the vessel at all service speeds. Steering gear and hydraulic cable lines were observed to be gouging a hole in fiberglass and wearing hydraulic tubes when operating steering in both directions.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. There are no directions of operation for emergency fuel stops.

Where a flexible fuel fill pipe section is necessary, suitable flexible tubing ... When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Non-conductive fuel fill hose is being utilized with no conductor between fill and tank.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Some breaker switches on the breaker panel on the bridge are broken and unable to be switched on and off.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (2) Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. Watertight hatch on stern of main deck inoperable.

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. Fall overboard prevention guard on the stern is cracked through completely.

The individual bilge suction lines shall be led to a central control point or manifold. Each line shall be provided with a stop valve at the control point or manifold and in addition shall be provided with a check valve at some accessible point in the bilge line. A stop-check valve located at the control point or manifold will meet the requirements for both a stop valve and a check valve. Bilge system is not equipped with a check valve. 46 CFR 182.25-5 (c) (Old T)

Mechanically propelled ''S" vessels which are ferry vessels or which carry more than 49 passengers shall comply, with the provisions of' Subchapter F of this chapter. The low melting points of many nonferrous metals and alloys, such as aluminum and aluminum alloys, must be recognized. These types of heat sensitive materials must not be used to conduct flammable, combustible, or dangerous fluids, or for vital systems unless approved by the Marine Safety Center. Vessel bilge strainers are constructed of plastic material. 46 CFR 182.40-5(b) (Old T) 46 CFR 56.60-20(a)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vent opening on the starboard side was unable to be kept open.

All cable must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Control cable runs above the main diesel engines are secured with plastic supports.

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Fuel filter for generator not equipped with approved filter bowl.

In systems and applications where flexible hoses are permitted to be clamped: (1) Double hose clamping is required where practicable Single hose clamps found in multiple locations on vessel at through hull connections below water line.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Multiple adult life jackets found with wasted retroreflective tape.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. 1 Child life jacket, multiple adult life jackets, and life ring buoy by bridge missing vessel name.

An adult life jacket must be provided for each person carried on board a vessel. 1 Adult life jacket missing for passenger count.

Except on a vessel limited to daytime operations, the compass must be illuminated. Magnetic compass on board is not able to be illuminated.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions were not mounted at the operating station.

Watertight doors and watertight hatches must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. Aft hatch not labeled.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency broadcast placard missing information specific to vessel.

All other vessels must undergo an inspection for certification as specified in § 176.404 and annual inspection as specified in paragraph (b)(1) of this section.(1) Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed an annual inspection within required time frame.

Each vessel required to have a Certificate of Inspection is subject to the annual vessel inspection fee listed in table 2.10-101 for its vessel category. Vessel has not paid annual vessel inspection fee since 2020.

Nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel's seaworthiness. Vessel is required to conduct a full marine survey based on vessel's structural conditions discovered during Annual Inspection. 46 CFR 176.500(b)(1)(iv)

Except as otherwise provided in this section, machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads of double diagonal wood, marine plywood, steel plate, or equivalent construction. a. Aft bulkhead in amidships machinery space is separated from deck flooring in the port quarter section. There is significant water intrusion further deteriorating wood supports. b. Excessive wastage on forward and aft bulkheads in machinery space connecting the adjacent spaces allowing light to freely show through. 46 CFR 182.35-1

... The inspection shall be such as to insure that the vessel, as regards the structure... is in satisfactory condition and fit for the service for which it is intended... a. Significant portions of the deck around the entire vessel are wasted and significantly sagging with questionable structural integrity. b. Starboard bow gunwale has been significantly damaged from apparent impact with dock causing superstructure to sink on the main deck. Damage is allowing a free flow of water intrusion inside sewage tank space. c. Engine room hatch access wood frame fell off decreasing the integrity of the hatch cover and presenting a hazard to the passengers. d. There is gap due to fiberglass separation between main passenger deck and fuel tank space behind bench seat. e. Inside water tank space; the wood core of the deck longitudinal has completely wasted away. f. Upper deck beam support has gone through main passenger cabin deck creating a hole on deck. g. Theres a crack on the starboard side quarter deck that allows water inside vessel. h. There are two holes one foot above the waterline on the starboard side shell that can be seen from inside the fuel tank space. i. Bow pulpit is deflected to starboard. j. Upper deck longitudinal framing is saturated with water. Submit repair plan to the coast guard for review and approval. Do not commenced any work without approval of the marine inspector. 46 CFR 176.05-10

… where passing through bulkheads, fuel lines shall be protected by close fitting ferrules or stuffing boxes. Nonmetallic flexible hose may not penetrate a watertight bulkhead. Fuel lines pass through bulkhead without required metal fittings. 46 CFR 182.20-40(b)(2)

Provisions shall be made by means of plugs, covers, ect., to prevent the admission of air into the space protected. Modifications to vessel's sitting benches, removed the only way of closing the vents that lead to engineroom creating an unsafe condition. 46 CFR 181.20-35(a)

All vessels of 26ft in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Bilge Piping on fuel tank space has wasted away, causing a full break in the pipe, making the system inoperable. 46 CFR 182.25-5

At each initial and subsequent inspection for certification, re-inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations are corrected. Fuel tank fuel lines/fittings are leaking inside the space presenting a fire hazard. Ensure all fittings are tight and no fuel leaks are present. 176.25-50 (a)

At each initial and subsequent inspection for certification, re-inspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Excess water in machinery and septic spaces. Empty and clean bilges. Once complete inform inspectors, so they can complete inspection. 176.25-50 (b)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Oil filter on the port side main propulsion engine is disconnected from the block, installed incorrectly, and attached to a structural support stanchion. 46 CFR 182.05-1

A suitable hull strainer shall be installed in the circulating water intake line. Sea strainer for the port side main propulsion engine is cracked and leaking. Replace with new. 46 CFR 182.20-10(b)(3)

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. The forward rails move excessively with little structural support. 46 CFR 177.35-1(a)

Propulsion machinery must be suitable for type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine loads. Port main engine stalls when maneuvering.

Deck longitudinal port/center in engine room overhead is fractured. Provide a repair proposal. Should include vertical support.

Machinery guards missing on port and starboard engines. Replace guards.

Replace light cover in engine room, port side/aft.

Deck rails must be designed and constructed to withstand a point load of 200 pounds at any point in any direction. Hand rail on starboard bow has broken welds from impact. Passengers not allowed on the bow until satisfactorily repaired. 46 CFR 177.900(b)

Life rings must be in accordance with Subchapter Q specifications. Liferings need to be replaced with SOLAS type retroreflective tape and clearly labeled with vessel name. 46 CFR 180.70(b)

Each portable fire extinguisher on a vessel must be of an approved type. Portable fire extinguisher on main deck, midship port side, not charged. Replace fire extinguisher. 46cfr181.500(a)

Internal combustion engine exhausts and similar sources of ignition must be kept clear and suitably insulated from combustible material. Sound dampening material in over hang of engine compartment is heavily saturated with oil. remove or replace material. 46cfr177.405(a)

A vessel of atleast 26 ft in length must be provided with individual bilge lines and bilge suctions for each water tight compartment. Bilge piping is corroded and busted. Replace both piping in engine compartment for aft compartments. Replace in kind. Vessel restricted to 49 passengers until deficiency rectified. 46CFR182.510(a)

A vessel must be provided with bilge pumps IAW Table 180.520(a). Electric bilge pump in forward compartment inoperable. Repair or replace. 46CFR180.520(a)

Rail on por bow was damaged due to allision with morred vessel. Top rail cracked in two places as the rail slopes down to main deck. 705: Passengers cannot access bow area until rail is repaired.

Renew bilge pipe with wasted hole in Engine Room.

Subdivision bulkhead between Engine Room and aft void is delaminated from amidships to port side. Grind back bulkhead to good material and renew as original.

Tabbing around steering gear mount platform in lazarette has delaminated. Renew tabbing around steering gear mount platform.

Small 2-3 inch delamination spots throughout hull (marked). Grind back to good material and renew.

Propellors both worn and damaged. Renew port and starboard propellor.

Delamination along amidships line between shafts at stern. Grind back to good material and renew as original.

Battery covers for both battery boxes in the engine room are missing. Replace box covers. 46CFR183.200

Both ring buoys are missing proper stenciling. Permenantly stencil the vessels name and hailing port on both ring buoys.
