SATISFACTION II

Vessel was missing 13 life jackets. Failure to ensure enough life jackets were onboard. Provide 13 life jackets onboard vessel to the satisfaction of the attending Coast Guard marine inspector.

Two Fire hoses had holes and were leaking. Failure to comply with fire fighting appliance requirements. Replace fire hoses to the satisfaction of the attending Coast Guard marine inspector.

Distress flares were expired. Failure to comply with distress flare requirements. Provide distress flares to the satisfaction of the attending Coast Guard marine inspector.

Two life buoys were sun bleached and worn. Water light battery was dead and lifeline broke when pulled upon. Failure to maintain life buoy and appliances. Provide 2 life buoys, water light and lifeline to the satisfaction of the attending Coast Guard marine inspector.

Type III MSD discharge valve was not secured. Failure to secure each Type III MSD prior to operating vessel in an area where the discharge of untreated sewage is prohibited by the EPA. Valve was secured during inspection.

Fire hose nozzles were a plastic unapproved type. Fire hose nozzle shall be of brass or other corrosion resistant material that comply with NFPA 1963. Replace three fire hose nozzle with correct type to the satisfaction of the attending Coast Guard marine inspector.

Vessel's pre-engineered fixed gas fire extinguishing system was found in the override position on the helm and the green light for for ready and the red light for discharged were extinguished leaving operator unsure of the operational status of the pre-engineered fixed gas fire extinguishing system. A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Provide service tech report that the pre-engineered fixed gas fire extinguishing system has been serviced and is fully operational.

During the fire drill there was confusion on the order and process to deploy the agent of the fixed fire fighting system. The directions on the pull box was incorrect. Crew unable to explain if engines are shut down by means of electrical, air intake or fuel shut out. Crew was unsure of the override/reset switch on the helm. Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Provide written procedures and diagram that explain the specific function and operation of the make/model fixed fire fighting system for the Satisfaction II, to the satisfaction of the attending Coast Guard Marine Inspector.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen in the water. Conduct satisfactory man overboard and abandon ship drills in the presence of an attending CG Marine Inspector prior to carrying passengers.

Chemical testing of personnel must be conducted as required by 46 CFR 16 subpart B and in accordance with the procedures detailed in 49 CFR 40. Provide evidence of satisfactory chemical testing program to attending CG Marine Inspector prior to carrying passengers.

A vessel must carry one floating water light approved in accordance with 46 CFR 161.010 unless limited to daytime operations. Prove proper operation of installed water light to attending CG Marine Inspector prior to night operations.

The operator of each vessel 12 meters or more in length shall carry, on board and maintain for ready reference, a copy of th Inland Navigation Rules as defined in 33 CFR Subchapter E. Provide evidence of a current copy of Inland Navigation Rules to attending CG Marine Inspector

No valid COD on board.

Update emergency instructions to accurately reflect actual operations aboard vessel.

Replace fire extinguisher located in pantry indicated during inspection with USCG approved B-II or 2-A.

Replace smoke detectors where overnight accommodations are available to passengers.

If console on upper-deck is completely de-energized, remove 10-B:C or B-I and replace with a A-II or 2-A.

Due to the river conditions, a man overboard drill and fire drill were not conducted during your vessel’s COI. Please schedule a follow up with this office prior to the due date.

On a vessel of more than 19.8 (65’) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel’s name.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers.
