Vessel had 23% LEL in 3 Stbd segregated ballast tank. When asked to recheck portable measuring device against known span gas [name withheld] was not able to complete span check. Vessel SMS required weekly testing of ballast tanks, and before entering port where ballasting is to take place. No tests were done prior to entry in the Port of New York. See Form A and Form B for details.
