FUEL OIL BARGE 101

A sign shall be displayed to warn persons approaching the gangway, while a vessel is moored or anchored unless it is empty and gas-freed. The sign shall state in letters not less than 2 inches high substantially as follows: Warning No open lights. No smoking. No visitors. Warning signs throughout the vessel are faded and illegible. Renew all warning signs and marking throughout the vessel and specifically around the generator room structure.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. Flame resistant stuffing material at cable terminal connections for all Class I, Div I equipment at the electric pump transfer stations for the deck cargo tanks are showing signs of wear and separation. Renew all flame resistant stuffing material at cable terminal connections.

Each vessel must carry lifebuoys approved under approval series 160.150. Lifebuoy on port side of the generator room structure needs to be replaced. Retroreflective tape is worn. Markings are worn. Line around the lifebuoy is frayed and deteriorated. Throw bag is deteriorated.

Inspection for certification—TB/ALL The inspection shall be such as to insure that the vessel, as regards the piping, main and auxiliary machinery, and other equipment is in satisfactory condition and fit for the service for which it is intended. The port cargo transfer pump main seal is leaking excessively when operated. Repair seal/transfer pump to prevent cargo leaking prior to use of transfer pump.

Each cargo tank of a tank barge must have a high-level indicating device. The high-level indicating device must (1) Indicate visually the level of liquid in the cargo tank when the liquid level is within a range of 1 meter (3.28 feet) of the top of the tank; (2) Show a permanent mark to indicate the maximum liquid level permitted under 46 CFR 39.3001(e) at even keel conditions; and (3) Be visible from all cargo control areas. All metering sticks are inoperable. Renew metering sticks to accurately gauge liquid level of cargo tanks.

Each nonmetallic transfer hose must: (i) Have no unrepaired loose covers, kinks, bulges, soft spots or any other defect which would permit the discharge of oil or hazardous material through the hose material, and no gouges, cuts or slashes that penetrate the first layer of hose reinforcement. (iii) Not burst, bulge, leak, or abnormally distort under static liquid pressure at least 1-1/2 times the maximum allowable working pressure The lube oil transfer hoses on reels (3 on the port side and 2 on the starboard side) have not been satisfactorily tested. Provide objective evidence of hose testing as required prior to use of hoses.

Each hose must be marked with one of the following: (1) The name of each product for which the hose may be used; or (2) For oil products, the words “OIL SERVICE” (f) Each hose also must be marked with the following:(1) Maximum allowable working pressure; (2) Date of manufacture; and (3) Date of the latest test required. The lube oil transfer hoses on reels (3 on the port side and 2 on the starboard side) are not labeled properly. Label hoses with the required information above AFTER satisfactory testing required by 33 CFR 156.170.

Inspection for certification—TB/ALL. The inspection shall be such as to insure that the vessel, as regards the piping, main and auxiliary machinery, and other equipment is in satisfactory condition and fit for the service for which it is intended. The waste oil transfer system is inoperable. Repair or tag-out (locally and at the switchboard) the waste oil transfer system. The system may be completely removed and piping systems must be capped/secured if the system is no longer serviceable.

EXHAUST PIPE INSTALLATIONS MUST CONFORM TO THE REQUIREMENTS OF ABYC P-1 AND NFPA 302. BOTH DIESEL GENERATORS MISSING EXHAUST LAGGING/INSULATION ON PIPING TO PREVENT FIRE IN THE EVENT OF A FLAMMABLE LIQUID LEAK. INSTALL EXHAUST LAGGING/INSULATION ON EXHAUST PIPING FOR BOTH GENERATORS. LAGGING/INSULATION SHALL BE INSTALLED DIRECTLY ON PIPING BEHIND THE EXISTING GRATED GUARDS.

These Rules apply to all vessels upon the inland waters of the United States. The Rules concerning lights (83.20 through 83.31) shall be complied with from sunset to sunrise, and during such times no other lights shall be exhibited, except such lights as cannot be mistaken for the lights specified in these Rules. Remove dead ended electrical cables from the Aft Mast.

Repair the fuel oil leak from the diesel lift pump on the #1 generator, to ensure it is fit for the intended service. Currently there is a very small leak.

Provide a proper flame screen on the waste oil tank vent. Currently the vent is missing.

Affect Permanent repairs to the damaged containment drip pan to meet the original approved plans. Currently a support bracket has been damaged by a side impact, the containment is intact but its ability to support the weight of a fully loaded condition is questionable.

The #2 void port side shell plated a gash that was 6' by 8'. No other significant damage or wastage.

An inset of 3' x 2 1/2" port side at #2 void space, with a fracture of 2' x 2 1/2" in on weld, and a fracture of 2' x 1/4" at gunnel. Vessel not to load cargo until permanent repairs are made.

Label all transfer hoses with "OIL SERVICE" and MAWP. Date of latest test and date of manufacture may be listed elsewhere. Do not label test pressure on hose.

Replace hatch cover gaskets on void space hatches (where gaskets do not extend the full circumference of the hatch).

Replace worn hydraulic hoses on port and starboard deck cranes.

Repair leaking lube-oil valve on #3 lube-oil tank (located at tank discharge line).

Repair leaking lube-oil valve on #3 lube-oil tank (located centerline immediately aft of the transfer pump).

Hydro-test STBD-side transfer hose. Do not use hose until hydro-test is completed.

STENCIL TRANSFER HOSES "OIL SERVICE ONLY".

Ensure all electric motors are intrinsically safe.

Replace all deteriorated hoses on both davits.

Seal on #2 cargo pump leaking product.

Packing gland leaking on #1 loading valve.

Renew all gaskets on hatch covers.

Transfer hoses not marked IAW 33CFR 154.500, issued CG 835 to show compliance by 22 Oct 2004.

Properly mount CO2 fire extinguisher in engine room.

Replace all void hatch cover gasket material.

Remove unservicable lube oil hose on starboard fueling station and properlyblank-off hose reel's pipe line.

Clean all cargo tank vent flame screens.

Replace brake pad for the starboard forward cable winch.

Properly label cargo hoses with identifying markings for pressure test documentation.

Prior to any transfer operations, provide for the USCG (MSO Memphis) review, Oil Transfer Procedures IAW 33 CFR 155.750.

Prior to any tansfer operations, demonstrate the remote manual sut-down forthe cargo pumps' engines IAW 46 CFR 32.50-35.

Prior to any transfer operations, identify on the vessel the remote manual shut-downs for the cargo pumps' engines IAW 46 CFR 32.50-35.

In accordance with 46 CFR 67.123, provide the correct vessel's name on the port & starboard sign boards. The correct name is permanently afixed to the vessel's hull. Vessel's name is: FUEL OIL BARGE 101.

HYDRO TEST CARGO HOSES AND PROVIDE PROOF OF SAME TO THE COGNIZANT OFFICER IN CHARGE OF MARINE INSPECTION.

Mark each cargo hose and provide certification of hydro test that can be identified with each individual hose._x001C_ _x001C_
