All vessels shall be provided with bilge pumps. Vessel unable to get suction and pump overboard in the engine room and hold below the bar. 46CFR182.25-10

Portable fire extinguishers shall be installed to OCMI satisfaction. Mounting brackets seized and difficult to remove portable fire extinguisher in emergency. Replaced mounting brackets to satisfaction of attending MI. 46CFR181.30-10

The name of the vessel and person capacity shall be clearly marked on each life float. Life floats were not marked that the MI's were able to verify. 46CFR185.604

Each life ring shall have tipe II retroflective tape. Retro worn and inneffective and requires replacement. 46CFR185.604

Fixed fire extinguishing systems shall be installed and maintained are are subject to hydrostatic testing at intervals of 12-years or less. MI unable to identify proof of hydrostatic test within 12-years. 46CFR181.20-1, NFPA 7.3.2.2

Each lighting fixture must be of the fixed type. Several portable overhead lights plugged into an outlet identified in interior spaces. Remove or make permanently fixed to the satisfaction of attending MI. 46CFR183.410

Wiring joints and splices shall be mechanically secure and made in a junction box. Several junction boxes and outlets found open, cracked or not watertight. 46CFR183.05-50

Visual and audible alarm required at operating station for high water level in unmanned spaces. #3 hold, engine room and lazerette spaces high water light at the operating station visually displayed high water in several compartments when one compartment was tested. 46CFR182.530

Each vessel that operates beyond 3-NM from the coastline shall have an EPIRB intalled to automatically float free. Hydrostatic release expired. Vessel restricted to within 3-NM from shore until replaced and attested to by MI. 46CFR180.64

Any vessel of at least 5 net tons must have a certificate of documentation. Vessel's COD onboard expired. Provide proof of valid COD and retain copy onboard. 46CFR67.7

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears and rotating machinery. Rotating belt exposed on the generator in an an exposed area in the engine room. 46CFR177.35-15.

Penetrations and opendings in watertight bulkheads must be kept as high and as far inboard as practicable and have a means to make them watertight. Watertight bulkead aft of the anchor chamber has an opening low and center in the bulkhead wall with cables running through it that is not watertight. Make bulkhead watertight and ready for further inspection by MI. 46CFR171.114

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent content and instructions. Vessel did not have all required equivalent contents of a first aid kit approved under 160.041. Provide equivalent contents or a first aid kit approved under 160.041.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessels two 22 man life floats where found to be not in serviceable condition as evidenced by extensive UV deterioration and large openings exposing the foam flotation material. The exposed foam flotation material was found to be severely UV damaged and crumbling into pieces. Both life floats must be replaced by vessel owner. A temporary repair of life floats may be authorized by attending MI until both life floats are permanently replaced. Owner must send MI repair proposal for life floats for approval. Upon approval of temporary repair proposal, life floats must be visually examined by attending MI and satisfactorily float tested in the water in the presence of the attending MI. Vessel is restricted from operating less then 1NM from shore until life floats are replaced or temporary repair is satisfactory.

An adult life jacket must be provided for each person carried onboard. Vessel owner/operator provided 120 adult life jackets which does provide the proper amount for 121 adult passengers listed on the COI. Vessel owner must provide an additional life jacket or reduce passenger count to a total of 120 adult passengers.

Vessel's less than 65' carrying more than 49 passengers shall have a piping system and bilge pump capable of pumping 25 gallons per minute, and a portable hand pump for each space aft of the collision bulkhead. Vessel's bilge pump is not operational. 46CFR182.25-5; 46CFR182.25-10

A visual and audible alarm must be provided at the operating station to indicate a high water level in unmanned spaces. The visual alarm worked in space #3 aft of the kitchen, however, the audible alarm was inoperable. 46CFR182.530

Penetrations and openings in watertight bulkheads must have a means to make them watertight. Two pipe penetrations from the tank room to the engine room need to be made watertight, and two pipe penetrations from the engine room to the steering gear room need to be made watertight. 46CFR171.114

A vessel must be fitted with ground tackle appropriate for the vessel and waters on which it operates. Forward starboard cleat not properly secured to the vessel. 46CFR184.10-1

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vessel is not equipped with closure devices for the engine room's air intakes. Provide a means to readily close air intakes to the engine room to the satisfaction of the attending marine inspector prior to movement of the vessel and seal the penetrations in the engine rooms forward and aft bulkhead. 46 CFR 182.465

Collision bulkheads must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. Make penetrations made by the vessel's hydraulic hoses watertight. 46 CFR 179.310 (a)(2)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Repair or replace exposed wire above the aft of the port engine. 46 CFR 183.200

emote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high. The labels for the vessel's fuel shut should be closer to the shutoff. Move label to its original position. 46 CFR 185.608

Vessels with upper decks that permit passengers are limited by stability criteria to a maximum number of passenger permitted on the upper deck and must be indicated with a durable marking of at least 1" numbers and letters at the entranceway to the deck. Marine Inspectors observed the marking with maximum passenger count at the entranceway to the upper deck was missing. Install marking to indicate a maximum of 47 passengers permitted on upper deck. 46 CFR 185.602(g)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation. Marine Inspector observed chart of large scale and area of operation was missing. Provide up-to-date chart for area of navigation. 46 CFR 184.420(a)(1)

Marine inspectors shall inspect all accessible parts of the exterior and interior of the hull to ensure a satisfactory condition of the hull structure. In the vessels second compartment from the bow submit a written repair proposal to the the connection between the port side stiffener and the hull to the satisfaction of the attending marine inspector. 46 CFR 176.25-5(A)(1)

Marine inspectors shall inspect all railings and bulwarks and their attachment to the hull structure. On the vessel’s port and starboard gangway access the connection between the handrail and the deck show signs of rot and water ingress. Submit a repair proposal to the satisfaction of the the attending marine inspector. 46 CFR176.24-5(4)

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in 46 CFR 176.500(b)(1) and your Certificate of Inspection must be endorsed. Vessel has not completed its annual inspection within the alloted period. 46 CFR 176.502

The owner or managing operator shall make a vessel available for drydock examinations and internal structural examinations required by 46 CFR 176.600(c)(1). Vessel has not completed its drydock and internal structural examinations within the alloted period. 46 CFR 176.600

At each inspection, galleys may be examined to determine serviceable condition. The grease extraction hood should be cleaned.

The life float’s floating water light must be attached around the body of the float with a lanyard of at least 18 feet. One of the vessel’s life float has an inadequate lanyard. Replace with an adequate lanyard.

A vessel must carry either a first aid kit approval series 160.041 or equivalent contents. Provide vessel with an adequately equipped first aid kit.

Limited coastwise route vessels must carry six hand red flares and six orange smoke flares or 12 hand red flares. Vessel is currently equipped with four red hand flares and three orange smoke flares. Provide adequate number of flares. Vessel is limited to a lakes bays and sounds route until satisfied.

Rails on passenger decks of a vessel engaged in excursion trips must be at least 39.5 inches high. Rail courses or equivalent must be installed between the top rail. On passenger vessels engaged in excursion trip must have bars, slats, or rail courses spaced not more than 4 inches. Vessel’s lower deck rail height is 36” and upper deck’s courses are 9.5” a part. Submit a repair proposal and conduct satisfactory installation prior to operating in the 2022 season.

A vessel must carry either a first-aid kit approved under 46CFR160.041 or a kit with equivalent contents and instruction. Vessel's first-aid kit missing Aspirin.

Vessel equipment shall be in working order to the cognizant OCMI as good Marine practice. Vessel's anchor winless inoperable preventing safe recovery of the anchor.

Vessel equipment shall be in working order to the cognizant OCMI as good Marine practice. STBD engine lube oil filter incorrectly installed.

Diesel fuel flexible hose shall be fabricated to ASTM D1692 standard. Port engine flexible fuel oil hose external coating waisted.

All electrical installations shall be to the satisfaction of the Marine Inspector. Dead end wires located in the pilot house need to be terminated or removed.

46 CFR 177.100 Construction & arrangement of the vessel must allow for safe operation of the vessel. The aft stbd corner of the superstructure has minor damage. Repair to the satisfaction of the attending MI.

Flexible electric cord and cables must meet Section 400-7 of the National Electric Code. Found household extension cord running to battery charger in engine room. Bring cable into compliance with National Electric Code to satisfaction of attending CGMI.

Machinery items shall be in proper operating condition and fit for the service in whcih they are intended. Found marine growth in port side fuel filter. O/O explained that he changes this filter frequently. Clean fuel tank and fuel filter to satisfaction and of attending CGMI.

Main propulsion machinery and cooling water system shall be fit for their intended service. Found leak in flexible hose on port side engine cooling system. Repair or replace hose to satisfaction of attending CGMI.

Main propulsion machinery should be in proper operating condition and fit for intended service. Found oil leak from starboard side engine flexible hose. Repair or replace to satisfaction of attending CGMI.

Main propulsion machinery should be in proper operating condition and fit for intended service. Port shaft showed excessive leaking. Repair or replace port side shaft packing to satisfaction of attending CGMI.

Bilge systems should be in proper operating condition and fit for inteded service. Found several clogged bilge suction strainers. Clean or replace to satisfaction attending CGMI.

Cable supports shall be supported by metal clips. Found plastic clips and zip ties throughout vessel. Replace cable supports to satisfaction of attending CGMI.

A vessel of at least 26 feet in length must be provided with individual bilge lines and bilge suctions for each watertight compartment. Bilge suction in lazarette had hole in piping three inches above suction pad. Replace bilge pipe in accordance with appropriate specification and to the satisfaction of attending Coast Guard marine inspector. (46 CFR 182.510(a))

Crew members shall conduct sufficient drills. Crew members were unable to safely recover person from water. Get suitable platform for recovery prior to carrying passengers to the satisfaction of the MI.

Pipes that penetrate the hull below 6 inches above the deepest subdivision load line must have means to prevent water from entering the vessel. The gate valve on the nonfunctioning sewage overboard is broken in the closed position. Cap valve with a plug of equal material to the satisfaction of the MI.

Vessel should be issued a Safety Radiotelephony Certificate by a technician. Call sign listed on Safety Radiotelephony Certificate is incorrect. Have technician correct call sign on Safety Radiotelephony Certificate to the satisfaction of the MI.
