The following emergency lighting must be arranged so that they can be energized from the temporary emergency power source; lighting for passageways, stairways, and escape trunks in passenger quarters, crew quarters, public spaces, etc., sufficient to allows passengers and crew to find their way to muster stations. At least 15 emergency lights were found to be inoperable in the forward and aft accommodation spaces. 46 CFR 112.15-1 (c)

Electric installations and electric equipment must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. Vessels emergency diesel generator failed during load testing to due a sheared f/o limit switch. 46CFR110.30-5

Machinery must be suitable in type and design for the purpose intended. Vessel experienced a loss of propulsion due to a faulty O-ring in their port side engine starter air valve. Vessel replaced the valve and ran multiple tests concluding the vessel is operational and seaworthy. 46CFR169.601(b)

Except as provided for in paragraphs (b) through (e) of this section, each vessel must undergo drydock and internal structural examinations as follows: If your vessel operated in fresh water at least 50 percent of the time since your last drydocking, it must undergo a dry dock and internal structural examination at intervals not to exceed 5 years unless it has been approved to undergo an underwater survey (UWILD) under § 167.15–33 of this part. Vessel has exceeded drydock and internal structural exam date and is not approved for UWILD. Vessel has requested extension IAW 46 CFR 167.15-30(e). Movement of vessel is prohibited until extension is approved by Commandant(CG-CVC). 46 CFR 167.15-30(a)(2)

Except as otherwise provided in these regulations, the height of the sills of access openings in bulkheads at ends of enclosed superstructures shall be at least 380mm above the deck. Vessel has cable way transits in some of the watertight door sills. WTD 1-120-0 was found with missing cable way cover. ICLL 88 II/12(3) 17ac

Each Class 1 door must have a quick action closing device operative from both sides of the door. WTD 4-65-2 that serves the access for the emergency fire pump could not be closed from either side. 46 CFR 170.270(a) 17c

Except as provided in paragraph (3), ventilator openings shall be provided with weathertight closing appliances of steel or other equivalent material. Ventilators on aft weather deck were found to have heavy scaling that prevented the use of weathertight closing devices. Additionally, a ventilator on the aft end of the house on the aft deck could not close due to a valve in the way. ICLL 88 II/19(3) 17c

Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. Shore steam line found with missing lagging. 46 CFR 56.50-1(K)

Except as otherwise provided by law or regulations in this subpart, the following standards shall be accepted as standard by the inspectors: American Bureau of Shipping “Rules for Building and Classing Steel Vessels” regarding the construction of hulls, boilers, and machinery in effect on the date of inspection. Temporary repair authorized and make permanent repair at shipyard. -Wastage in bosun stores outboard forward corner corrosion on bulkhead to paint locker. -Wastage in port pilot locker lower bulkheads. -Wastage in port ladder well to 01 deck. -Wastage in Paint locker: forward port corner deck corrosion. -Wastage in line locker aft corner. Submit repair proposal for approval and repair to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 167.15-25(a)

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. The Officer in Charge, Marine Inspection?may grant an extension of the ten year replacement to coincide with the vessel's next drydocking. Nonmetallic expansion joints due to be replaced 31AUG2023. Vessel to replace prior to credit drydock. 46 CFR 61.15-12(b) 705c

A device must: Be designed to prevent overboard discharge of treated or untreated sewage or any waste derived from sewage (Type III). MSD holding tank is leaking into bilge. Submit repair proposal and repair to satisfaction of attending Marine Inspector prior to credit drydock. 33 CFR 159.53(c) 705c

Periodic Safety tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Test Procedure 12.1 Lube Oil Pressure Shutdown and Alarm referenced in PSTP Rev. I was unable to be tested as approved by MSC Approval E2-2301682 dated May 9, 2023. 46 CFR 61.40-6(a)

Passenger ships of 500 gross tonnage and over shall carry at least one rescue boat complying with the requirements of 5.1 of the Code. The normal equipment of a fast rescue boat shall include a VHF radiocommunication set which is hands free and watertight. Rescue boat missing hands free VHF radiocommunication set. Repair by next credit drydock. 74 SOLAS (20) III/21.2.1 LSA 5.1.4.11

Each launching mechanism must be arranged so it may be actuated by one person from a position on the vessel's deck, and except for secondary launching appliances for free-fall launching arrangements, from a position within the survival craft. Port #4 lifeboat remote control wire became tangled during lowering. Demonstrate lowering of port #4 lifeboat to the water to the satisfaction of attending marine inspector.

If the emergency generator prime mover is a diesel engine, there must be an audible alarm that sounds on high cooling water temperature. Emergency generator high cooling water temperature alarm unable to be tested satisfactorily in the presence of the Marine Inspector. 46 CFR 112.50-1(h)

Nonmetallic flexible hose must be in accordance with SAE J1942. FO Purifier hose has unapproved patch. 46 CFR 56.60-25(b)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The removable railings adjacent to the embarkation ladders for the rescue boats and MES are seized in place and render the ladders unable to be safely utilized in an emergency. 46 CFR 199.190(a)

ach machinery space ventilation system must have two controls to stop the ventilation, one of which may be the supply circuit breaker. The machinery space emergency shutdown was found to be inoperable from the Engine Room, Fire Control Station, and Bridge. 46 CFR 111.103-3

Each survival craft must be stowed So that neither the survival craft nor its stowage arrangements will interfere with the embarkation and operation of any other survival craft or rescue boat at any other launching station. Provide means of access to both port and starboard MES survival craft for means of embarkation. Current means of embarkation access is impeded by the rescue boat davits and operation.

Each lifeboat must be arranged to be boarded and launched directly from the stowed position. Provide access for all lifeboats to be boarded and launched from the stowed position. Current installation does not allow boarding or launching from the stowed position of any lifeboats.

Repairs and alterations to lifesaving appliances. Insofar as possible, each repair or alteration must be made with material, and tested in the manner, specified in this subchapter and applicable to the new construction requirements in subchapter Q of this chapter. #1 life boat shall be repaired IAW OEM spec to damage sustained to midforward port side fender and impact to chine glass. 46 CFR Subchapter Q 160.135

Repairs and alterations to lifesaving appliances. Insofar as possible, each repair or alteration must be made with material, and tested in the manner, specified in this subchapter and applicable to the new construction requirements in subchapter Q of this chapter. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the rules and regulations. Provide documentation that all new life saving appliances, testing and maintenance have been updated in NSE, IAW with all OEM requirements.

Engine design. All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Vessel reported loss of jacket water on STBD M/E while underway. Make repairs to the satisfaction of attending Marine Inspector surveyor after returning from current voyage and before any subsequent voyages.

Every nautical school ship subject to the regulations in this part shall be inspected annually, or oftener if necessary, by the Coast Guard to determine that the hull, boilers, machinery, equipment and appliances comply with the regulations in this part. Openings in the superstructure must be weathertight. Numerous weathertight doors in the vessel’s superstructure had hold-back devices installed, holding the weathertight doors open, exposing the interior of the superstructure to the surrounding weather.

Equipment or materials used for the construction of the vessel must conform to Navy or Coast Guard (or equivalent) specifications. Numerous equipment and materials located in engineroom are under repair or scheduled maintenance is being performed. Repair and reassemble all required equipment and thoroughly clean engineroom.

Each lifesaving appliance arrangement must meet subchapter W. Each side of each lifeboat must be marked in block letters and numbers with the name of the vessel and name of the port marked on the stern of the GOLDEN BEAR.

Illustrations and instructions in English must be posted in all cabins, in spaces occupied by persons other than crew, and conspicuously displayed at each muster station. Emergency instructions missing from numerous cabins and required locations throughout vessel.

Training materials must be onboard each vessel and must consist of a manual, written in easily understood terms and illustrated, detailing 1) boarding, launching, and clearing of survival craft; 2) method of launching within the survival craft; 3) procedure for releasing survival craft from launching appliances; 4) the use of all survival equipment; 5) use of the survival craft engine; 6) recovery of the survival craft (including stowage and securing). Vessel’s training materials do not cover the use (noted above) of the newly installed lifeboats and marine evacuation system.

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft’s launching carols that illustrate the purpose of controls, illustrate the procedures for operating the launching device, give relevant instructions or warnings, can be easily seen under emergency lighting conditions, and display symbols in accordance with IMO Resolution A.760(18). Lifesaving instructions are not posted in the vicinity of all newly installed primary lifesaving equipment.

Each oil tanker of 150 gross tons and above, ship of 400 gross tons and above other than an oil tanker, and manned fixed or floating drilling rig or other platform shall maintain an Oil Record Book Part I (Machinery Space Operations). Oil Record Book found missing masters signatures on consecutive pages and multiple apparent inaccuracies for required entries.

The master or person in charge of a ship to which this section applies shall ensure that a written record is maintained on the ship of each of the following garbage discharge or disposal operations: Discharge to a reception facility or to another ship. The vessel had no records of garbage discharge or disposal since 25 August 2021.

The plan must contain the following six sections… Steps to control a discharge. This section of the plan must contain a discussion of procedures to address the following scenarios: Drawings and ship-specific details: Supporting plans, drawings, and ship-specific details such as a layout of a general arrangement plan, midship section, lines or tables of offsets, and tank tables must be included with the plan. The plan contains written ship-specific details, however it does not have supporting drawings or general arrangement plans which include the required material.

This section of the VRP must include -The location, crew responsibilities, and procedures for use of shipboard equipment that might be carried to mitigate an oil discharge. The plan was found without reference to the location of the equipment or its procedures for use. The equipment was found unlabeled and without procedures for use.

For nontank vessels with an oil capacity of 250 barrels or greater -(2) A vessel owner or operator must comply with the vessel response plan training requirements of 33 CFR 155.1055. Vessel was unable to provide records for the preceding 3 years of training provided to persons having responsibilities under the plan.

A vessel owner or operator required by § 155.1035, § 155.1040, or § 155.5035 to have a response plan shall conduct exercise as necessary to ensure that the plan will function in an emergency. Both announced and unannounced exercises must be included. The vessel owner or operator shall ensure that adequate exercise records are maintained. Vessel was unable to provide records of any required exercises.

Records of daily hours of rest must be maintained onboard the vessel. Each affected person under paragraph (a) of this section must receive a copy of the records pertaining to them, which will be endorsed by the master or by a person authorized by the master and by the seafarer. Vessel ceased recording daily hours of rest in 2020, no current documentation or record of work or rest hours were found available for review.

The master, owner, operator, agent, or person in charge of a vessel bound for a port or place in the United States, unless specifically exempted by § 151.2015 of this subpart, must ensure the maintenance of written or digital records that include the information required to be reported by § 151.2060 of this subpart and the sediment information in paragraph (a)(1) of this section. Vessel's BWM records were not available for operations after 2020.

Every company should develop, implement and maintain a safety management system which includes instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant international and flag state legislation. Vessels Safety Management System implementation has not ensured compliance with relevant international and flag state legislation for the safe operation of ships and protections of the environment as evidenced by the discrepancies found with the Oil Record Book, Vessel Security Plan, Garbage Management, Non-Tank Vessel Response/Shipboard Oil Pollution Emergency Plans, STCW Rest Hour logs and Ballast Water Management documents. Vessel shall arrange for an external ISM Audit.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces shall be of "A" Class construction. Discovered missing overhead panel in electrical equipment room 1-81-1, disrupting the required structural fire protection boundary. 46 CFR 92.07-10(c)

The integrity of a stairtower shall be maintained by "A" Class doors at every level. Fire screen doors at 1-75-01 , leading to main machinery space, and adjacent to dry stores (lead to the 2 deck) found inoperable. 46 CFR 92.07-10(d)(4)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Dryer #4 exhaust hookup on 03 deck discovered not connected for service. 46 CFR 92.05-1

Pipe and duct laggings shall be of approved incombustible materials. Vent fan exhaust in 03-111-0 fan room discovered with poorly maintained gasket material. 46 CFR 92.07-10(d)(7)

On all systems test time delays, alarms, and ventilation shutdowns. Discovered three fire dampers not fully closing for the EDG room. 46 CFR 167.45-1(a)(9)

Each cable entrance in a damp or wet location must be made watertight by a terminal or stuffing tube. Discovered loss stuffing tubes on the navigation deck, IVO the paint room on the main deck, and throughout the vessel’s exterior decks. 46 CFR 167.40-1(a)(2) & 46 CFR 111.81-3(c) [Mid-J]

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Discovered seized shackles for davit launched IBAs on stbd side of vessel. 46 CFR 167.35-1 & 46 CFR 199.190(a)

Vent outlets from all tanks which may emit flammable or combustible vapors must be fitted with a single screen of corrosion-resistant wire. Discovered numerous vents with damaged and/or deteriorated flame screens throughout the vessel decks. 46 CFR 167.25-1(a)(1) & 46 CFR 56.50-85(a)(8)

Where pipes are carried through watertight bulkheads, decks or tank tops, or are carried through fire control bulkheads and decks, the integrity of the structure shall be maintained. Discovered deteriorated vents and temporary patches throughout the vessel. 46 CFR 167.25-1(a)(1) & 46 CFR 56.50-1(a)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Discovered loose support brackets for waterlights throughout the vessel that require tightening. 46 CFR 167.35-1 & 46 CFR 199.190(a)

Each vessel must have a sound-powered telephone system for communications between the wheelhouse and the bow lookout station. Discovered missing indicator knob and circuit plug for forward (2JV) sound-powered telephone system. 46 CFR 167.40-1(a)(2) & 46 CFR 113.30-5(g) [Mid-J]

Electric installations on vessels must ensure protection of crew, other persons, and the vessel from electrical hazards. Discovered numerous corroded wire supports on top navigation deck. 46 CFR 167.40-1(a)(2) & 46 CFR 111.01-1(b) [Mid-J]

Except as otherwise provided, the electrical equipment may be considered acceptable if it complies with the requirements of USCG electrical engineering requirements in Subchapter J (Electrical Engineering) of this chapter. Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Electrical outlet on aft mooring deck is hanging and unsecured. 46 CFR 167.40-1(a)(2) and 46 CFR 111.01-1(a)(2)

On all systems test ventilation shutdowns as stated in the system manufacturer's instruction manual. Carbon dioxide fire extinguishing system in paint locker is designed to close the damper upon activation. Damper is seized in open position and must be freed. 46 CFR 167.45-1(a)(9)

Each survival craft, except for inflatable craft, must be stripped, cleaned, and thoroughly inspected and repaired, as needed, at least once each year. Stbd enclosed lifeboat engine compartment needs to be cleaned and made ready for use. 46 CFR 167.35-1 and 46 CFR 199.190(f)(1)

Emergency instructions. Illustrations and instructions in English, and any other appropriate language as determined by the OCMI, must be posted in each passenger cabin and in spaces occupied by persons other than crew, and must be conspicuously displayed at each muster station. Stbd muster station instructions need replacing due to wear/deterioration. 46 CFR 167.35-1 and 46 CFR 199.80(c)

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. More than a dozen lifejackets are missing lights. 46 CFR 167.35-1 and 46 CFR 199.70(b)(4)(i)

On all systems test ventilation shutdowns as stated in the system manufacturer's instruction manual. Aft paint locker CO2 suppression system ventilation closure is activated after time delay. Piping shall be installed with a branch line ending with 2" dirt trap. 46 CFR 167.45-1(a)(9)

Quick-Closing Valve for the High L/O Suction for the Main Engine failed to close properly.

Hydro of number 2 boiler failed. retest required.

The galley vent on the 04 deck of the forward house found with a seized closure preventing closure in case of a fire. Closure access panel also found painted over and inaccessible for maintenance.

Watertight door, port side main deck adjacent to the fire control room, found with misadjusted dogs which were preventing the door from being made fully watertight as designed.

At least two lifebuoys on the vessel must be fitted with a self-activating smoke signal approved under approval series 160.157. Installed ring buoys on the port and stbd bridge wings found to be ‘deck buoy’ type in lieu of the required weighted ‘bridge buoy’ meeting the required Q-spec. 46CFR199.70(a)(iii)

Port side after house 01 deck laundry vent found clogged with lint and debris. Ensure duct is cleaned from the laundry room to the duct termination to reduce unnecessary fire risk.

F/O tank sounding tube at 3-144-4 found missing closure device.

A survey, either general or partial shall be made everytime an accident occurs which affects the safety of the vessel or whenever important repairs are made. Vessels EDG overspeed and shutdown; proper operation shall be demonstrated after repair.

Each master of a vessel that operates beyond the boundary line must ensure observance of the principals concerning watchkeeping set out in Regulation VIII/2 of the STCW convention and section A-VIII/2 of the STCW code. Vessel unable to provide detailed evidence of procedures for in port watchstanding that meet Regulation VIII/2 Part 5.

The company should establish procedures, plans, and instructions for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Vessel shall conduct internal audit focusing on procedures for in port watchstanding. Audit and corrective action shall be completed within three months.

An inspection of passenger and crew quarters, toilet and washing spaces.., etc. shall be made, in general, at least once each month. Vessel's crew is not conducting sanitary inspection of occupied rooms.

Use of non-approved fire detection systems may be acceptable if.. components are installed, tested, and maintained in accordance with an appropriate industry standard. Vessel staterooms have household type smoke detectors but crew does not inspect or test them after installation. Vessel shall verify operation of all installed detectors and provide maintenance plan.

Vessel's mast damaged as a result of an allision with a crane in Barbados. Vessel is to make permanent repairs to mast to the satisfaction of attending CG marine inspector from the nearest OCMI.

Onboard a seagoing vessel to which this part applies, no person may assign any person to perform shipboard duties and no person may perform these duties unless the person performing them has received the training and familiarization listed in subparagraphs (1) & (2). Provide training familiarization to crew identified in item 1.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Personnel listed in item 1 were found not to have valid medical certificates. Deficiency initally issued as Code 17 and downgraded to Code 50 on request for extension.

Every person employed or engaged on a vessel subject to 46U.S.C.8701 must produce an MMC or MMD with the appropriate endorsement to the master or person in charge (PIC), if appropriate, before signing shipping articles. Vessel found with multiple crew in the stewards/pursers/other unlicensed departments who do not possess MMD's. 46CFR12.701 Deficiency initally issued as a Code 17 and was downgraded to a Code 50 after request for extension.

Each operation described in paragraphs (d), (e), and (f) of this section shall be fully recorded without delay in the Oil Record Book so that all the entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the person or persons in charge of the operations concerned and each completed page shall be signed by the master or other person having charge of the ship. 33 CFR 151.25(h) On 1 May 2019, five weeks of Oil Record Book entries were missing & Master's signature missing since 6/25/2018. On 3 May 2019, ORB was rectified with missing entries but Master had signed all incomplete ORB pages despite not being aboard for the entirety of the period.

Operational Readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Several structural deficiencies were found with lifeboats 1 thru 4 and they are currently unable to demonstrate operational test of the boats. Demonstrate launching, operation, and recovery of lifeboats 1 thru 4. 46 CFR 199.190(a)

Lubricating oil system shall be provided with necessary coolers, heaters, filters, ect. for proper operation. SSDG #1 was found with a clogged lube oil overflow line which caused a backflow info the generator. Vessel shall clear the overflow line.

When new cable trays are installed, all MCT's shall be installed in accordance with good Marine practice. NVIC 9-97 change one shall be used as the reference to acomplish this.

Electrical cable penatrations of A-rated and B-rated bulkheads and decks must be made fire-tight using a USCG-approved fire-stop sealant or system (ref:46CFR72.05-10/NVIC 9-97 ch1). The VIGOR installed fire-stop material identified as MAGNAROCK is not CG approved. Removed all VIGOR-repaired and VIGOR replaced MAGNAROCK fire sealant from multi-cable transit (MCT's) penetrations in A and B rated bulkheads and decks and re-seal these MCT's with a current CG-approved fire stop sealant.

Vessel foudn with a portable gasoline tank on the aft port 01 deck rail. Tank was noted as missing drip pan for use in refilling smaller tanks and there was no portable fire extinguisher within 3 meters as required. 46CFR147.45(i)

Vessel found with gasoilne cans stored outside of paint lockers. Vessel shall ensure all gasoline cans or deck equipment containing gasoline are properly stowed in a paint locker. 46CFR147.45(b)

Hydraulic starting systems for emergency diesel generators must provide capacity for 6 starts. Vessel shall provide means to ensure starting system meets capacity.

Fire stations 42 and 44 are listed as having fog applicators on the fire control plan, however the nozzels on station are incompatable with the applicators. Vessel shall ensure compatable nozzles are placed on station.

The vessels periodic safety test procedures have been revised. Vessel shall provide approved plans for the periodic testing of all automated vital machinery from the Marine Safety Center and demonstrate tests to the satisfaction of the attending marine inspector.

During routine annual inspections, lifeboat technicians noted deterioration and conducted temporary repairs to the No. 1 & No. 3 davits. Permanent repairs shall be completed prior to completion of the next scheduled dry-dock.

Ships service diesel generators were identified as having leaking oil lubrication lines to the fuel oil pumps which are currently patched. Vessel shall replace leaking lines and clear all excess oils and rags.

Vessel found with fractured supports for the stbd main engine lube oil supply pipes (fwd most 2 inboard, fwd most 1 outboard). Vessel shall secure supply line supports.

Submit boiler automation procedures to MSC for revisions to the low fuel oil pressure test. 46CFR61.35

Several fire station valves have been found to be leaking, station # 18, 4, 13, 19, 10.

Engineering control room watertight door operating station operates differently than other stations. Provide operating instructions to include holding down the close button until door is fully closed by verification (due to noise level in engine room it is difficult to hear when door is closed.

Identified several fire screen doors that did not properly close all the way. Many corrected on the spot, 07 remain for deficiency follow up.

The watertight door indicator lights located in fire control room that indicate whether the watertight doors are open or closed are inadequate (burnt out or too dim to tell the difference), renew lights.

#17 and # 18 Fire station deck enclosures have wasted door hinges that prevents access to the station. Renew or remove enclosures.

Identified several locations of severe holed/wastage on fuel/oil on deck containment. Renew identified areas of containmnet.

Make new installed GMDSS available for inspection/testing for verification of proper operation.
