Electrical installations should be such as to minimize the risk of fire and explosion from flammable products [IGC 10.1.2]. Electrical equipment, cables, and wiring should not be installed in hazardous locations unless it conforms with the standards not inferior to those acceptable to the Organization [IGC 10.1.4, refers to IEC 60092:502]. Cables shall enter an explosion protected enclosure only by means of a gland or equivalent device cable of maintaining the integrity of the enclosure [IEC 7.5.1].
Multiple explosion proof electrical installations on deck were full of water and/our corroded from the inside of the fixture. Additionally, cable entrances into explosion proof fixtures missing necessary glands/packing thus exceeding the maximum experimental safe gap of the fixture. All of the identified fixtures are located in hazardous locations (gas-dangerous zones, as defined by IGC 93/1.3.17).

On ships carrying flammable or toxic products or both, a water-spray system for cooling, fire prevention and crew protection should be installed to cover: exposed cargo tank domes and any exposed parts of cargo tanks, cargo liquid and vapour discharge and loading manifolds, and boundaries of superstructures and deckhouses normally manned, cargo compressor rooms, store rooms containing high risk fore items and cargo control rooms, all facing the cargo area.
Multiple clogged nozzle heads and missing end caps along the entirety of the DWS system causing an approximately 50% degradation of the system and lack of coverage for all locations listed above. Additionally, a hole was found in the DWS piping over the #2 cargo tank top.

The system should be capable of delivering powder from at least two hand hose lines or combination monitor/hand hose lines to any part of the above-deck exposed cargo area including above-deck product piping [11.4.2].
PSCOs observed that half of the DCP hose station’s are cracked as the result of dry-rotting.

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall satisfy the following requirements: the doors shall be self-closing; hold back hooks not subject to central control station release are prohibited.
Nearly all self-closing fire doors within the superstructure had inoperable handles and/or faulty self-closing mechanisms. Additionally, four hold-backs were identified.
