Vessel reported that winch for the starboard side lifeboat, which is designated as its rescue boat, is malfunctioning. Port side lifeboat is now designated as rescue boat but cannot be recovered within the SOLAS designated time frame.

Personnel responsible for the operation of an incinerator installed in accorandce with the requirements of paragraph 6.1 of this regulation shall be trained to implement the guidance provided in the manufacturers operating manual as required by paragraph 7 of this regulation. PSCO observed the refractory in the main burn chamber near the doors opening was found to be crumbling and partially missing. Where this refractory was missing, a 02 by 02 inch hole was found leading from the first chamber wall into the air space separating the first and second insulation walls.
