A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. MI observed a small pinhole in the main sanitation pipe that allowed residual sewage in bilge. Operator is to supply a repair proposal to OCMI, if a temporary repair clamp is used the Operator shall ensure a permanent repair is proposed prior to next drydock. Permanent repair shall be made in sufficient time with approved proposal, final inspection of pipe shall be made to satisfaction of attending MI. Vessel shall not carry passengers until a temp repair has been completed iaw the OCMI approved repair proposal.

Vessel had new fire-fighting panel and equipment installed. The new fire panel was tested and operational. MI observed the old fire alarm in pilothouse still had power. Operator shall remove or secure and tag-out the old fire panel to satisfaction on attending MI.

(a) Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Vessel had moving blankets stuffed into the ventilation of the steering room and other compartments to keep the spaces warm so the pipes didn't freeze. When the blanket fell or was placed on the exhaust by a contractor from a previous installation, the hot dry exhaust pipe caught the blanket fire. Vessel is restricted prior to movement until a damage survey can be completed by the attending marine inspector. 46 CFR 116.405 (a) 60C
