A vessel must have one or more ring life buoys as follows: A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter. MI observed life buoy was in non-serviceable condition, informed o/o to replace life buoy. 46 CFR 180.70(a)(2)

All machinery must installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. A leak was observed at the fuel pump supplying the vessel’s generator. //46 CFR 182.01-5//

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. The circuit directory on the vessel’s aft switchboard did not accurately reflect the connected loads. //46 CFR 183.01-15//

A ship, except a ship of less than 26 feet in length, must have a Discharge of Oil Prohibited placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station. Vessel's Oil Pollution Placard was mounted at the operating station. //33 CFR 155.450(a)//

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12. A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12, except that it does not contain the burn preparations. It must be clearly marked on the first-aid kit that it does not include the burn preparations. Vessel's first aid kit did not have 10 sterile compression bandages for wounds in assorted sizes and was not clearly marked that it did not include the burn preparations. //46 CFR 184.710// //46 CFR 199.175(b)(10)//

All portable fire extinguishers installed on vessels shall be of an approved type. Two of the vessel’s extinguishers—located on the starboard side outside the pilothouse, and aft of the pilothouse—were not mounted in approved brackets. //46 CFR 181.30-5//

An adult life jacket must be provided for each person carried on board a vessel. The vessel was required to have 85 adult life jackets on board as specified in its COI but only 82 were available at the time of inspection. //46 CFR 180.71//

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Three electrical installations onboard were found using extension cord-type devices or multi-outlet adapters. One was located on the upper passenger deck and two were located in the galley. //46 CFR 183.01-15//

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information: date of the training; and general description of the training topics. Vessel had no record of crew training. //46 CFR 185.420//

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Drills shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: date of the drill and general description of the drill scenario. Vessel’s drill records did not include the general description of the drills. //46 CFR 185.520//

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Vessel’s portable watertight container was not marked. //46 CFR 185.615//

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] emergency exit sign was not labeled correctly. //46 CFR 185.606//

A vessel on a lakes, bays, and sounds route must carry three hand red flare distress signals and three hand orange smoke distress signals approved in accordance with 46 CFR 160.021 or other standard specified by the Commandant. Vessel was missing 02 hand orange smoke distress signals. 46 CFR 180.68(b)(1),(2). Vessel Limited to within 30 minutes of the dock.

Each lifeline on a ring life buoy must be of at least 60 feet in length. Lifeline attached to vessel's ring life buoy was less than 60 feet long. 46 CFR 180.70(c)(2)

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible. All pages of COI were not posted. 46 CFR 176.302

Each item of lifesaving equipment carried on board a vessel but not required under this part must be approved by the Commandant. Decorative ring life buoy is not approved lifesaving equipment. 46 CFR 180.25(a)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions taken in the event of fire, heavy weather, or man overboard conditions. Vessel's man overboard emergency instructions did not match new procedures. 46 CFR 185.510(a)

Electrical equipment on a vessel must be installed and maintained to protect the vessel from electrical hazards caused by or originating in electrical equipment. Vessel had uncapped wires below operating station in pilothouse. 46 CFR 183.200(b)

An adult life jacket must be provided for each person carried on board a vessel. Vessel was lacking 05 adult life jackets. Total persons restricted 80. 46CFR180.71(a)

All electrical installations shall be to the satisfaction of the [name withheld] generator battery had low voltage and generator would not start. 46CFR183.01-15

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Steering compartment's visual high water alarm was not illuminating. 46CFR182.530(a)

Portable fire extinguishers installation and location must be to the satisfaction of the [name withheld] deck accommodations space fire extinguisher was not mounted in a bracket. 46CFR181.520

Each vessel must be equipped with at least two operable portable battery operated lights. Vessel did not have any portable lights on board. 46CFR183.430

A vessel must carry either a first aid approved under approval series 160.041 or a kit with equivalent contents and instructions. Eyewash and Aspirin were expired. 46CFR184.710

Each ring life buoy with a floating water light must be secured around the body of the ring life buoy. Floating water light was not secured around the body of the life ring. 46CFR180.70(d)(2)

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46CFR164.018. Ring life buoy's retroreflective tape was worn and nonserviceable. 46CFR185.604(h)(i)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets on main deck were mixed together with adult and not properly labeled on container. 46CFR180.78(a)(5)

The number and identification of the items stowed inside and their sizes must be marked in clearly legible letters and numbers on each container for life jackets. Letters and numbers must be at east 2 inches high. Life jacket container on main deck reflected wrong number of adult life jackets. 46CFR185.604(f)

An adult life jacket must be provided for each person carried on board a vessel. One lifejacket was found unserviceable and was destroyed in the presence of the MI. 46CFR180.71(a)

Main distribution panels must be of the dead front type. Vessels center leg neutral panel had two missing breakers. 46CFR183.10-15

Each distrobution panel or switchboard must be totally enclosed and of the dead front type; center leg box (left) latch inop, replace to ensure dead front.

Battery must be free from exposure to water, splash, or spray and shifting; 2 of 3 bateries in engine space missing lid; replace to meet standard.

Each waterlight must meet approval standards; battery installed does not meet manufacturer requirements; replace to meet standard.

The number and identification of the items stowed inside and their sizes must be marked clearly and legibly; top deck storage capacity incorrect for adult lifejackets; adjust to reflect accordingly.

As appropriate for the intended voyange, a vessel must carry adequate and up to date charts; charts cannot be verified as current or up to date; furnish charts with printed date or demonstration of update.

An adult lifejacket must be provided for each person carried onboard a vessel; one adult lifejacket wasted; destroyed in presence of the OCMI.

Each life jacket must be marked with Type I retroreflective material; retro wasted; replaced wit approved tape.

Vessel must have a visual and audible alarm to indicate high water level in machinery space; alarm inop; replaced and retested successfully.

Vessel on Lakes, Bays, and Sounds route must have flares (3 red and 3 orange) to meet subchapter Q requirements; expired flares kept onboard in white emergency flares box; removed from vessel.

An adult life jacket must be provided for each person carried on board a vessel. Found 35 adult life jackets unserviceable. Limit number of passengers to be carried to 80. Replace life jackets to the satisfaction of the attending marine inspector.

A vessel must carry adequate and up to date charts of large enough scale to make safe navigation possible. Found vessels charts out of date. Replace with new chart for area of operation.

Distress signals shall be limited to a period of 3 years from the date of manufacture. Found expired distress flares for vessel. Replace with new flares prior to carrying passengers.

Found wire on top deck leading to a power supply not secured. Secure wire with appropriate metal banding every 24 inches to the satisfaction of the attending marine inspector.

Found leak in sewage piping inlet to MSD holding tank; repair.

Found 01 portable fire extinguisher missing bracket. Provide bracket.

All pipes and marine fittings must be suitable for the purpose intended. Vessels fire main is leaking at joint fitting. 46 CFR 181.15-5(c)

Vessel with installed toilet facilities must have a MSD in compliance with 33 CFR 159. MSD fill pipe is detaching from tank causing the MSD to leak. 46 CFR 184.702

Machinery and equipment installed on each vessel must be suitable for its operation and the purpose intended. Forward bilge high level alarm not permanently installed. 46 CFR 182.100

Flooding into the bow thruster space due to shaft seal failure. Make proper repairs to bow thruster system and align shaft seal. Conduct operational underway test in the presence of attending marine inspector. 46 CFR 176.840

Wiring to high level bilge alarm in bow thruster space is severed. Each machinery space must be fitted with an operational high level bilge alarm that gives a visual and audible alarm at the operating station. Make proper repairs. Test in the presence of attending marine inspector. 46 CFR 182.530(a)

Serious Marine Incident testing not arranged, company does not have Alcohol testing kits onboard to test within 2 hrs. Were never made aware of the ruling.

Supervisors have not been properly trained on how to identify potential reasonable suspicion personel.

Crewmembers need to receive proper training prior to assuming safety sensitive positions on Alcohol awareness and company policy.

Failure to submit Management Information Systems report. Company in business for 14 yrs has not submitted a report, owner admits not aware of this responsibility.
