Stairways and ladders shall be arranged to provide ready means of escape to the lifeboat and liferaft embarkation deck from all passenger and crew spaces and from spaces in which the crew is normally employed, other than machinery spaces. PSCO observed a moveable cart from the galley being stowed at the landing of a stairway used for means of escape. Cart obstructed the escape path. Cite: 74 SOLAS 81 Amend II-2/28.1 Code: 10c

The master or person in charge of a ship shall ensure that one or more placards are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. PSCO observed the garbage room was missing the MARPOL garbage placard. Cite: 33 CFR 151.59 Code: 10c

Exhaust ducts from galley ranges in which grease or fat is likely to accumulate shall be fitted with a fire damper located in the lower end of the duct. PSCO observed fire damper in the Veranda galley was not operational. Cite: 74 SOLAS 01 II-2/41-2.4.3 Code: 10c

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Such means shall consist of a door-latching mechanism incorporating a device that releases the latch upon the application of force in the direction of escape flow. PSCO observed thumb latches located on doors for the motion flow room as well as the gym. Doors used for means of escape did not have a door-latching mechanism that releases the latch upon application of force in the direction of escape flow. Vessel underwent a major extension project in 2021, therefore, areas located in MVZ 5 must adhere to most recent SOLAS requirements. Cite: 74 SOLAS 20 II-2/13.3.2.6.2 Code: 10c

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. PSCO observed fire door 340 failed to latch. Cite: 74 SOLAS 81 Amend II-2/30.2 Code: 10c

The speed of door closure shall, if necessary, be controlled so as to prevent undue danger to persons. PSCO observed the following: fire door 180, fire door 580, and fire screen door 640 closed in 5 seconds. Cite: 74 SOLAS 81 Amend II-2/30.4 Code: 10c

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/42 or II-1/43. The embarkation lighting forward of lifeboat #2 was not illuminating. 74SOLAS 97 III/11.4 10c

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed a light in the caudro 44 galley was leaking water from within. 74 SOLAS 04 II-1/40.1.3 16ac

The loadline ring, lines and letters shall be painted white or yellow on a dark background or in black on a light background. They shall also be permanently marked. The deckline was painted white on a white background and was not black on a light background. Load Line Convention 8 10c

Not less than one half of the total number of lifebuoys shall be provided with lifebuoy self-igniting lights, not less than two of these shall also be provided with lifebuoy self-activating smoke signals and be capable of quick release from the navigation bridge. PSCO observed the self-igniting light on deck 8 starboard side midship was inoperable. 74 SOLAS 97 III/7.1.3 10c

Each lifebuoy shall be marked in block capitals of the Roman alphabet with the name and port of registry of the ship. PSCO observed the ring lifebuoy located on deck 5 aft starboard side did not have the name or port of the ship registry marked. 74 SOLAS 97 III/7.1.4 10c

The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 to 26.4. FD(s) 622,640,A306 are required to maintain an "A" rating. PSCO observed the afor listed doors were not latching and maintaining the required boundary. 74 SOLAS 81 Amend II-2/26.1 10c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. PSCO observed the collar of the emergency generator space exhuast to be wasted. Deficiency Code: 02199 Action Code: 10c 74 SOLAS 14 1/11.a

The construction of all doors and door frames in "A"Class divisions shall provide resistance to fire as wells as flame. The PSCO found three (03) "A" class doors taht had the lock sets removed leaving an opeining that would pass flame and smoke. SOLAS (1981) II-2/30.2

Firefighting systems and appliances shall be kept in good working order and ready for immediate use. The PSCO found the key located in the break glass box for the emergency generator fixed firefighting release box was wasted and would not have operated the latching mechanism.

Except where otherwise stated the normal equipment of every lifeboat shall consist of food rations totaling not less than 10,000 KJ for each person the lifeboat is permitted to accommodate. The PSCO found a total of 14 food rations with the airtight seal being compromised, allowing are to enter inside found rations. SOLAS (1983) III/41.8

Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. various quick closing valves in the engine room are not operational. 74 SOLAS (97 Cons) Ch. II-2/Reg. 15.2.5 10c

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to the bulkhead. Escape door and door frame from engine room to deck 3 galley allows air passage. 74 SOLAS (81 Amend) CH. II-2/Reg 30.2 50a

Fire doors in main vertical zone bulkheads shall be of the self-closing type and be controlled so to prevent undue danger to persons. Fire Doors 236A, 640, 627 do not fully close. 74 SOLAS (81 Amend) Ch. II-2/Reg 30.4 50a

Oil filtering equipment shall be such as will ensure that any oily mixture discharged into the sea has an oil content not exceeding 15 ppm. Vessel was unable to demonstrate operational use of the OWS. MARPOL I/14 16a

No fuel oil tank shall be situated where spillage or leakage can constitute a hazard by falling on heated surfaces precautions shall be taken to prevent any oil that may escape under pressure from any pump, filter, or heater from coming into contact with heated surfaces. Lagging was missing from various parts in the oil purifier room. 74 SOLAS (97 Cons) Ch. II-2/Reg 15.4 16a

The normal equipment of every lifeboat shall have sufficient tools for manual adjustments to the engine and its accessories. PSCO found missing tools in all 4 lifeboats. LSA (17) IV/4.4.8.27, 16c

Fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: the structural fire protection, including fire resisting divisions and penetrations at openings and penetrations in the divisions. A fire door frame penetration in an A-class boundary was found. 74 SOLAS 2014 II-2/2.1.1.1

Additional aids for escape shall be provided as necessary to ensure accesibility, clear marking, and adequate design for emergency situations. A push button to open the door to the auxiliary machinery space was found to have no clear markings of its function to open the door and grant access.

Means of escape, including stairways and exits, shall be marked by lighting or photo luminescent strip indicators above the deck at all points of the escape route including angles and intersections. PSCO found missing segments of photo luminescent strip at the bar, aft, deck 7. 74 SOLAS 1997 II-2/28.1.10, 10c

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO found that FM-200 fixed-fire suppression system hose in emergency diesel generator space was in poor condition with fish hooks and a kink.

All doors shall be capable of remote and automatic release from a position at both sides of the door. Fire doors # 356 and machinery space changing room door found not to be closing. 74 SOLAS 1997 II-2/30.4.3, 10c

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. The following spaces were found to be used as storage containing combustible materials: 1. spa storage: flammable liquids stored in a storage locker that does not have fire detection system, suppression systems, or insulation. 2. Combustibles stored underneath stairways in Deck 2, MVZ1.

Found #3 cylindar on #2 MDE failed, fractured block. Engine is non functioning under class BV survey due by 8 DEC 2018.

Provide class report attesting to the proper operation of the steering gear. USCG inspectors were unable to test steering gear due to divers in the water conducting maintenance on the rudder.

Provide class report attesting to proper repair of leaks on pipe of fire pump # 2.

Provide class report attesting to proper repair of hydraulic leak forward of watertight door # 2.

Fixed Flexi fog nitrogen bottle selonoid switch not operable.

Vessel had no davit raft available for testing as per MOC policy LTR 4-98 by 15OCT05. Launching and recovery of lifeboats on the port side was not conducted due to bunker barge moored alongside. Vessel is required to demonstrate launching of davit liferaft and launching/recovery of lifeboats in presence of USCG Inspector by 15 OCT 05.

Prove proper operation of #1 boiler in regards to nozzle leakage and supply line solenoid valve opeation to determine source of leak.

Fuel Oil settling tank top adjacent to emergency fire pump was found with fuel oil all over the tank top resulting from a fewl leak from and unknown source.
