Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Main engine throttle failed during operational testing. Replace affected spring mechanism as temporary preventative measure prior to carrying passengers. Submit plan for permanent repair.

Each life jacket carried on a vessel on a Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. 04 adult and excess child lifejackets found without lights. Ensure all required lifejackets per COI, (52 adult, 6 child), have working lights and ensure any additional necessary child jackets, when child passenger count exceeds 6 individuals, also include working lights prior to carrying passengers.

A vessel must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station. Re-wiring of radar is in process. Ensure radar functions prior to carrying passengers.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel exiting "laid up" status with new operators maintaining preventative maintenance plan. Conduct 3rd party rigging survey and submit report thereafter.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. While monthly checks of portables are conducted, each extinguisher requires record of date and individual conducting checks. Informed operator to utilize tags attached to cylinders to record checks.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. 40B:C fire extinguisher for machinery space found "locked" into it's respective bracket with inspection tag. Fixed fire suppression system found inoperable with safety pin in way of activation lever. Ensure all fire protection and detection systems are in working order and ready for immediate operation and incorporate findings into internal inspection/check procedures.

Each U.S. flag vessel subject to the Great Lakes Agreement must have an inspection of the required radiotelephone installation at least once every 13 months. All emergency exits used for means of escape must be marked on both sides with "EMERGENCY EXIT, KEEP CLEAR". Forward emergency escape hatch not marked on the exterior. Mark emergency escape. Cite: 46 CFR 185.606

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Retroreflective on multiple life jackets was worn. Replace retroreflective tape. Cite: 46 CFR 185.604(h)

Where flexible fill pipe section is necessary, such hose must be secured at each end by clamps. It must be accessible and when flexible section is a nonconductor, the metallic sections of the fill pipe separated must be joined b a conductor for protection against generation of static charge when filling with fuel. Fuel fill piping was not bonded and easily viewed/accessible. Add bonding strap to piping and make piping accessible/ available for inspection. Bonding strap found unecessary as pipe found electrically continuous to the hull. Must make pipe accessible/available for inspection. Cite: 46 CFR 182.445(g)

Owner must provide satisfactory evidence of required servicing to the marine inspector. Fire equipment scheduled to be serviced after USCG inspection. Provide fire equipment servicing records to marine inspectors. Cite: 46 CFR 176.810(b)(iv)

Paint and flammable liquid lockers must be constructed of steel or equivalent material, or wholly lined with steel. Vessel had flammable liquids stowed in the lazarette spaces. Removed or properly stow flammable liquids. Cite: 46 CFR 177.405(d)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts, Coast Pilots [name withheld] navigation publications and charts were out of date. Obtain up-to-date publications and charts for intended voyage area. Cite: 46 CFR 184.420

All vessels must have navigation lights in compliance with International and Inland Navigation Rules. Stern light was found not operational due to wiring issue and was repaired during inspection. Cite: 46 CFR 183.420

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft non-ferrous metal straps with no sharp edges. Where passing through bulkheads, fuel lines shall be protected by close fitting ferrules or stuffing boxes. Renew and bond fill line for galley stove diesel tank, replace fuel line using USCG A1 approved hose, ensure all fittings are secured with a double hose clamp assembly, and protect the fuel line where it penetrates through the wood sub floor.
