All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment except for ballast, oil and water tanks which have acceptable means for filling and emptying independent of the bilge system. Vessel must prove system capability of pumping and draining water from each watertight compartment prior to movement.

The rescue boat launching and recovery arrangement does not comply with 46 CFR 199.160(e) and 199.160(g) because the installed 1,500-lb davit cannot recover the rescue boat with its full complement of persons and equipment; NCDOT must either upgrade the rescue boat/davit system to full regulatory compliance or obtain an approved exemption from CGD East in accordance with 46 CFR § 199.20(d) prior to return to service following the next scheduled drydock.

If port lights are fitted in spaces used alternatively for the carriage of cargo or passengers as permitted by § 171.116(d) of this chapter, dead covers must be fitted on the port lights. On both mens and womens heads, port light covers are missing dogs and or nuts to properly secure the port light covers. Conduct repairs to the satisfaction of attending CG Marine Inspector.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. MI received notification from the [name withheld] stating while conducting morning checks on the M/V KINNAKEET, black smoke was discovered in the E/R. Crew determined STBD MDE starter never disengaged after start-up. The STBD MDE battery bank then exploded. Conduct repairs IAW manufactures recommendation and provide proof of proper operation to the attending marine inspector.

MI received notification from the [name withheld] stating the M/V KINNAKEET discovered a water leak from the installed hard piping from the water pump to the blower on the port MDE. Conduct repairs IAW manufactures recommendation and provide proof of proper operation to the attending marine inspector.

The inspector shall examine all accessible parts of the vessel's equipment to be assured that it is in a satisfactory condition. The potable water pump is not making pressure due to faulty head tank(leaking). Make effective repairs.

Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. Missing insulation cooling outlet pipes for both main engines.

Suitable covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. Found exposed pulley on injection pump for MSD. Extend existing cover over the belts.

Each passenger vessel of less than 500 tons gross tonnage must carry at least one rescue boat approved under approval series 160.156 that is equipped as specified in table 199.175 of this part. During inspection of rescue boat that was replacing another vessels damaged rescue boat, fiberglass was cracked, wasted retro tape and extinguisher bracket.

Each port light located between the bulkhead deck and the next deck above the bulkhead deck must have an inside dead cover than can be secured watertight. Missing dogs on port lights in both restrooms.

WATERTIGHT DOORS INSTALLED BELOW SEVEN FEET ABOVE THE DEEPEST LOAD LINE ARE REQUIRED TO BE WATERTIGHT. DOOR TO THE AUXILLIARY MACHINERY SPACE HAS A SOFT GASKET INSTALLED MAKING THE DOOR WEATHERTIGHT. VSL MUST PROVIDE CORRECT GASKET TO ENSURE WATERTIGHT DOOR IN THAT LOCATION IS WATERTIGHT.46 CFR 170.255

EXHAUST SYSTEMS FOR INTERNAL COMBUSTION ENGINES MUST BE FREE OF ANY LEAKS TO PREVENT ATMOSPHERIC CHANGES WITHIN THE SPACE INSTALLED. STBD MDE HAS AN EXHAUST LEAK. VSL MUST PROVIDE EXHAUST SYSTEM FREE OF EXHAUST LEAKS.46 CFR 58.01-35

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Systems of this type include the following: Ship's service generating systems. Vessel captain notified SCC stating the crew discovered oil in the #1 generator's coolant system. Crew discovered the oil cooler was leaking. Provide findings and repair/replace faulty equipment with in-kind equipment. Ensure proper test trials are conducted. Notify Marine Inspector once repairs and test trials are completed.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. During re-inspection, MI discovered the forward seating arrangement was not properly secured. Secure seating IAW manufactures specifications and make available for inspection by attending MI.

Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use. During re-inspection, MI discovered the installed raft light on sponson was expired. Replace expired raft light with valid in-kind equipment and make available for inspection by attending MI.

An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Captain notified Command Center, the engineers discovered an oil cooler leak on the #1 SSDG. The vessel is authorized to conduct sea trials/load trials with no passengers/vehicles. Provide MI with repairs/attestation letter.

All valves and closing appliances which may be required to operate for damage control purposes shall be conspicuously marked with 1" block letters. Vsl installed bilge piping system has multiple valves which are not labeled. Vsl must provide labeling for all valves in the bilge piping system.46 CFR 78.47-38

This subpart prescribes regulations governing the design and construction of Marine Sanitation Devices. Standing water was found on men's bathroom deck. Repair leak on F/W inlet piping to toilet.

The inspection shall be such as to ensure that the vessel as regards the life savings appliances is in satisfactory condition and fit for the service for which is intended. The rescue boat was found inoperable during the drill. Provide cause of casualty and make effective repairs. Prove proper operation to the attending Marine Inspector.

Automatic operation of a safety control must be alarmed in the machinery spaces and at the cognizant remote control location. The high jacket water alarm for #1 SSDG is showing intermittent lights on EOS panel. Make effective repairs and prove proper operations.

The master reported a crewman discovered a pin hole leak in the port main diesel engine fuel line on the outboard side. Trouble shoot make repairs (replace in-kind) operationally test to satisfaction of the coast guard, prior to carriage of passengers or cargo.

Where extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. Vessel reported that the port fire pump is inoperable. Make permanent repairs to the port fire pump and prove proper operation.

Port side engine room closure device is missing securing hardware, preventing the closure from being isolated in the event of fire.

The steering hoses at motor/pump are damaged replace in kind as marked.

The port and std MDE’s air sep hoses are damaged (as marked) inspect entire system and replace damaged portions as necessary.

The pilot house steering failure alarms are inoperative. Prove proper operation to the satisfaction of the USCG.

The port MDE high pressure fuel oil lines are chaffed (damaged) replace lines marked by MI, inspect the remaining line and properly secure in brackets.

The engineers control station, has loose components (alarm panel) and gauges. Make properly secured.

The port and std SSDG’s have damaged supply and return fuel lines (as marked) which need replacement in kind.

The port and std SSDG’s have keel cooler piping, missing insulation on hot surfaces. Replace or repair in kind.

The engine room, battery ground isolator is damage ivo battery banks. Replaced in kind.

The port and std MDE’s have keel cooler piping, missing insulation on hot surfaces. Replace or repair in kind.

Davit wire rope is damaged. Replace in kind, conducted inspection of all components, and perform weight test to the satisfaction of USCG.

The std engine reduction gear to engine block was separating and bolts were loose.

The std SSDG high pressure fuel line is chaffed (damaged) replace in kind, inspected remaining and properly secure in brackets.

The MSD, Engine Room, Eng. Control station have damaged light fixtures. Replace in kind.

The engine room control station, breaker panel is missing breaker information. Properly identify all breakers.

The port side steering room, bilge has loose clamp. Properly install.

The engineering control station, has a loose speaker in overhead.

All batteries in engine room need to have maintenance inspection and corrosion cleaned from terminals. Remove all wing nuts and replace with nylock or lock nut to make connections properly tight.

The port and std fuel oil strainers are loose and missing hardware.

The port and starboard MDE’s shafting stuffing boxes were missing locking nuts.

The port and std MDE’s have damaged supply and return flexible fuel lines (as marked by MI) which need replacement in kind.

Re-install overhead console in pilot house with proper hardware. Locking tabs became loose and are not securing the VHF radio's and vessels horn controls.

The rescue boats gas tank was damaged.

Make repairs to the fire pump noted as leaking during operational testing this date from pump casing.

#1 MDE had wiring chaffing on top of fuel oil piping lines.

#2 MDE turbo piping near exhaust was missing lagging.

#1 SSDG turbo oil seal had small leak.

EDG and engine room switch board had breaker unmarked.

The general alarm back up batteries had excessive corrosion on the terminals.

EDG supply fuel line was cracked and damaged.

Provide proper notifications were made about the loss of SSDG #1 as a result of overspeed indiacated by engineers log (Dated 21Apr19 @ 1550) was sent to Coast Guard.

Port and stbd shafting were leaking excessively and were missing lock nuts.

Make proper repairs to #1 reduction gear oil leaks at the pump housing and clutch actuator.

The breaker rating and plaques on the emergency and main switch board are conflicting. Prove correct approved rating.

Port and Stbd MDE's have loose fuel oil piping brackets.

Improper made wire connections under console in engine room for the MDE's were identified.

Main switch board in engine room has wasted and damged wire insulation on wire run into the main panel outboard at the top.

MSD space has a damaged light cover.

MSD panel has a missing button on the front alarm panel.

MSD space has a damaged flammable locker that is not meeting flame spread as designed, due to corrosion.

Stbd generator keel cooler piping is wasted.

Adjust P&S shaft packing glands due to excessive leaking.

Install drip shield IWO STBD aft Battery charger

First Aid Kit is a DOT approved kit that doesnt meet 46CFR160.041 requirements. Missing meds.

Child Life jacket storage boxes behind pilot house need to be properly marked.

Unable to test EDG low lube oil and overspeed shutdown sensors on the engine. Provide means to prove proper operation of shutdown devices.

Submit repair proposal for removing water/moisture from throttle and control system. As per conversation with [name withheld].

STBD MDE control has moisture in the system not allowing the clutch to shift.

Throttle air control delay over 2 min. Cracked tubing to cluck air PSI. Tubing appears to be to short causing crack.

Crankshaft bolt broke and siezed oil pump, requiring a new engine to be installed.
