Discovered improperly ended wiring on the aft end of the deckhouse roof. Remove or properly terminate the exposed wires on the deckhouse roof.

Vessel had valid FCC bridge to bridge certificate, but did not have a current FCC safety certificate. Provide vessel with valid safety cert.

Vessel's port side fire hose was not NFPA rated. Provide port side fire station with NFPA rated fire hose.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Prior to carriage of passengers, drug and alcohol program shall be established and crewmembers shall be subjected to random chemical testing. 46 CFR 16.230(a)

The master shall conduct sufficient abandon ship, man overboard, and fire drills and give sufficient instructions to make sure that all crew members are familiar with their duties during such emergencies. Prior to carriage of passengers, drills must be conducted in presence of attending Marine Inspector. 46 CFR 185.520(a) 46 CFR 185.524(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The port sidelight and sternlight were inoperable. 46 CFR 183.420

Abandon ship, man overboard, and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Prior to carriage of passengers, vessel's log shall be maintained. 46 CFR 185.520(f) 46 CFR 185.524(d)

Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed annual inspection which was due by July 27, 2023.

An inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter. No later than the month and year on its servicing sticker affixed under 46 CFR 160.151–57(n). All (3) of the vessels IBAs are expired, IBAs must be serviced prior to movement. 46 CFR 185.730(a)(1)

Each hydrostatic release unit must be serviced, within 12 months of its manufacture and within 12 months of each subsequent servicing... Hydrostatic release units are expired, replace all hydrostatic units attached to IBAs prior to movement. 46 CFR 185.740(a)(1)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has expired drydock interval. 46 CFR 176.600(c)(1)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Wood is rotted on the upper starboard side of the lazarette. Provide temporary proposal, prior to permanent repair during drydock.

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction and a uniform load of 50 pounds per foot. The rail cap on the upper deck aft of the wheelhouse is partially rotted. Repair prior to credit drydock 2023.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for...guards and rails in hazardous places. The aluminum retainer bracket on the forward end of the accessibility ramp is cracked. Repair prior to credit drydock 2023.

A vessel must comply with the applicable requirements for any radio...including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission. Call sign on station license did not match the call sign on the FCC bridge-to-bridge certificate nor the FCC safety radiotelephony certificate.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Passenger marking must be changed from 63 to 60 passengers in accordance with the stability letter.

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke distress signals. 8 distress signals were present instead of the 12 required.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Provide receipt from date of purchase and mark expiration date on the hydrostatic release as two years from date of purchase.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead-end wires in the operating station toilet must be capped.

A vessel of more than 19.8 meters (65 feet) in length must have at least one fire axe located in or adjacent to the primary operating station. Fire axe was not present.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. The medical certificate for the master was expired. Vessel must be operated by a properly credential mariner.

A fixed fire extinguishing system must have a device provided to automatically shut down all engines that draw intake air from the protected space prior to the release of the extinguishing agent. Vessel's fixed CO2 system does not shut down the generators, which draw intake air from the machinery space.

A fixed fire extinguishing system must have a device provided to automatically shut down ventilation serving the protected space prior to release of the extingusihing agent into the space. Vessel does not have ventilation closures for the machinery space exhaust stack.

On a vessel of not more than 65 feet in length carrying more than 49 passengers, and on a vessel of more than 65 feet in length the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. Vessel's fire pump only outputs at a pressure of 50 psi.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel has been observed conducting structural repairs with notifying OCMI.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Exposed unused indicator light breaker ring connector behind main switchboard panel requires electrical tape to insulate, stripped terminal/loose studs and missing nuts on several breakers behind switchboard require repair/tightening, high level bilge alarm for FWD Collision Bulkhead requires label, and starboard main engine starter/alternators positive connection requires protective "red" insulation cover.

Upon application for any Certificate of Documentation, the owner of a vessel must designate a hailing port to be marked on the vessel. The hailing port must include the State in which it is located. The hailing port is currently listed on the COD as Colts Neck, NJ. The COD must reflect the vessels actual hailing port of Berkeley, CA within 30 days. 46CFR67.119

The owner of a vessel requesting initial inspection for certification shall submit for approval to the cognizant Officer in Charge, Marine Inspection (OCMI) for approval. The vessel has outstanding MSC Electrical Approvals as documented in MSC Letter Serial: E2-2000155 dated April 29, 2020. The breakers in electrical sub-panel breakers for the Tank Room and Pilot House must be replaced with UL Rated breakers. The breakers are currently on backorder, but must be installed prior to the carriage of passengers. 46CFR177.202(a)
