Resolution: Vessel's crew replaced with overhauled pump on #4 generator #4 f/o pump, pump plunger, barrel and O-rings were renewed. #5 generator f/o leaks were overhauled and no visible f/o leaks were noted at the time of re-inspection.

External high pressure fuel delivery lines between high pressure fuel pumps and fuel injectors shall be protected with jacketed piping system capable of containing fuel from high pressure lines failure. PSCO noted the double jackets were not securely installed with all bolts creating gaps on the ends of the lines serving all seven cylinders.
