All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Port Sidelight was not functioning, Restricted to daytime operations.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. The Certificate of Inspection was not posted on the vessel.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. The stability letter was not posted on the vessel.

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. The grounding wire connected from the metal fuel tank was not connected to the top of the metallic fuel pipe as required by the regulations.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include - (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; or (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Means to prevent the discharge of sewage was not met IAW with the regulations of 33 CFR 151.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Ring life buoy on board was not marked with retroreflective tape IAW the regulations.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Vessel had 03 life jackets that require new retroreflective material.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel had 03 different types of life jacket on board with a corresponding donning placard to only one type of life jacket.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Medicine in the first-aid kit was expired.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Vessel did not have required up-to-date charts on board.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Exposed wires were discovered in both the port and starboard hull compartments.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. The current Certificate of Documentation was not on board the vessel.

A vessel of 12 meters or more in length shall be provided with a whistle. The whistle shall comply with the specifications in Annex III to these Rules (33 CFR part 86). The vessel did not have an whistle on board IAW with the regulations

A vessel must have two independent means of controlling each propulsion engine. Control must be provided for the engine speed, direction of shaft rotation, and engine shutdown. (1) One of the means may be the ability to readily disconnect the remote engine control linkage to permit local operation. (2) A multiple engine vessel with independent remote propulsion control for each engine need not have a second means of controlling each engine. Vessel had new engines installed, which are not provided with independent means of control

In addition, the owner shall, prior to receiving a Certificate of Inspection, submit for approval to the cognizant OCMI, at least two copies of the following plans, manuals, analyses, and calculations that are applicable to the vessel as determined by the OCMI: Machinery installation, including but not limited to: Steering and steering control, including rudder details. Steering diagram was not provided to cognizant OCMI for approval prior to installation.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Alterations were made that affect the safety of the vessel, to include replacing the engines and steering console, without the approval of the cognizant OCMI. No drawings or plans were submitted prior to changes being made to the vessel.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel had 06 life jackets with the wrong vessel name marked on them.

The lights prescribed in these Rules shall have an intensity as specified in Annex I to these Rules, so as to be visible at the following minimum ranges; stern light 3 miles. Vessel's sternlight was nonoperational, vessel is restricted to daylight operations.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Vessel is utilizing blade fuses under the console.

A vessel more than 26 feet in length and less than 65 feet in length must have 1 fixed power bilge pump and 1 portable hand pump. Vessel is missing portable bilge hand pump.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel had 4 lifejackets needing replacing. Operator had 4 spares to replace those.

The owner or managing operator shall make a vessel available for drydock examinations as required by this section. Vessel owner failed to make vessel available for drydock by 28 Feb 2021. Vessel must complete a satisfactory drydock inspection prior to returning to service.

An adult life jacket must be provided for each person carried onboard a vessel. 11 Life jackets were taken out of service. Limit adult passengers to 40 until life jackets are replaced.
