ADVENTURE I

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. MI observed no instructions mounted. Rectify to the satisfaction of the attending MI.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. MI observed no maintenance records for portable fire extinguishers. Rectify to the satisfaction of the attending MI.

USCG Type A1, A2, B1, or B2 flexible hose may be accepted. MI noted weathered and worn sections of flexible fuel hose on starboard side of fuel tank from the shutoff valve to fuel filter and to priming bulb. Replace worn sections of fuel hose with approved type. 46 CFR 182.720(e)(3)(v)(c)

The required fire extinguisher maintenance (monthly checks) shall be logged. MI observed insufficient proof that maintenance was being performed. 46 CFR 176.810(b)(1)(iv)

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative, and shall be logged. MI observed insufficient proof that maintenance was being performed. 46 CFR 185.728(a),(c)

Vessel must undergo an annual inspection as specified in paragraph (b)(1) of this section. Vessel must undergo an annual inspection within 3 months before or after the each anniversary date. Vessels anniversary date was March 7th, 2025, and vessel is past 3 months from said date. Vessel shall not operate with passengers until an annual inspection has been complete. 46CFR176.500(b)

At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must: Have a breaking strength of at least 5 kilonewtons (1,124 pounds) MI Observed the lifeline on the life ring to be deteriorated. Vessel must replace lifeline prior to carrying passengers. 46 CFR 180.70(C)(5)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed a cracked weld on the rescue platform. Vessel must rectify prior to carrying passengers. 46 CFR 185.70

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. Vessel restricted to daylight operations until deficiency is rectified. 46 CFR 180.70(D)(3)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. MI Observed the incorrect batteries in the floating waterlight. Vessel restricted to daylight operations until deficiency is rectified. 46 CFR 180.70(D)(1)

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. MI observed a lack of a flame screen on the fuel vents. 46 CFR 182.15-35

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Sluice valves are not permitted in watertight bulkheads. The submersible electric bilge pump in the port side compartment number 3 was inoperable when tested. 46 CFR 182.500

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The child life jackets and adult life jackets were found stored together in a centerline portable container without separation or identifiable markings to distinguish between the two. 46 CFR 180.78

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC station authorization was found expired and is issued for the EPIRB and VHF. 46 CFR 184.502

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions; Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Minimize accidental personnel contact with energized parts; and prevent electrical ignition of flammable vapors. The operator installed an inverter without plan approval. It was also discovered that the primary overcurrent protection for the DC circuit was never installed on this vessel. The conductor providing primary power is an 8AWG marine grade conductor. 46 CFR 183.200

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Identified fuel valves are mounted to the railing of the vessel and not closest to the fuel tank supply. The valves were originally located on the tank but were moved. 46CFR 182.455(b)

Beckets. Each ring buoy shall be fitted with four beckets located at equidistant points about the body of the buoy. The beckets shall be passed around the body of the buoy with the free ends to the outside, and shall be securely cemented to the buoy with a suitable waterproof adhesive which is compatible with the unicellular plastic used in the buoy body. The ends of the beckets shall be turned under at least 1 inch, one end to go around the grab line, and the other to be laid flat against the first end. The beckets shall then be stitched to the grab line with not less than five hand stitches made with two parts of thread or machined stitched with not less than three stitches per inch. The ring buoys beckets (04) were fraying apart. Replace beckets or replace entire ring buoy. 46 CFR 160.050-4(d)

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The ring buoy markings for the vessel's name was missing multiple letters and the other letters were coming off making the name not legible. 46 CFR 185.604(b)

Each ring life buoy with a floating water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the ring life buoy. The lanyard was not secured around the body of the ring buoy. Captain corrected on the spot by attaching around the body of the ring buoy. 46 CFR 180.70(d)(2)

Each floating water light installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the water light to be quickly disconnected from the ring life buoy. The corrosion resistant clip was seized were it was hard hard to open and close. Repair or replace corrosion resistant clip. 46 CFR 180.70(3)

(e) Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. (e)(3)(i)Flexible hose may only be used at a pressure not to exceed the manufacturer's rating and must have a high resistance to saltwater, petroleum oils, and vibration. Nonmetallic fuel hose supply from fuel tank to emergency shut-off valve was cracked and dry-rotted. 46 CFR 182.720(e)(3)(i)

(a) Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB inspection log with required monthly tests completed was not on the vessel. Captain stated the test log was in the office. Completed functional test of EPIRB, asked the Captain to log accordingly in current test log and provide proof. 46 CFR 185.728(a)(c)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The disposable hydrostatic release unit did not have expiration date of two years after the date which the unit was installed. Captain marked the date accordingly on the hydrostatic release unit. 46 CFR 185.740(b)

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). A total of 100 aspirin tablets are required. First aid kit only had 36 aspirin tablets. 46 CFR 160.041-4(b)

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. Magnetic compass had been removed and Captain had been using compass on cellphone. Captain is reinstalling magnetic compass. 46 CFR 184.402(a)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of 4.06. Alcohol swabs were expired. 46 CFR 185.212

The vessel must be equipped with child size lifejackets equal to at least 10 percent of the maximum number of passengers permitted. All 5 child size lifejackets had unserviceable retro-reflective tape. Replace. Vessel is limited to daylight operations only. - 46CFR176.808(e)(1)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Communications Act Radiotelephony Certificates may be obtained from the Commission’s National Call Center—(888) 225–5322—or from its forms contractor. Vessel owner installed new radio installation that has not been inspected by an FCC-licensed technician. Inspect radio and provide valid Radiotelephony Certificate to USCG. - 47CFR80.903

The owner or operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics. Training and drills were not logged. Provide updated training record to USCG. - 46CFR185.420

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test shall be logged or otherwise documented, as applicable. Owner had no record of monthly EPIRB testing. Provide record to USCG. - 46CFR185.728(c)

A vessel shall be inspected for compliance with the standards required by this subchapter. Two areas of rot discovered in plywood deck port side midships with a hole in the deck. One area of rot discovered in plywood deck starboard side aft. Grind back and make permanent repairs. - 46CFR176.25-1

A submersible electric bilge pump may be used as a power bilge pump provided that a positive shutoff valve is installed at the hull penetration. All bilge discharge positive shutoff valves have flex hose between the hull and the valve. Remove flex hose and install valves at the hull penetrations. - 46CFR185.520(e)(7)

Wiring joints and spices shall be mechanically secure and made in junction boxes. Junction box covers missing on port and starboard side midship junction boxes below deck. Replace. - 46CFR183.05-50(b)

Light and power conductors for interior wiring shall be insulated. Multiple dead end wires discovered throughout vessel. Cap or remove. - 46CFR183.05-45(e)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel owner replaced embarkation "three step box" with a hinged and deck mounted aluminum embarkation ladder with railing. Update Beaching Plan dated 01/11/2017 to include changes and submit to OCMI for review. - 46CFR176.700(a)
