Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. It is not possible to confirm the open/closed position of the 03 installed shut off valves from the reach rods outside the space.

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Expired chemical testing kits were found onboard. Provide capability to test crewmembers within two hours.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is permanently mounted. Cable must be constructed in accordance with U.S. Navy Standards, Institute of Electrical and Electronic Engineers [name withheld] 45, or be a suitable commercial grade cable, with stranded conductors, Receptacle in galley area installed with residential grade solid conductor wire. Each life float and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Port upper life float was found with non adhered retro reflective material. Replace retro reflective marking with required type II material. 46CFR185.604(I)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Receptacle on aft bulkhead of engine room found without a cover. Provide a cover for receptacle. 46CFR183.01-5

Batteries shall be located as high above the bilge as practicable and secured against shifting with motion of the boat. They shall be accessible with not less than 10 inches head room. Batteries in stbd box not secured from shifting. Secure batteries. 46CFR183.05-20(b)

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Engine exhaust piping lagging was removed. Provide hot piping surface protection as required. 46CFR177.970

Gages to indicate cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. L/O gauges at operating station do not operate properly. Make repairs to gauges to allow for proper operation. 46CFR182.15-5(b)

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with §161.012. Each life jacket light must be securely attached to the front shoulder area of the life jacket. 22 life jackets onboard do not have lights. Provide lights for 22 life jackets.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Stbd main engine RPM gauge is not functioning. Provide functioning RPM gauge.

Each cargo ship equipped with a radiotelephone station in accordance with Part II of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 1500 watts peak envelope power, the operator must hold a marine radio operator permit or higher class license. No licensed radio operator onboard. Provide radio operators permit.
