EACH CRANE AND CRANE FOUNDATION ON A UNIT MUST BE DESIGNED IN ACCORDANCE WITH THE AMERICAN PRETOLEUM INSTITUTE SPECIFICATION FOR OFFSHORE CRANES; APR SPEC 2C. DURING INSPECTION, OBSERVED MULTIPLE CHAFED HIGH PRESSURE HYDRAULIC HOSES, INCLUDING ONE LOSE AND LEAKING NEAR THE FITTING, ON THE WEST CRANE. THIS OBSERVED HAZARD HAS POTENTIAL TO LEAD TO CRANE FAILURE, AND IN TURN, POTENTIALLY LEADING TO EITHER SERIOUS INJURY OR DEATH. REQUIRE A THIRD PARTY SURVEY OF BOTH WEST AND EAST CRANES, REPLACING/REPAIRING ANY HIGH PRESSURE HYDRAULIC HOSES AS NECESARY, AND PROVIDE THE USCG A THIRD PARTY REPORT, ATTESTING TO THE PROPER OPERATION OF BOTH EAST AND WEST CRANES IAW OEM STANDARDS; TO THE SATISFACTION OF THE MARINE INSPECTOR.

Each nonmetallic transfer hose must have no gouges, cuts or slashes that penetrate the first layer of hose reinforcement. Additionally, each hose assembly must have either fully threaded connections, ANSI B16.5 or ANSI B16.24 flanges, or ASTM F1122 quick disconnect couplings. Inspector noted diesel fuel hose below western crane with multiple cuts through outer jacket exposing wire reinforcing. Additionally hose was connected to automatic shutoff nozzle utilizing double hose clamps instead of threaded connection.
