Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Port and starboard running light wires are deteriorated.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] fluid piping for propulsion is missing support brackets.

Each hatch cover must be attached to the hatch frame or coaming by captive chains, hinges, or other devices to prevent its loss. Install the same on the two bow hatches.

The bilge suctions shall be fitted with suitable strainers. The bilge strainer in the stern space is worn/corroded. Replace the same.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections are not being completed and logged.

Except as allowed by paragraphs (b) and (c) of this section, before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, survival craft embarkation areas, and ring life buoys; (2) The stowage location(s) of life jackets; (3) Either: (i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or (ii) That passengers may contact a crew member for a demonstration as appropriate, prior to beginning an oceans or coastwise voyage; (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and (6) If the vessel is operating with reduced manning or equipment requirements in § 176.114 of this chapter. Passenger safety orientation was not being done at the beginning of each voyage.

A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. Public address system inoperable.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit contents were expired.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette; (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. No audible alarm at operating station.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Light near female head is missing guard.

Master has informed USCG that there are watertight integrity issues in isolated area of upper deck above aft bar overhead lights. Master has also informed that temp repairs have been made. Vessel shall make permanent repairs to affected area by the end of next Alternate Hull Examination.

Vessel has growth in water tank. Perform tank cleaning and flush prior to serving potable water via tank to passengers.

03-Develop a written drug and alcohol testing program.

-Develop a Employee assistance program which includes the training and education required in 46 CFR 16.401

Provide positive test result for [name withheld] as discussed.

Engine room fire station hose is too short at 23'. Replace with proper length.

Items in First aid kit expired, replace with new.

Life ring needs to be labeled with vessel name.

Vessel needs to be identified from outside of vessel. Name of vessel on Bow/Stern. Homport of vessel on Stern.

Conduct monthly inspection of vessel Fire extinguishers

Remove any combustible materials from from engine room.

Missing Belt guard on genset, re-install properly.

Missing hardware on 110v panel near main switchboard. install proper screws.

Remove excess water from batteries and inside boxes. Batteries appear to be overfilled.

Replace hoses on Steering Ram.

Provide proper DAPI program elements, training, random testing, records, pre-elployment and members enrolled.

Upper deck life ring needs new line. Replace line or entire ring.

No. 1 fire station piping has leak where piping meets main deck. Conduct proper repair. This fire station needs proper labeling.

Remove all plastic waste recepticales from vessel and replace with non-combustible material.

Provide proof the vessels PA system functions on all deck

The sanitary pump that delivers river water to toilets for flushing is inoperable. Repair/replace sanitary pump located in lazarette and test. Supply local OCMI with pump replacement nomenclature/details and test repair prior to using sanitary system/toilets.

Vessel is required to have 3 red flares and 3 orange smoke or 6 red flare day/night for river route service. Vessel only has 4 for inspection. Meet above requirement to satisfaction of Marine Inspector.

Ensure that hull gauging for annual condition assessment are completed to fulfill inspection conducted and reported on 15APR2021 to the satisfaction of Marine Inspector.

One (1) child life jacket not servicable. Restricted to 14 children until extra life jacket restored. COI requires 15 child life jacket (10%) of total passengers allowed.

Vessel hydraulic hoses for steering and propulsion on the exterior are deteriorated/rotted. Both rudder exterior hoses shall be replaced. Port and starboard propulsion sides have three (3) hoses each that shall be replaced. Hoses must meet SAE J-1942 and hose end fittings must meet SAE J-1945. Complete for next annual inspection.

Provide evidence that MIS data collection form is submitted to Commandant by either mail or internet as per 46 CFR 16.500(b) to satisfaction of Marine Inspector.

Replace broken paddle board on starboard paddle wheel to satisfaction of Marine Inspector. Believed damage from ice season, vessel not pulled out during. 46 CFR M176.25-5 "Old T"

Ensure vessel air receiver system is ready to operate vessel whistle. Prove whistle can operate to satisfaction of Marine Inspector and prior to carriage of passengers. 46 CFR 176.30-1 "Old T"

Provide list of employees in the drug and alcohal program

Replace Ring Buoy with approved type

Provide Local Notice to Mariners, Rules of the Road, Coast Pilot

Update emergency procedures. Procedures list lifefloats, no safety harness for crew during MOB.

Replace lifebuoys. grab line disintegrated when twisted, multiple tears in ring.
