Each abandon ship drill must include: Summoning passengers on a vessel on an overnight voyage to areas of refuge or embarkation stations and ensuring that they are made aware of hose the order to abandon ship will be given. Vessel did not have crew onboard to manage passenger evacuation and muster at time of inspection. Vessel to complete abandon ship drill with simulated passengers with marine inspectors prior to carrying passengers.

At each inspection for certification, the vessel must be fully prepared for inspection and testing of fire protection equipment, including fire-confining appliances such as doors. Vessel’s SFP was originally built to NVIC 11-83 / Subchapter H per MSC letter H2-1-610 of 15 July 1991, with subsequent SFP modifications reviewed to Subchapter K standards by MSC letter H2-0101204 of 25 April 2001. Both H [72.05-25 (b)(9)] and K [116.435(c)(6)] require doors in stairtowers and stairways to be self-closing. The fire door to STBD mid crew and the fire door to the forward crew berthing were incapable of closing and latching under their own power.

When it is determined by the Commandant that the gross register tonnage for a particular passenger vessel, which is attained by exemptions, reductions, or other devices in the basic gross tonnage formulation, will circumvent or be incompatible with the application of specific safety requirements in the passenger vessel regulations for a vessel of such physical size, the Commandant shall prescribe the regulations to be made applicable to such vessel. Vessel has an existing fire control plan that does not show the fire dampers used for fire containment and extinguishing on the vessel. Vessel is to update the fire control plan to reflect the location of all fire damper onboard. 46 CFR 175.05-15(b)

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel had FCC registration for an EPIRB unable to be located on the vessel. 47 CFR 80.1061(f)

Provision shall be made by means of plugs, covers, etc., to prevent the admission of air in the space protected. Vessel has a CO2 protected locker to store flammable materials that is rusted to the point of the locker being open to the environment, therefore compromising the boundary of the protected space. 46 CFR 181.20-35 (a)

Galley damper on STBD main deck of vessel is inaccessible from within the vessel and requires crew to climb the side of the vessel to fully close. Vessel is to install a suitable means of safely closing the damper from within the vessel outside the space the damper serves.

Each space accessible to passengers or used by the crew on a regular basis must have at least two means of escape. Starboard side door leading out from the category 9 electrical room underneath the wheelhouse is seized and cannot be opened from the inside or outside.

Fire Control Plan rev D dated 3/17/26 shows battery management room to be surrounded by A-class bulkheads. Provide a proposal to the OCMI within 30 days for how to bring the onboard installation into full regulatory compliance no later than 12/31/26.

Vessels described by 46 CFR 114.110(e) must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Temporary installation includes interconnected system with additional audible alarms in each accommodation space. Extended requirement for permanent installation to 30 NOVEMBER 2024.

The minimum clear opening of a door or passageway used as a means of escape must not be less than 810 millimeters (32 inches) in width, however, doors or passageways used solely by crew members must have a clear opening not less than 710 millimeters (28 inches). Passenger and crew cabin doors measure less than the minimum requirement for means of escape.

Each survival craft must be stowed in a position that is readily accessible to crew members for launching. MI found two 25-persons life rafts not stowed in cradle.

Each flexible cord and cable must meet the requirements in section 24.6.1 of IEEE 45-2002, Article 400 of NFPA NEC 2002, NEMA WC-3, NEMA WC-70, or UL 62 and no flexible cord may be used except (1) or (2). MI found temporary hot start wire for Jacket Water system.

Each generator arranged for parallel operation must have reverse-power or reverse-current trips. MI observed engineer unable to demonstrate reverse-power trip.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel suffered a machinery space fire. Provide gas free certification of CO2 released space, conduct survey of areas affected by fire and provide report of damage to attending USCG marine inspectors, prior to departing Ketchikan, AK

At each initial and subsequent inspection for certification, the marine inspector may require that a man overboard drill be held under simulated emergency conditions specified by inspector. Due to weather, vessel could not get underway to conduct drills. Vessel to conduct MOB drill in presence of marine inspector. 46CFR115.808

A door must have a latch with a minimum throw of 20 mm (0.75 inches). Latch and door handle on door to reefer stores was loose and unable to latch. Provide proof that handle is tightened and latches properly. 46CFR116.435(c)(3)

Tubular gauge glasses must be adequately protected from mechanical damage. Sightglasses installe in tank room did not have guards to protect from damage. Provide proof that gauge glasses on port and starboard fuel tanks are adequately protected. 46CFR119.440(a)(7)

Each lighting fixture must have a guard or be made of high strength material. Light in the laundry room did not have cover. provide proof that cover has been installed. 46CFR120.410(a)

Each floating waterlight must be approved IAW 161.010 in Subchapter Q. Waterlight on stern ring life buoy was inoperable. Provide proof that waterlight is operational. 46CFR117.70(d)(1)

MOB, Fire and Abandon ship drills were not conducted during inspection. Drills need to be conducted before getting underway with passengers. 46CFR115.800

DAPI proram was not inspected, since vessel is currently not in operation. Needs to be inspected before getting underway with passengers.46CFR16.230

x3 Fire door don't latch shut.

x2 Fire Door gasket are worn out. FD to main engine room, and FD to emergency generator room.

Fire damper in the galley was not operational

Need automation test report for x2 SSDG & x1 EDG.

46 CFR 182.05-1: Propulsion must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its design limitations. Port engine is inoperable due to casualty to turbo oil line.

Prove proper operation of Starboard main engine starting system to the CG Inspector attending.

REPAIR PORT SIDE M/E AND REDUCTION GEAR TO FULL CAPABILITY

Install missing control valves for the engineroom forward & aft ventilation fans.

Provide missing "Simple Diagram Instructions" for remote CO2 release located passageway in a way of the CO2 locker.

Install missing gate valve handle for the EDG keel cooler located by the through hull fitting aft lazarette bilge.

Install missing sewage aft deck discharge valve handle & install missing piping end cap/plug.

Determine locking pin found remove on the pre-engineered CO2 system in the EDG room as per vessel SMS or as per manufacturer's instructions. Correct accordingly & results found send copy to local OCMI.

Resolve vessel "Call Sign" as per FCC certificates. Vessel have two call signs: WDC4580 & WDF7009.

Pump in mid crew void, port side failed to run, prove operation of pump.

Hole located in piping on suction side of fire pump from wastage. Repair to original scantling.
