The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. "The objectives of the Code are to ensure safety at sea, prevention of human injury or loss of life and avoidance of damage to the environment, in particular, to the marine environment, and to property (ISM Code 1.2.1) Safety management objectives of the company should, inter alia: Provide for safe practices in ship operations and a safe working environment. Asses all identified risks to its ships, personnel and the environment and establish appropriate safeguards. After survey of the ship, no change shall be made in the machinery covered by the survey with sanction of the admnistration. PSCO observed a modified gasket on salt water cooling pipe, and several rubber patches throughout engine room on main diesel engine and generator #1.

The function of the detection system shall be periodically tested to the satisfaction of the administration by means of equipment producng hot air. PSCO observed master testing heat detector with a lighter due to no heat dection testing gun on board.

The oil record book shall be completed each occasion of bunkering of fuel or bulk lubricating oil. PSCO observed a bunkering evolution took place without proper logging in oil record book.

ANy discharge into the sea of oil or oily mixtures from ships of 400 gross ton and above shall be prohibited. Oil is leaking from stern tube of the vessel due to failed shaft seal. Provide proposal for repairs to the satisfaction of class and coast guard prior to departure.
