ON 12 SEPTEMBER 1994 THE VESSEL PACIFIC BLUE WAS OPERATED BY PACIFIC BLUE INC. WAS OPERATING IN HONOKOHAU HARBOR HAWAII EQUIPPED WITH INSTALLED TOILET FACILITIES WHICH WERE EQUIPPED WITH A TYPE III MARINE SANITATION DEVICE WHICH WAS RENDERED NON-OPERABLE BY OPEN OVERBOARD DISCHARGE VALVE.
