Except as noted in § 176.114 of this part, each vessel inspected and certificated under the provisions of this sub chapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel Certificate of Inspection (COI) has Sector Honolulu's Work Instruction 31 (WI-31) in the COI's Route Permitted and Conditions of Operation. The vessel does not have child-sized AED pads with the AED kit required by WI-31, as the vessel is certificated to carry children passengers on board. Obtain child-sized AED pads and store with AED kit on board the vessel, to the satisfaction of the attending marine inspector. 46 CFR 176.100(b)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel does not have iodine in first-aid kit. Obtain required iodine and store in vessel's first-aid kit, to the satisfaction of the attending marine inspector. 46 CFR 184.710

Each vessel must be marked as required by part 67, subpart I, of this chapter. The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and stern of the vessel. Vessel's name on the port bow is partially illegible. The "N" last "A" in "MAKANA" are partially missing, making the name illegible. Replace letters "N" and last "A" to the satisfaction of the attending marine inspector. 46 CFR 185.602(a) & 46 CFR 67.123(a)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Automotive Blade type fuses were installed for the autopilot and stereo systems. Correct electrical install by 1) Refer to manufacturer marine installation requirements for blade type fuses and install the proper marine type of blade fuse, or 2) Install cartridge type fuses per the above regulation to the satisfaction of the attending marine inspector. 46 CFR 183.380 (i)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12. The vessel did not have the required number of aspirin onboard, ISO 18813:2006 requires a minimum of 48 doses on board. Provide the minimum required number of aspirin to the satisfaction of the attending marine inspector. 46 CFR 184.70

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Engine compartment fixed gas extinguishing system closure devices missing. Repair of the engine compartment ventilation may proceed without Sector Honolulu OCMI approval, however required correction of material, design, equipment, construction, installation, etc. that are found not to be in compliance with Coast Guard requirements will be to the applicable regulatory requirements to the satisfaction of the cognizant OCMI prior to placing the vessel in service. 43 CFR 182.465(h)

Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Ring life buoy not approved type. Replace ring life buoy with an approved type to the satisfaction of the attending Marine Inspector. 46 CFR 180.70(b)(1)

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. The flame screens or flame arresters shall be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Fuel vent screens damaged, corroded, wasted wire mesh. Replace fuel vent screens to the satisfaction of the attending Marine Inspector. 46 CFR 182.20-35(d)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Provide abandon ship and man overboard drill log to the satisfaction of the attending Marine Inspector. 46 CFR 185.520(f)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Fire fighting drills and training log not on board the vessel to be reviewed by the attending Marine Inspector. Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel COD on aboard the vessel. Provide valid COD to the satisfaction of the attending Marine Inspector. 46 CFR 67.7

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and(3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The bilge high level alarm visual and audible alarm at the operating station for the head space and engine room are the same alarm. Install a separate bilge high level alarm for the head space and prove proper operation to the satisfaction of the attending Marine Inspector. 46 CFR 182.530(a)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. This subpart summarizes the responsibilities of credentialed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. The regulations in 49 CFR part 40 should be consulted to determine the specific procedures which must be established and utilized. Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. EPIRB test required log or otherwise documented not on board the vessel to be reviewed by the attending Marine Inspector. Tested EPIRB in the presence of the attending Marine Inspector. 46 CFR 185.728

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems. No EPIRB onboard. Install EPIRB and demonstrate proper operation the satisfaction of the attending Marine Inspector. Vessel is restricted to three miles from shore until the above is satisfied. 46 CFR 176.808(a)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. No painter onboard. Install painter for life float. 46 CFR 180.175 (d)

At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must: Be of at least 18.3 meters (60 feet) in length; Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Vessel lifeline not 60 ft in length and not identified as UV resistant or dark in color. Replace to the satisfaction of the Marine inspector. 46 CFR 180.70 (c)(2)(6)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The master of the vessel shall ensure that: (a) Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; and (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. No logbook onboard. Provide physical log. 46 CFR 185.728 (a)(c)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. No logbook onboard. Provide physical log. 46 CFR 176.810 (b)(1)(ii)& (iv)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. No logbook onboard. Provide physical log. 46 CFR 185.524 (d)(1)(2)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master unable to provide medical certificate. Provide medical certificate. 46 CFR 10.301 (a)

The employer shall provide an Employee Assistance Program for all crewmembers that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. No evidence of completed training for crewmembers or supervisor. Provide satisfactory evidence of completed Employee Assistance Program training. 46 CFR 16.401

Except as provided in paragraph (c) of this section, each vessel must, in each condition of loading and operation, comply with the applicable requirements of (1) Part 170 of this chapter, except subparts G and H; and (2) Part 171 of this chapter, subparts A and B. CG Marine Safety Center has determined that the original stability verifications for this series of vessels were based on faulty assumptions. Vessel shall conduct a new stability test. 46 CFR 178.310(a)

Except as provided in paragraph (c) of this section, each vessel must, in each condition of loading and operation, comply with the applicable requirements of (1) Part 170 of this chapter, except subparts G and H; and (2) Part 171 of this chapter, subparts A and B. Vessel is authorized to only operate on the following route: "PACIFIC OCEAN, STATE OF HAWAII, SOUTHERN AND WESTERN COASTS OF THE ISLAND OF OAHU BETWEEN A LINE EXTENDING SOUTH BY SOUTHWEST FROM KOKO HEAD TO A LINE EXTENDING SOUTHWEST FROM KAENA POINT, NOT MORE THAN 20 MILES FROM A HARBOR OF SAFE REFUGE, VESSEL MAY CARRY A MAXIMUM OF 22 PERSONS OF WHICH 20 MAY BE PASSENGERS." 46 CFR 178.310(a)
