AMAZING GRACE

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. The starboard spreader mount was found damaged on the aft mast. A rigging survey detailing damage and likely cause is required as well as a repair proposal. This is to be reviewed and approved by the attending marine inspector prior to repair work being conducted. Copies of the last inspections of the rigging and last survey shall also be submitted for review to the USCG. 46CFR177.330

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The lifeline fitted to the ring life buoy was found to be degraded due to UV light and shall be replaced.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The vessel's name was no longer visible on ring life buoy.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. No retroreflective material was present on the ring life buoy.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. None of the stowage locations life-jackets were marked.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Deck rails must include a top rail with the minimum height required by this section, and lower courses or equivalent protection as required by this section. The starboard side netting used as an equivalent to fixed rails did not have a means to secure the bottom aft corner to the bulwark.

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84). Found that the plastic lens of the Port/STBD side light and stern light were UV damage reducing the intensity as required by the regulations.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The ring buoy becket grab lines were found to be degraded due to UV sun damage.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters (5/16 inch); (5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The one (01) required ring buoy was not fitted with a lifeline.

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. In the crew cabin, port side, aft the 120 volt AC duplex outlet was missing its cover.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Found two (02) 14 gallon plastic gas tanks stored in the forward v-berthing, remove fuel tanks from the vessel.

All vessels shall be provided with not less than two avenues of escape from all general areas accessible where crew are normally employed. The escape route from the forward berthing was missing section of deck that made it difficult to access the overhead escape hatch. 46 CFR 177.15-1(a) Old T

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Both the port and starboard valves did not indicated the closed position.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. The following restrictions apply: (1) Flexible nonmetallic hose must be complete with factory-assembled end fittings requiring no further adjustment of the fittings on the hose, or field attachable type fittings may be used. Hose end fittings must comply with SAE J-1475 (incorporated by reference; see 46 CFR 175.600). Field attachable fittings must be installed following the manufacturer's recommended practice. If special equipment is required, such as crimping machines, it must be of the type and design specified by the manufacturer. If field attachable type fittings are used, each hose assembly must be individually hydrostatically tested to twice the maximum operating pressure of the system. Found no manufacture markings on non-metallic fuel hoses from fuel tank to the propulsion engine, could not verify hose met SAE J-1942 standards. All fuel hoses were double clamped at fittings from the fuel tank to the propulsion engine, not complying with SAE J-1475 hose end fitting standards.

Provide adequate scaffolding (or suitable substitute) so that the attending marine inspectors can visually inspect the bowsprit area.

Grind marked area at the port quarter in-way-of the vessel name and hailing port to remove bubbled paint and make available for inspection.

Remove vessel name bolted panel and make area behind it available for inspection.

Grind bubbled hull paint at port and starboard bow (marked areas).

Permanently repair area at coaming (at port side) where there is a temporary repair which does not exhibit proper bonding between the exiting and new concrete.

Remove the following carriage bolts and nuts and make available for inspection (counting forward to aft): a. Main mast (port side): 1st lag screw b. Main mast (stbd side): 2nd lag screw c. Fore mast (port side): 3rd lag screw d. Fore mast (stbd side): 3rd lag screw

Remove rudder post base plate where post penetrates the hull, and related bolts and make available for inspection.

Replace all three (03) headstays as its service life seems to have reached its end as indicated in the vessel preventative maintenance plan and rigging component’s inventory sheet.

Grind and remove paint on starboard beam to further examine possible areas of water intrusion and wood rot.

Make areas around the port side beam in-way-of the main mast chain plates and make available for inspection. There seems to be wood rot in the surrounding areas.

Provide a full 3rd party rigging survey report.

Remove all paint and corrosion on all exterior chain plates and make available for further inspection.

Remove all rotted and/or damaged wood at top (cap) of coaming, both port side and starboard side.

Clean metallic “Y” assembly connecting the vessel’s running gear below the dolphin striker and make available for inspection.

Remove damaged/rotted wooden channels supporting the chain plates and make available for inspection. Identify damaged/rotted areas and discuss the proposed repairs with the attending marine inspectors.

Clean areas where the inner headstay brackets meet the hull. There are signs of wastage. Make available for further inspection.

Grind/remove all visible corrosion on chain plates and make available for inspection.

Remove and clean angle bar supporting H-block (at anchor locker space).

At the bow and below the anchor locker, remove concrete repairs (showing signs of fractures) and make available for inspection.

Clean and expose concrete areas where angle bar penetrates the concrete hull. There are signs of concrete damage. Make available for inspection.

At forward space, port side storage area behind 2nd drawer, grind marked area and make available for inspection.

Make proper repairs to ensure that 2nd scuttle overhead hatch cover is weathertight.

At main cabin, remove all inner port side chain plates and make available for inspection. This is to include the chainplates behind the refrigerator.

Provide the most recent Sail and Rigging 3rd party survey. 46 CFR176.840

A hatch on a vessel that operates only on a partially protected waters must be weathertight. The forward hatch cover was found to have rotten areas near the hinges, where it connects with the trunk and is not making a proper weather tight seal, and is not secured by the hinges. Conduct repairs to both the trunk and hatch and make it weather tight.

The H block located forward of the watertight hatch cover was found to have both structural support members with missing wood, caused by chafing and friction above deck. Conduct permanent repairs as original design prior granting credit on next dry dock.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a means to reset devices used to automatically shut down engines. Current pre-engineered fire extinguishing system failed to prevent the engines from shutting off; once activated, engines couldn't be shut-off. Demonstrate proper operation of emergency override switch to the satisfaction of an attending USCG Marine Inspector.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must be installed in accordance with the manufacturer's instructions. The cable that operates the hook that breaks the ampule is being obstructed by the metal safety guard covering the ampule.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The existing visual alarm panel was removed after the installation of the new engine. Install a visual high water alarm at the operating station.

On a vessel of at least 26 feet, a visual and audible alarm must be provided at the operating station to indicate a high water level in the machinery space bilge. A submersible bilge pump # 1 float switch that activates both the pump and the alarm where found inoperable.

VHF-DSC Radio must be properly connected to a GPS receiver or have a VHF-DSC marine radio with integral GPS, and have entered a valid MMSI number. Current installed VHF radio was not displaying GPS coordinates and not having displayed the MMSI number.

The design, materials and construction of mast, post, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. Vessel owner must submit to the OCMI thefollowing documents and plan: 1. Sail and Rigging Preventive Maintenance Plan (PMP) that is amended and updated with the current changes in the rigging and recommended inspection regime. 2. Provide the most recent Sail and Rigging 3rd party survey.

Abandon Ship, man overboard, fire drills and training shall be logged or otherwise documented for review by the U.S. Coast Guard upon request. Provide a suitable drills and training log to be on board to document all drills and training conducted.

As appropriate for the intended voyage the vessel must carry adequate and up to date charts, tide and current tables. Provide current and updated nautical charts, tide tables and current tables to be on board the vessel for the master reference.

A vessel must be fixed with a gas fire extinguishing system capable of shutting down the engine. The extinguishing system does not shutdown the engine. IAW 46 CFR 181.400

A vessel must have at least three 2.5 gallon buckets with lanyards and words "FIRE BUCKET" stenciled in a contrasting color. Only one fire bucket was found, without a lanyard and labeling. IAW 46 CFR 181.600

The vessel must provide plans for re-powering of the vessel's engine to the [name withheld] have not been submitted. IAW 46 CFR 176.600

A vessel must have sidelights with an arc of the horizon of 112.5 degrees as fixed to show light from right ahead to 22.5 degrees abaft the beam. The sidelights do not have the proper arc of visibility. IAW 33 CFR 83.21(b)

A documented vessel's name must be visible on the exterior of the port and starboard side. The markings are not on the vessel. IAW 46 CFR 67.123

An adult lifejacket must be provided for each person carried on board a vessel. The vessel is missing two adult lifejackets. IAW 46 CFR 180.71

Placards containing instructions for donning and use of lifejackets must be posted in a space accessible to passengers and crew. The vessel does not have a placard posted onboard. IAW 46 CFR 185.516

Each ring life buoy must be marked with Type II retro reflective material IAW IMO Resolution A.658(16). The ring life buoy does not have retro reflective material. IAW 46 CFR 185.064(i)

A vessel must have a platform if the vessel cannot adequatley recover a person. On the port side, the attachment point must be moved aft to center the opening. The starboard side must have a similar arrangement. IAW 46 CFR 185.524

A vessel must ensure the required crew members are covered by a consortium, if it chooses to satisfy random testing in this manner. The vessel's 2020 consortium list was not available. IAW 46 CFR 16.230(g)

Each vessel must be equipped with at least two operable portable battery lights at the operating station and access to propulsion machinery space. The vessel does not have two portable lights. IAW 46 CFR 183.430

At each initial and subsequent inspection for certification of a vessel,the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. During COI exam inspector found several severely corroded turnbuckles, pins, terminals and toggles. Head-stay pin at Cranse Iron was pulling out. Turnbuckle on jib boom and starboard whisker on bowsprit are two-blocked. Port side aft shroud chain plate block was crushed and bolt holding shroud was pulled partially out of block. Starboard intermediate aft shroud dead-eye lanyard is worn. Possible crack on starboard chain plate for starboard bobstay. Several wooden blocks show signs of cracking and/or completely broken. Port side port light fasteners severely corroded. Inspection was stopped and a third party rigging survey will be required. During 3rd party survey a CG marine inspector shall be present. 46 CFR 176.802 (a) (3)

05109- A United States vessel which transports more than six passenger vessels for hire while such vessel is being navigated on any tidewater within the jurisdictions of the United States adjacent or contiguous to the open sea, or in the open sea to carry a radiotelephone installation properly connected to a GPS receiver or have a VHF-DSC marine radio with integral GPS. VHF radio was found not being connected to a GPS nor displaying any coordinates upon testing. 47 CFR 80.901; 80.905; 80.1101(c)(2).

13103-Gauges to indicate engine revolutions per minute (RPM) must be provided for all propulsion engines installed in the vessel. Prove proper operation of the RPM gauge located in the operating station, which was found to be inoperable.

04106-A vessel must be provided with an auxiliary means of steering as required in 46 CFR 182.620(a)(1). Vessel is not equipped with an auxiliary steering system.

Degraded Exhaust Manifold gasket not allowing compression buildup.

Provide a means of auxiliary steering.

Provide a means of resetting the propulsion engine at the operating station.

Provide a dive report to assess the condition of the hull following an accidental grounding.

Provide smoke detector in galley. UL listed

Provide waterlight for life ring.

Install 30x30 mesh screen on fuel tank vents.

Provide FCC safety equipment certificate.

Make vessel available for Hull Exam iaw COI.

Find local Drug & Alcohol testing facility.

Provide 3 red flares & 3 smoke distress signals.

Service Fire Extinguisher in Fwd compartment.

Replace life safety equipment to proper locations post vessel maintenance.
