All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI observed STBD side navigational light nonoperational. Repair STBD side navigational light to meet International and Inland Navigation Rules standard and to the satisfaction of the attending MI. Vessel restricted to daytime operations. Repair within two weeks. 46 CFR 183.420

A vessel of not more than 65 feet in length is not required to carry a rescue boat unless: (1) The vessel carries passengers on an open or partially enclosed deck; and (2) The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. MI observed crewmember using MOB recovery ladder without sufficient means to secure oneself to the vessel during MOB recovery operations. Configure MOB recovery crewmember's lifejacket with adequate means to secure oneself to the vessel. Provide satisfactory proof of implementation to the attending [name withheld] within two weeks. 46 CFR 180.210 (b) (1) & (2)

Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. MI observed 01 crew were unable to furnish valid medical certificates. 46 CFR 10.301(b)(2)

At each initial an subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition... Electrical Cable. MI observed an exposed electrical cable in the engine compartment. Remove or cap the exposed cable. old 46 CFR 179.15-15

A Certificate of Inspection is valid for 5 years for all other vessels. Upon administrative review, MI discovered vessel overdue for renewal inspection. Vessel shall not operate with passengers until completion of renewal inspection. 46 CFR 176.107 (b)

Prior to the carriage of passengers, provide a current Certificate of Documentation. (COD) Documentation expired 31JAN05. IAW 46CFR67.317, O/O failed to renew COD prior to 31JAN05.

Provide and commence utilizing a current training logbook IAW 46CFR185.420. The master had no log book.

Prior to the carriage of passengers, have your fixed fire extinguishing system serviced and inspected. Last inspection was FEB 2004, so it was due FEB 2005.

Prior to the carriage of passengers, prove proper operation of the fixed fire extinguishing system. System does not shut engine down when the power head is pulled off the bottle.

Prior to the carriage of passengers, repair high bilge level alarm in engine room. Alarm does not work.

Prior to the carriage of passengers, provide proof that all members are enrolled in your drug and alcohol program. No audit could be completed because company did not renew contract with ASAP last year

Prior to the carriage of passengers, conduct U/W drills. Crew left inspection to conduct urine tests for ASAP. This is the third annual, so U/W drill are required.
