The company and the ship shall comply with the requirements of the International Safety Management Code. In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause and appropriate action is taken. 8” wide temporary fiberglass patch found on 4” diameter auxiliary salt water cooling system outlet, supplying cooling water to critical main propulsion reduction gear, fuel coolers and reefers. Patch placed on cooling line in August 2021. No required reports made to company or class society. Parts received on board in December 2021. Crew has still not taken action to make permanent repairs for 4 months. Further, Main engine lube oil purifiers found out of service for 6 months. Main engine lube oil purifiers found on continuous survey machinery list which requires vessel to notify Class if an issue is found. Ship acknowledged no notification to Class – confirmed by Class (BV) representative onboard. Not making proper reports mitigates the purpose of the SMS which provides safe practices in ship operation and a safe working environment.
NOTE: All deficiencies are of a technical and operational nature which collectively provide objective evidence of a serious failure, or lack of effectiveness in implementation of the ISM Code. Sector San Diego requires as ISM audit to be conducted within the scope of maintenance and reporting of non-conformities.

Company should establish procedures, plans and instructions for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Vessel conducted non-routine 14 day cruise operation whereby garbage was generated beyond ship’s capacity to properly store it – garbage management plan does not address such a situation. Garbage found in steering gear room (a category 10 space where storage of combustible is prohibited) and above sprinkler heads in garbage room and adjacent garbage holding room – filled completely to the ceiling.

Observed the following fire doors not sealing correctly or missing gasket sealing materials: Door 291, 286 and A214. Door 594 (emergency escape trunk from machinery space) self-closing device is not operational and will not remain shut due to also having a broken latch. SOLAS 74am (04 Cons Ed) II-2/9.1.3

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. 1) Pinhole leak found on fire main topping up pump supply pipe. 2) 2” diameter circle bulkhead penetration in purifier room. 3) Two pinhole leaks found on steam condenser piping with jubilee patches. 4) Excessive water and hydraulic fluid found under port and starboard steering gear. 5) General alarm light on alarm column in purifier flat non-operational.
