All ships 500 gross tonnage and upward shall have an operational mode indicator to determine and display rudder angle. PSC Officer observed a 3 - 5 degree variance of the ruder angle indicators on all modes of steering.

15109 - Maintenance of the Ship and Equipment - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed that the crew was conducting pressure test on the Wuxi Xingtai Shipping Equipment immersion suits using Stearn's manufacturer instructions. The pressure test caused immersion suit seals to fail. 11119 - Immersion Suits - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed 05 immersion suits failed at the seam. 11119 - Immersion Suits - An immersion suit which has buoyancy and is designed to be worn without a lifejacket shall be provided with a releasable buoyant line or other means to secure it to a suit worn by another person in the water. 11118 - Lifejackets - A lifejacket shall be provided with a releasable buoyant line or other means to secure it to a lifejacket worn by another person in the water. PSCO observed all lifejackets onboard lacking a buddy line. - 74 SOLAS (20) III/7.2.1 & LSA Code 2.2.1.16 - 40ac

11131 - On Board Training and Instructions - The training manual shall contain instructions and information on the lifesaving appliances provided in the ship. The following shall be explained in detail: donning of lifejackets, immersion suits, and anti-exposure suits, as appropriate. PSCO observed the training manual has not been updated to reflect current life-saving equipment. - 74 SOLAS (20) III/35.3.1 - 40ac

After any survey of the ship under regulations 7, 8, 9 ,or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. PSCO observed a portable pneumatic pump used to transfer oil from a stuffing box L.O. Drain Tank to a stuffing box L.O. Circulation Tank to be circulated through a modified purification system and then to a L.O. sump Tank. This bypasses the original design of the tank to utilize the purifier room. A previous Chief Engineer implemented this procedure without class approval. 74 SOLAS(14)I/11(b)

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are inteded. Vessel experienced a propulsion irregularity as reported by the pilots.

While taking on ballast into cargo hold #3 oil sheen was observed on the water's surface. The vessel's crew witnessed oil coming from the bottom of the cargo hold in the vicinity of a manhole cover used to access the double bottom starboard diesel tank.
