PSCO observed that the crew was conducting pressure test on the Wuxi Xingtai Shipping Equipment immersion suits using Stearn's manufacturer instructions. The pressure test caused immersion suit seals to fail. PSCO observed a portable pneumatic pump used to transfer oil from a stuffing box L.O. Drain Tank to a stuffing box L.O. Circulation Tank to be circulated through a modified purification system and then to a L.O. sump Tank. This bypasses the original design of the tank to utilize the purifier room. A previous Chief Engineer implemented this procedure without class approval. 74 SOLAS(14)I/11(b)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. While taking on ballast into cargo hold #3 oil sheen was observed on the water's surface. The vessel's crew witnessed oil coming from the bottom of the cargo hold in the vicinity of a manhole cover used to access the double bottom starboard diesel tank. Soundings of the double bottom starboard diesel tank indicated an increase in volume and the presence of water.
