Port MDE has coolant leak. Effect permanent repairs and demonstrate proper operation prior to carriage of passengers.

Port MDE Exhaust flange is leaking causing water and gas to enter the engine room. Make repairs to the satisfaction of CG Marine Inspectors prior to the carriage of passengers. 46CFR182.430(b)

Both MDE's have numerous fuel leaks on injector lines. Determine source of the leaks, effect repairs and demonstrate proper operation prior to movement.

Active fuel leak sighted at integral fuel tank sight gauge lower fitting.

Make permanent repairs to port side #2 void spray rail (chine area) in accordance with submitted repair proposal. Repair shall be completed and approved prior to the carriage of passengers.

Owner representative reported vessel is taking on water in the #2 void through a spray rail (chine area) plating fracture. Make vessel available for CG Marine Inspector damage survey prior to the carriage of passengers. Any deviation from the approved repair plan must receive prior authorization.

RTV on port trim tab has failed and is ingressing water into the Laz.

Overboard discharge valve for fire/ bilge system is cracked from freezing and is discharging water into the machinery space.

RTV on wet exhaust was not given enough time to cure properly and is leaking into the machinery space.

Raw water pump on STBD main is leaking. Needs O ring replaced.

Found numerous wire nut under helm connecting 12v system wiring.

Found what appeared to be key chain rings attaching clips on life float painter lines.

Fuel tank wall/engineroom fwd bulkhead fractured causing fuel leak into the engineroom. Submit repair proposal and effect permanent repairs once approved to the satisfaction of the attending Marine Inspector.

Fuel tank void has multiple stress fractures that require permanent repairs. 3 sighted and marked. Submit repair proposal & effect repairs IAW approved plans and MI oversight.

Found possible fuel leak under the port main. Unclear of origin. Directed rep to clean and monitor after each run to find source of potential leak.

Excessive water found in the #2. Remove water and identify the source. Monitor the void and notify the CG if any changes.

Found cracked flange that secures the steering ram to the transom in the lazarette. Repair plan discussed welder. While grinding, welder melted the interceptor control module which left a 1 inch hole in the hull below the waterline.

There are multiple cracks on the focsle, stbd quarter and the forepeak and #2 void. Company plans to address them this offseason.

Found the starboard aft window cracked.

Old fuel line valve found, temporary cap placed on with agreement made that valve will be removed this off season.

Found numerous cracks on the focsle, found cracks in the forepeak at the base of the samson post, found numerous cracks in the #2 void.

Found 6 adult pfd's to be unserviceable due to UV damage.

Found exposed live 12v wiring behind console going to the FM radio.

Found the port steering ram bracket in need of a bushing to take up the slop in the pin hole which is being chaffed.

Found the cotter pin on the stbd rudder post broken.

Emergency instructions need to be posted in the pilot house.

Found a small fuel leak in the tank void. Investigate the source submit repair proposal to the CG.

Hydro jet engine is currently unserviceable. Prior to use, make necessary repairs to the satisfaction of the CG.

Found the vessel's air compressor that services the horn unservicable. Replace motor with UL approved replacement.

Found the exhaust lagging on the port MDE in need of tightening.

Sighted excessive oily/water mixture in the engine room bilge. Remove and clean bilges so crew safety (slip/trip/fall) & fire hazards are eliminated.

Sighted stbd quarter sustained damage from repeated hard docking or piviting. Effect repairs as agreed upon.

Vessels Port engine suffered a low voltage alarm and subsequently shut down. Make repairs and provide proof of proper operation of the port side propulsion.

Fuel Tank leak

Fractured welds sighted & marked on structural members in forepeak, #2 void and fuel tank compartments. Fractured welds sighted & marked on fuel tank support brackets. Effect permanent repairs to all fractured welds.

Hydro-jet engine exhaust support bracket in laz is missing mounting hardware, must be properly mounted & secured. Complete prior to use of hydro-jet engine.

Hydro-jet engine shaft removed. Hydro-jet engine shall not be run until properly repaired and operationally tested/demonstrated as witnessed by CG Marine Inspectors.

Make permanent repairs (as discussed and agreed upon) to #2 void portside support stanchion lower attachment where weld is fractured.

Vessel is required to have on board a valid FCC Safety certificate, as per 47 CFR 80.401. Vessel's FCC Safety certificate is expired. Obtain FCC safety certificate for vessel.

Vessel is required to have on board a Great Lakes Agreement certificate, as per 47 CFR 80.953. Vessel does not have a Great Lakes Agreement certificate. Obtain a Great Lakes Agreement certificate.

Each life float must be approved under approval series 160.027. The life float on the port side is deteriorating and must be replaced prior to next operating season to the satisfaction of the attending marine inspector.

A vessel must comply with the structural design requirements of one of the standards listed in 46 CFR 177.300. In the forepeak, the following welds were found fractured: on the port side, weld between 2nd transverse frame from the bow and the 7th longitudinal frame from the keel, and on the stbd side, the weld between the aft bulkhead and the 1st transverse frame. In the 2nd space from the bow, the following welds were found fractured: weld between the forward bulkhead and the 8th longitudinal frame from the keel on the stbd side, and the weld between the forward bulkhead and first transverse frame, where the deck meets the hull. In the fuel tank compartment, the following weld was found fractured: the weld between the 2nd longitudinal frame from the forward bulkhead and the 10th transverse frame from the keel on the starboard side. In the engine room, the welds between both the port and stbd engine exhausts and the stiffener on the aft bulkhead are fractured. In the lazarrete, the welds between the hull and the deck are fractured in the stbd quarter portion of the space. Repair these fractured welds to the satisfaction of the attending marine inspector, prior to operating next season.

A vessel must comply with the structural design requirements of one of the standards listed in 46 CFR 177.300. There is a quarter sized hole in the hull on the stbd quarter, where the deck meets the side shell plating. Conduct temporary repairs to ensure the space is watertight w/in 2 weeks and conduct permanent repair prior to next operating season to the satisfaction of the attending marine inspector.

The part of the exhaust system between the point in cooling water injection and the engine manifold must be effectively insulated. Stbd exhaust system is not effectively insulated. Make stbd exhaust system effectively insulated.

Make proper repairs/replacement to stbd red gear/MDE coupling.

Vessel internal structures are not in proper condition as required. Multiple minor fractured welds attaching side shell plating longitudinal stiffeners in way of forepeak and voids number 1 and 2. One fuel oil tank support bracket at port side forward corner is cracked. Complete permanent repairs to the satisfaction of the attending marine inspector prior to operating in 2019 and no later than 31MAY2019.

Remove all accessories and after market software/monitoring equipment that has been installed without approval from the Marine Safety Center/[name withheld] proper operation of the propulsion system controls and both main diesel engines.

Complete permanent repairs per approved repair proposal to the lazarette bulkhead fracture.

The vsl was operating with an audible bilge alarm silenced. IAW 46 CFR 119.530 bilge alarms must be audible and visual. Attest that the Captains have been trained to not operate w/ alarms in silenced mode.

Found bridge fire extinguisher in its bracked on the wheelhouse deck. Properly mount fire extinguisher on the bulkhead iaw 46 CFR 118.520.

Provide support and chafe protection for wiring in enginerom in way of battery banks and hydrojet prime mover.

Dead end wires in engineroom not marked and remain connected to source. removed during inspection.

2 panelboards in engineroom have openings where cables have been removed, no longer suitable for wet location, corrected during inspection.

Batteries move excessively within battery boxes in ME cranking batteries, corrected during inspection.

Wiring to P/S cranking motors and engine control harness not mounted or supported, subject to chafe and mechanical damage, corrected during inspection.

Control and instrumentation wiring on hydrojet engine unsupported and subject to chafe, corrected during inspection.

Steering machinery linkage/swivels not lubed, port quadrant excessively loose on stock. Corrected during inspection.

Painters excessively detiorated, unserviceable all life floatw, including tethers for multiple floats on a painter. Corrected during inspection.

Various sections of hot MDE exhaust piping exposed near pressurized oil lines, corrected during inspection.

Frame 3 in forepeak fractured IWO floor attachment, structural design to be modified and implemented as per proposal yet to be submitted to OCMI for approval.

Main girders have very minor fractures IWO attachment to main propulsion engine foundations, P/S, appently due to incomplete/incorrect welding at initial construction, corrected during inspection. See narrative entry, this activity.

Stbd MDE block blown out by piston.

Unable to verify is MIS was submitted.

Vessel was leaking fresh water from their cooling system into the exhaust. Causing a high temp alarm and the shutdown of one engine of two.
