During annual inspection, stability was questioned by attending marine inspectors prompting a new assessment of stability based on current cargo carriage configuration. Stability study and calculations completed by COLUMBIA-SENTINEL ENGINEERS, INC and provided to local OCMI on 16MAY2024. Submit calculations to the CG Marine Safety Center and obtain updated, approved stability booklet within 60 days. OPERATOR PROVIDED DOCUMENTATION TO MSC. DEFICIENCY TO REMAIN OPEN UNTIL STABILITY LETTER IS ISSUED BY MSC.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Crew could not provide proof of chemical testing program- provide consortium certificate. 46 CFR 16.230(a)

Each person shall have a separate berth and not more than one berth may be placed above another. Only three berths were observed onboard, prior to voyages longer than 12 hours, provide proof of 4 berths.46 CFR 92.20-20 OPERATOR IS IN PROCESS CLEARING REMAINING BERTHS ONBOARD, WILL REMAIN ON VOYAGES LESS THAN 12 HOURS.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. While onboard, flammable liquids were observed throughout the accommodation space. Provide a flammable liquid locker or paint locker for storage. 46 CFR 92.05-1

Each vessel must have the following.. Crew could not produce updated charts, light list, coast pilot, Rules of the Road, Sailing, Directions or Tide Tables. Provide updated items.33 CFR 164.33

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. Vessel's Radiotelephony certificate was expired- provide a valid certificate.47 CFR 80.903

Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. Crew could not find drill records- perform drills within 7 days and provide proof of a log. 46 CFR 199.180 (C)

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended and so that they can be kept in a clean, workable, and sanitary condition. Inspectors observed a visible gap in the frame around the weather-tight door aft of the galley. Repair the door to remove the visible gap or propose alternate plan addressing safety. 46 CFR 92.20-15. DEFICIENCY TO BE CORRECTED AFTER 2024 OPERATING SEASON. Changed to 705 deficiency code. Changed action code from 50c (30 days) to 705 deficiency code.

All pressure vessels must be provided with protective devices. The protective devices must be in accordance with the requirements of UG-125 through UG-136 of section VIII of the ASME Boiler and Pressure Vessel Code (incorporated by reference; see 46 CFR 54.01-1) except as modified in this subpart. The ship service air receiver in the engine room does not have a pressure relief valve. Properly install a proper sized pressure relief valve with a hand lift device set equal to or less than 225 psi. 46 CFR 54.15-5

Machinery guards. Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. The starboard ship service diesel engine is missing the machinery belt and pulley guards. Properly install all machinery guards on the same. 46 CFR 58.01-20

A vessel in maritime service must be licensed by the FCC. Vessel master was unable to show proof of a valid station license while onboard the vessel. 47 CFR 80.13

The vessel operator shall keep a written record available for inspection by the COTP or OCMI: The date and result of the most recent test and inspection of each item tested or inspected as required by 33 CFR 156.170. Transfer hoses were marked with a test date of 06/16/2022 but vessel operator was unable to provide a record of transfer hose test being conducted. Operator will need to provide a written record of the testing which was completed on 06/16/2022 or have the transfer hoses re-tested prior to being issued 3302(i) permit.

Each lifebuoy must be capable of being rapidly cast loose. Several items were being stowed on the starboard, aft, ring life buoy mount preventing the ring life buoy from being rapidly cast loose. Remove items to ensure ready availability of lifesaving appliance.

Fire pumps must be fitted with a pressure gage on the discharge side of the pumps. Properly repair or replace the fire main pressure gauge. 46 CFR 95.10-5(e)

Each section of firehose must be lined commercial firehose that conforms to Underwriters' Laboratories, Inc. Standard 19 or Federal Specification ZZ-H-451E. Hose that bears the label of Underwriters' Laboratories, Inc. as lined firehose is accepted as conforming to this requirement. Vessels fire hose is worn and damaged and leaking in several places. Renew the worn and damaged fire hose with 75' hose meeting Underwriters' Laboratories, Inc. Standard 19 or Federal Specification ZZ-H-451E. Hose that bears the label of Underwriters' Laboratories, Inc. as lined firehose is accepted as conforming to this requirement. 46 CFR 95.10-10

The material design, construction, workmanship, and arrangement of main propulsion machinery and each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards set by ABS steel vessel rules. The Main Diesel Engine lube oil pressure guages on the bridge were not operating. Repair guages to original condition to indicate lube oil pressure for the engines.

As appropriate for the intended voyage, all vessels except barges, vessels operating exclusively on rivers, and motorboats other than those certificated for ocean or coastwise route, must carry adequate and up to date; Charts, Coast Pilot [name withheld], Notice to Mariners, Tide tables, Current tables, and all other nautical publications necessary. Vessels charts and nautical publications are out of date. Update all charts and nautical publications. 46 CFR 97.05-5

Maintenance and inspections must be completed on vessel's life saving appliances and equipment and logged. There are no lifesaving maintenance records onboard the vessel. 46 CFR 199.190(b)

Each lifejacket and immersion suit must be marked in block capital letters with the name of the vessel and fitted with Type 1 retro-flective material approved under approval series 164.018. One adult PFD and one immersion suit is missing the vessels name and four PFD that were set aside needed new Type 1 retro-flective tape applied. 46 CFR 199.70(b)(3) and 46 CFR 199.03(b)(3).

Fixed CO2 fire systems. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Provide servicing report to attending CG Marine Inspector proving current satisfactory service report. 46 CFR 91.25-20(a)(1)(iv)

Damage discovered during an internal structural exam (ISE), the Officer In Charge, Marine Inspection, will require the owner/operator to submit a repair proposal, once approved will be required to make permanent repairs to the vessel. Found one cracked transverse frame on the stbd side shell, mid engine room just below where the side shell meets the main deck. Damage to be repaired at the vessels next dry-dock. 46 CFR 91.40-3(c)

There shall be at least two means of escape for where the crew may be normally employed. Properly label engine room escapes "EMERGENCY EXIT, KEEP CLEAR" in the engine room. Inside the engine room the two escapes are not marked. 46 CFR 92.10-5(a).

The Master shall ensure that all exposed cargo hatches and other openings in the hull of his or her vessel are closed, made properly watertight by the use of tarpaulins, gaskets or similar devices, and in all respects properly secured for sea before leaving protected waters. On the main cargo deck the forward bulkhead access covers port and starboard are loose and not made watertight. Properly repair so they can be made water tight. 46 CFR 97.15-20(a)(1)

The hull openings for the #1 stbd side void had a missing door. Reinstall watertight door and ensure all watertight hatches have all dawgs in place.

All three life rings stenciling of vessel name has worn away. Re-stencil life rings as required.

The life ring floating light battery was of the wrong type. Renew/replace battery with one of approved type.

Provide Day-shapes and Not-Under-Command lights for vessel.

Repair/renew magnetic compass light.

Ensure all fire extinguishers are stowed while vessel is underway.

Repair water supply line to stbd stearn tube seal.

Permanently install air compressor to include wiring.

Vessel’s sewage was aligned to go straight overboard with all valves open to the sea. Align sewage valves to holding tank when vessel is within 3 NM of land or in a no discharge zone.

Ventilators must be arranged to prevent flooding of the apparatus. Port side exhaust fan has water dripping into fan motor from exhaust vent. Locate origin of water, ensure exhaust fan is operating correctly.

The material design, construction, workmanship, and arrangement of main propulsion machinery and each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards set by ABS steel vessel rules. The engine hour meter for the port generator broke off its mount and is now held in place by a bungee cord. Repair/renew mounting bracket for engine hour meter.

Vessel is required to have two electric generating sources. Vessel’s starboard generator went out of service during dry dock period around mid-February. Generator was removed from vessel in March. Vessel must install second electric generating source in kind or alternatively submit plans to the USCG Marine Safety Center for new generator.

Exhaust piping from engines may not leak. Port generator exhaust pipe is leaking exhaust. Repair/renew exhaust pipe to original condition.

The fuel piping system must be in satisfactory operating condition and fit for the service it was intended. The port generator’s Racor filter base is leaking fuel and is being captured in a plastic bucket. Repair/renew filter housing to original condition. Remove and properly dispose of plastic fuel bucket.

Piping system installed on ships shall have fittings for safe and efficient operation of the vessel. The port exhaust pipe’s hangers were broken and is now held in place and supported with electrical wire. Renew/repair pipe hangers to properly support piping.

Re-weld or remove old tool box tray port aft corner of steering gear space prior to receiving dry dock credit

Remove deck plate and oily water from engine room bilge and make ready for inspection.

Update all nautical publications and charts.

Repair both generator fuel leaks.

Repair exhaust hanger for port generator.

Remove all dead ended wires.

Install covers on exposed terminal strip.

Repair/replace deck plate under anti-fatigue mats outboard of starboard and port engines.

Ensure all crew members are participating in emergency drills at least once a month. Provide record of drills for 90 days to marine inspector.

Install emergency lighting in accordance with cite.

Ensure all electrical distribution panels are labeled as required for intended purposes.

Provide written repair for failed time delay for fixed fire extinguishing system. 30 sec time delay is discharging at 22-24 seconds.

Excessive oil accumulation in the e/r bilge port side below port engine. Properly remove and dispose of oil. Clean and maintain bilges clear of oil.

Black water (sewage) was piped directly overboard, the overboard valve was found in the open position and Master confirmed black water going overboard at all times. Close overboard valve and stop dumping untreated black water within three miles of land.

The steering system is leaking hydraulic fluid into the steering compartment bilge. Secure all leaking fittings and clean bilge of all hydraulic oil.

Both Port and Starboard E/R exhaust fans screens were oily and presented a fire risk. Clean both screens.

Starboard E/R exhaust fan does not work. Repair/replace fan to return to original condition.

Properly plumb black water system to include a metallic valve at the sideshell immediately next to skin of vessel.

Vessel must stop all oil and fuel leaks on machinery in the engine room.

Vessel must replace all oil soaked lagging, remove all oil and fuel from bilge and clean both E/R fans.

Vessel must install lagging over all exhaust piping in the engine room. Main engines and generators are missing lagging.

Vessel's 3302(i) Permit has expired. Vessel cannot complete any bolk fuel deliveries until vessel completes and receives new permit. Vessel must prove servicing paper for all portable and semi-portable fire extinguishers.

Vessel must maintain records for all fuel tranfers onboard.

Vessel must provide flares as required by 46 CFR 25.25-19 as per vessel COI for uninspected passenger endorsement.

Vessel must have COD endorsement for fisheries before vessel can engage in any fishing operation.

Provide proof of satisfactory completion of training as required by 49 CFR 176.13 prior to carriage of explosives.

Between 25 November 2018 and 20 December 2018, vessel transited from Sitka, AK, to Auke Bay, AK, without an operating AIS unit, in contravention of 33 CFR 164.46 and an order from the COTP mandating compliance with AIS requirements. Restore AIS to operating condition prior to departure from Auke Bay.

Provide flame screens on fuel oil vents (2).

Provide draft marks per 46CFR97.40-10.

Prove operation of engine room alarms for low lube oil pressure and high temperature.

Provide fire ax and mount per fire control plan.

Repiar five stress cracks in engineroom companion way non-watertight bulkhead doorways. Cracks radiate outward from corners on port and starboard door openings from 2 inches - 6 inches. Cracks are to be die pentested, gouged and full penetration welded. To be complete NLT 14DEC00.

COmplete installation of fire pump/main piping IAW approved plans and proveproper operation at required pressure IAW 181.300.

Prove proper operation and required GPM output of both power bilge pumps IAW 182.520. Tests to be conducted from all spaces/voids.

make all penetrations between engineroom and galley deck vapor tight IAW 177.405.

Provide evidence that all piping and flex hoses in fuel lines and bilge/firemain system meet material requirements._x001C_ _x001C_

remove all dead end or disconnected wire/cable runs in engineroom.

Replace existing plastic wire hangers with acceptable type.

Prove prop operation of high level bilge alarm in engineroom IAW 182.530.

Install flame impingment bowls under all (4) fuel strainers.

Provide approved plans and install fixed F/E and detection system in engineroom.

Provide copy of MSC approval letter for current trim and stability booklet.

Stencil all PFD's with vessel name.

Conduct U/W operational test of vessel steering and propulsion.

Install deck rails IAW 92.25.
