On port side poop deck, the lower course is partly missing near the quarter.

For any vessel that is equipped with ballast water tanks and bound for ports or places in the United States and is not bound for the Great Lakes from outside the EEZ, submit the ballast water report no later than 6 hours after arrival at the port or place of destination, or prior to the departure from that port or place of destination, whichever is earlier. This vessel made two port calls in the United States; Sandusky, OH on 04 December 2024 and Superior, WI on 11 December 2024, and did not submit required ballast water reports until 20 December 2024 after inquiry from MSU Duluth following an arrival screening. 10c.

If a vessel's voyage is less than 96 hours, then the vessel must submit an NOA before departure but at least 24 hours before arriving at the port or place of destination. While the vessel was en route from Meldrum Bay, Canada to Marysville, MI a voyage of less than 96 hours, the vessel submitted their E-NOA to the National Vessel Movement Center on November 18, 2024 @0950 GMT (04:50 EST) with an ETA of November 18, 2024 @ 0800 EST. Additionally, on November 18, 2024, when Port State Control Examiners were able to screen the vessel's NOA, the vessel was seen to be already in U.S. Waters at the Port of Marysville, [name withheld] in Marysville, MI with only 3 hours notice. 33 CFR 160.212(4)(ii)

An inspection either general or partial depending on the circumstance shall be made whenever any important repairs or alterations are undertaken. Vessel reported that the # 3 port ballast tank was taking on water at or near the centerline bottom plate . Vessel shall ensure that appropriate repairs are completed and verified to the satisfaction of the vessel's classification society. 46 CFR 91.45-5(a) 17ac

A lifebuoy must have lifeline that is of breaking strength which is not less than 5 kilonewtons. Several lifelines on lifebouys are brittle and broke. Inspecte lines on all lifebouys and replace as necessary.

Electric installations on vessels must ensure protection of passenger, crew, other persons, and vessel from electrical hazards. Electrical panels 23 and 19 in the aft conveyor tunnel entry space are missing electrical blanks.

In general compliance with the standards established by the American Bureau of shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficenty of the vessel. Vessel ran aground while departing BNSF. Vessel is required to notify classification society and recieve a report validating the seaworthiness of both internal and external structure, and components of the vessel prior to depature.

The steering gear compartment must include handrails and either gratings or other non-slip surfaces to ensure a safe working environment if hydraulic fluid leaks. Vessel does not have non-slip surfaces in steering gear compartment.

Vessel recieved damage to Port side near forward peak and #1 ballast, Provide class report attesting to the condition and seaworthiness.

WHENEVER THERE IS A HAZARDOUS CONDITION EITHER ON BOARD A VESSEL OR CAUSED BY A VESSEL OR ITS OPERATIONS, THE OWNER, AGENT, MASTER, OPERATOR, OR PERSON IN CHARGE MUST IMMEDIATELY NOTIFY THE NEAREST COAST GUARD SECTOR OFFICE. VESSEL REPORTED THAT WHILE GOING ASTERN, THE VESSEL INSTEAD WENT FULL SPEED AHEAD AND THROTTLE CONTROL WAS LOST FROM BOTH THE WHEELHOUSE AND LOCALLY. PRIOR TO MOVEMENT, THE VESSEL SHALL PROVIDE EITHER A CLASS OR TECHNICIAN REPORT CONFIRMING REPAIRS HAVE BEEN MADE.
