Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Owner did not provide notification or plans of changes to vessel to include fuel tank and structure mods. MI's received plans from the shop doing the work after it had already started. Vessel is required to have a simplified stability test done prior to carrying passengers.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vsl was missing alcohol test equipment.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Wing nuts were used as battery connections. Owner to install proper connections.
