All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel does not have an anchor light as prescribed by Navigation Rule 30(b). Provide an anchor light as prescribed by this rule to the satisfaction of the attending marine inspector. 46 CFR 183.420

Each vessel certificated to operate on a limited coastwise route within three miles of land in warm water must either have on board an FCC Type Accepted Category 1 406 MHz EPIRB, installed to automatically float free and activate, and not be required to carry survival craft. Vessel EPIRB hydrostatic release unit expired. Vessel is restricted to Lakes, Bays, and Sounds route until the hydrostatic EPIRB hydrostatic release unit is renewed to the satisfaction of the attending marine inspector. 46 CFR 180.205(e)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The EPIRB is installed under the Bimini top and is not float free. Vessel is restricted to Lakes, Bays, and Sounds route until the EPIRB is installed to automatically float free to the satisfaction of the attending marine inspector. 46 CFR 180.64

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel exhibits an all-round white light mounted vertically on the port side flying bridge, which is not permitted under International Navigation Rule 23 (d)(iii). Arrange vessels navigation lights to meet prescribed lights under this rule to the satisfaction of the attending Marine Inspector. 46 CFR 183.420

Each floating electric water light shall meet the requirements of ANSI/UL 1196. Water light not in approved bracket. Mount water light in the approved bracket near the life ring buoy to the satisfaction of the attending marine inspector. 46 CFR 161.010-2

Each circuit breaker must meet UL 489 or other standard specified by the Commandant, and be of the manually reset type designed for: Inverse time delay; Instantaneous short circuit protection; and Switching duty if the breaker is used as a switch. Non UL 489 breakers found in tank space. Replace breaker with approved UL 489 breaker to the satisfaction of the attending marine inspector. 46 CFR 183.380(m)

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Fuel fill flexible piping is deteriorated and beyond service life. Replace flexible pipe with approved piping in accordance with reference cite for measurements, and bonding requirements to the satisfaction of the attending marine inspector. 46 CFR 182.20-30(d) Old "T"

A device must: Be designed to prevent the overboard discharge of treated or untreated sewage or any waste derived from sewage (Type III). The MSD system was not locked, nor handle removed. Remove handle in closed position or lock overboard discharge valve to prevent discharge of sewage into navigable waters to the satisfaction the attending marine inspector. 33 CFR 159.13(c)

When the flexible section is a nonconductor of electricity, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static spark when filling. The metallic deck fill section of the filling pipe bonding cable is not joined to the tank fill connection and are separated. Connect bonding cable as described above to correctly bond fuel filling pipe to prevent against static charge to the satisfaction of the attending marine inspector. 46 CFR 182.445(g)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Blade type fuses installed under the flying bridge console and for the oil change pump. Replace those fuses identified with proper UL listed marine use cartridge type fuse to the satisfaction of the attending marine inspector. 46 CFR 183.380(i)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kid is missing iodine and aspirin. Provide aspirin and iodine to the satisfaction of the attending marine inspector. 46 CFR 184.710

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. The engine space is not vapor tight. Seal the engine space openings to prevent fire extinguishing agent from escaping to the satisfaction of the attending marine inspector. 46 CFR 181.420(a)(4)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Bilge pump in forward space is not UL 1113 approved. Replace with a UL 1113 bilge pump to the satisfaction of the attending marine inspector. 46 CFR 182.520(e)(1)

Over current protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Newly installed battery charger missing fuse(s) for each positive conductor from the charger to the battery/battery bank. Install the required cartridge type fuse(s) or UL 489 circuit breaker(s) as desribed by 46 CFR 183.380(m) of the appropriate size for the model of battery charger installed as per the manufacturers installation instructions; to the satisfaction of the attending marine inspector. 46 CFR 183.380(k)(1)

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. Fuel tank vent pipe missing required flame screen. 1) Replace fuel tank vent; or 2) Provide a screen for the fuel tank vent as described above to the satisfaction of the attending marine inspector. 46 CFR 182.20-35(d) Old "T"

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Found dead end wires under operating console. Terminate electrical wiring properly to prevent injury to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure, and personnel injury. Starboard engine inboard forward and outboard aft engine mounts cushioned snubber are bottomed out indicating overloading. Adjust, repair, or replace engine mounts in accordance with [name withheld] manufacturer guidance to the satisfaction of the attending marine inspector. 46 CFR 182.01-05 Old “T”

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crew members and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Crew and supervisory training documentation could not be provided. Provided objective evidence of crew and supervisory training to the satisfaction of the attending marine inspector. 46 CFR 401(b)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacturer's instructions. Eight holes in the aft engine room bulkhead allow fixed fire extinguishing medium to communicate through bulkhead and into aft lazarette. Make vapor tight to ensure quantity of extinguisher is sufficient. 46 CFR 181.420 (a)(4)

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on engine side of both sea water intakes for both engines. Ensure double clamps are in correct position on both sides of sea water hoses. 46 CFR 182.410(d)(1)

The requirements for machinery and boilers for steam and electrically propelled vessels are contained in applicable regulations in Subchapter F of this chapter. Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Rotating belts for both engines are missing guards. Installed guards to the satisfaction of attending marine inspector. 46 CFR 182.05-5(b) 46 CFR 58.01-20

Except as provided in 80.13(c), stations must have an authorization granted by the Federal Communications Commission. Vessel's Radio Station Authorization has expired. Provide current an updated FCC license with new vessel name within two weeks. 47CFR80.405(a)

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in § 176.500(b)(1) and your Certificate of Inspection must be endorsed. Vessel shall not carry passengers until an renewal COI inspection have been satisfactorily completed.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel representative reported that vessel's Fireboy system failed inspection by servicing company. 46 CFR 176.810(b)(2)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The port and starboard engine room ventilation dampeners do not seal. Provide proper closure devices for port and starboard engine room vents.

Vessels pre-engineered fire extinguishing unit needs to be capable of shutting down the vessels propulsion engines. 46 CFR 181.420

Vessel needs to conduct satisfactory sea trials and perform the required drills. 46 CFR 176.808 & 46 CFR 176.802

Vessel needs to have visual indicators to show discharge of the pre-engineered fire extinguishing unit. 46 CFR 181.420 (b)(1)
