Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. There was no shutoff valve for the fuel tank. 46 CFR 182.3455(b)(4)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. The vessel only had (2) 2 ½ gallon fire buckets and none were labelled “FIRE BUCKET.” 46 CFR 181.610

Deck rails must be designed and constructed to withstand 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. The point and uniform loads do not need to be applied simultaneously. Equivalent protection in way of a chain spanning the entry areas of the vessel were not of adequate strength. 46 CFR 177.900
