The Notifier indicator panel located in the main deck portside was found to have its main cover door not closing properly as its box frame loose, the access key in the lock accessible to passengers, and marked as restricted area.

During the MOB drill, MI's observed free swinging doors at the bulwark rescue gate. When the vessel crew prepare to rescue a person in the water (PIW), a rescue ladder is attached to the bulwark and the bulwark rescue gate is opened. While the vessel is being maneuvered to recover a PIW the rescue gate doors swing freely. This presents a hazard to the crew and to the person being recovered. Bulwark rescue gate doors must have a means of latching them open to minimize potential injuries.
