Remove corrosion and rust from port fuel tank top, piping, and fittings. Make necessary repairs. This corrosion is directly below the hatch in above main deck compartment that is designated for the emergency fuel cut off valves.

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions. During DD inspection, the stbd generator was in process of gasket replacements and water pump maintenance. Rep was also unsure if port side generator was fully operational. Both generators shall be fully operational to the satisfaction of the attending marine inspector prior to carriage of passengers.46 CFR 183.200(a)

Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). Engine driven fire pump displayed excessive salt buildup and loose belts. Prove fire pump operates as designed prior to carrying passengers to the satisfaction of the marine inspector.46 CFR 181.120(a)

Pipes used for wet exhaust lines must be Schedule 80 or corrosion-resistant material and adequately protected from mechanical damage. Stbd FRP wet exhaust in machinery space displayed salt build up, wasted clamps, and signs of water leaks. Make necessary repairs to the satisfaction of the marine inspector.46 CFR 182.430(d)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel.(a) Wooden hull vessels: Lloyd's Yachts and Small Craft (incorporated by reference, see 46 CFR 175.600); Wood rot was displayed in engine room over head, below stbd wet exhaust, main deck overhead, and bow sprit. Make all wood repairs as original to the satisfaction the the attending marine inspector. 46 CFR 177.300(a) As discussed with vsl master [name withheld], on 05Sep2025 additional wood rot was discovered in overhead at the deck penetrations for the fuel piping above stbd fuel tank. This is added within deficiency #4 "make all wood repairs as original to the satisfaction of the attending marine inspector".

Fuel tanks must be adequately supported and braced to prevent movement. The supports and braces must be insulated from contact with the tank surfaces with a nonabrasive and nonabsorbent material. Port and Stbd Fuel tank straps displayed wastage, replace or make necessary repairs to the satisfaction of the marine inspector.182.440(b)(3)

Provisions must be made to prevent or mitigate the effect of galvanic corrosion due to the relative solution potentials of copper, aluminum, and alloys of copper and aluminum, which are used in conjunction with each other, steel, or other metals and their alloys. In fuel tank compartment, union at water pump displayed salt build up/wastage; remove, clean/adjust, or replace.182.730(b)(2)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Wood rot was present below port and stbd battery boxes in engine room. Provide repair proposal within 30 days. 46 CFR 177.100

Mechanically propelled S vessels which carry more than 49 passengers shall comply with Subpart F of this chapter. Rigid non-metallic piping materials are acceptable for use in bilge, ballast, and machinery connected piping systems on all other vessels. Electrical driven fire pump located in tank room forward void was fitted with PVC piping from the pump to the hydrant. 46 CFR 182.40-5

S The power fire pump shall be self-priming and of such size as to discharge an effective stream from a hose connected to the highest outlet. Power driven fire pump on starboard ME PTO Clutch driven produced inadequate water pressure which trickled from the nozzle. No ability for remote start. 46 CFR 181.10-1

On a vessel of not more than 65 feet in length carrying more than 49 passengers, and on a vessel of more than 65 feet in length, each hose must be lined commercial fire hose that conforms to UL 19 & 50 feet in length and 1.5 inches in diameter. Replace port side 50 ft. fire hose due to worn wasted and pin hole leaks, and damaged nozzle. 46 CFR 181.320

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried. Replace 06 child life jackets. 46 CFR 180.71

An adult life jacket must be provided for each person carried on board a vessel. Replace 01 adult life jacket. 46 CFR 180.71

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Replace retroreflective tape on ring life buoy. 46CFR185.604

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Replace light float lanyard on ring buoy. 46CFR180.70

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Post child life jacket donning placards for two different types of child life jackets. 46 CFR 185.516

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. Label lifejackets stowed on bridge with vessel name. 46CFR 185.604

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Install Machinery guard around FIREPUMP pulley on starboard engine 46CFR177.960

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. :Install metal retaining clips 24” spacing all engine room wiring 46CFR 183.340

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Remove oil rags from engine room bilge to prevent bilge suction from being clogged 46CFR176.830

A dry exhaust pipe must be provided with noncombustible hangers and blocks for support. Install support bracket on starboard engine exhaust. 46 CFR 182.430

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Label fuel shut off valves 46 CFR 185.608

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Starboard anchor chain shackle and swivel not fit for service. 46 CFR 184.300

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection.Label fire extinguisher location on nav bridge. 46 CFR 181.520

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Secure breaker panel cover and latch in way of engine room access. 46 CFR 183.200

An emergency instruction placard containing the following information will satisfy the requirements of fire- cut off air supply to fire - close items such as hatches, ports, doors, ventilators, and louvers, and shut off ventilation system.: Label port and starboard engine room and bridge vent closures. 46 CFR 185.512

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom.Replace all garbage cans with non combustible type Bridge, pax area, workshop. 46 CFR 177.405

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Provide log for drills and training. 46 CFR 185.520

Bilge check valve found inoperable in fwd compartment. Bilge suction is provided by engine driven fire pump. Valve replaced and is now operable.

13199 - Lazarette bilge high level alarm inoperable. Show proper operation of alarm.

99101 - Provide DOT approved alcohol testing device(s) on board.

05199 - Emergency broadcast placard faded and bears incorrect call sign. Replace with legible placard bearing new call sign.

Resolve all notes on MSC serial number #E2-1800637 letter dated 23Feb2018 and MSC serial number E2-1801073 letter dated 29MAR2018.

Revise electrical 1-line drawing to show proper size of installed conductor between generator and main AC panel.

Lifejacket lights were inoperable, vsl is limited to a limited coastwise route.

Painter was not permanantly attached to lifefloat body. Permanantly secure painter around lifefloat body.

Bottom portion of port lifefloat was waterlogged. Repair IAW manufacturer's instructions.

Batteries were expired in 03 waterlights (02 lifefloats and 01 waterlight). Replace batteries.

Lifeline on stbd lifefloat was replaced with a yellow synthetic line not of a dark color. Replace lifeline with approved type IAW regs.

Generator set windings were oil soaked. Clean windings.

Repair or replace damaged fuel line attached to stbd fuel tank in fuel tank compartment to satisfaction of CG Marine Inspector.

Replace flexible hose on port fuel tank and ensure double hose clamps installed to satisfaction of CG Marine Inspector.

Replace one leaking hydraulic hose on fwd anchor winch to satisfaction of CG Marine Inspector.

Vessel dry-dock date expires 31 Dec 2011, vessel must complete drydock prior to operating.

Voltage regulator not operating correctly after vessel struck by lightning while underway. Prove proper operation of Port side generator.

Side band radio OOC after being struck by lightning. Vessel limited to Coastwise operations until single side band is proven operational and ground wire hooked to new antenna.
